Diagnostic criteria for individual organ system failure are imprecise, a factor that adds a considerable amount of ambiguity to this area of clinical research. A 71-year-old African American woman presented with a past medical history significant for diabetes mellitus, coronary artery disease, hypertension and hyperlipidemia. She was being treated with atorvastatin and verapamil in addition to a few other medications. She complained of nausea, vomiting, myalgia, generalized weakness and dark urine. Initial evaluation revealed clinical icterus and mild mental confusion. Admission laboratory results showed features of multiple organ dysfunction. Over the course of four to five days the patient deteriorated to multiple organ failure resulting in death.

To determine limitations in survival and problems of single and multiple organ failure (SOF, MOF) following trauma in Bavaria, we reviewed 433 consecutive patients with multiple injuries treated at the Klinikum Grosshadern from 1978 through 1982. Most patients were young and were injured in traffic accidents. This report describes a case of life-threatening acute respiratory distress syndrome with multiple organ failure complicating probable scrub typhus. The appearance of primary HIV infection ranges from an asymptomatic presentation to a symptomatic illness resembling infectious mononucleosis.
