CNS muscarinic receptors and muscarinic receptor agonists in Alzheimer disease treatment.

This review explores the main aspects that form the basis of the cholinergic-oriented treatment of Alzheimer disease. Muscarinic acetylcholine receptor subtypes in the brain and periphery are discussed. It includes a new and updated overview of the involvement of muscarinic receptors in Alzheimer disease and the recent development of new and highly selective M1 muscarinic receptor agonists with disease-modifying potential. Activation of the M1 muscarinic receptor is a rational therapeutic strategy for the treatment of schizophrenia and Alzheimer disease, as this receptor plays a pivotal role i

Muscarinic M1 receptor agonist actions of muscarinic receptor agonists in rabbit vas deferens.

In the electrically field-stimulated rabbit vas deferens, muscarinic receptor agonists increase twitch-height by actions at postjunctional M2 receptors and decrease twitch-height by actions at prejunctional M1 receptors. In the present studies, in contrast to previous reports, muscarinic receptor agonists primarily decreased twitch-height, produced minimal increases in twitch-height, and, produced identical responses in both epididymal and prostatic tissue segments, thus permitting a more detailed investigation of the M1 receptor component of action of muscarinic receptor agonists in the rabbi

Development of covalent agonists for the muscarinic acetylcholine receptor.

Binding affinity and residence time of endogenous and synthetic receptor ligands are often poor at the tissue to be treated. Covalent ligands targeting G protein-coupled receptors (GPCRs) have gained significant interest because they unite the high specificity and selectivity with the prolonged activity resulting from irreversible or slowly reversible binding. Using the muscarinic acetylcholine receptor (mAChR) agonists iperoxo (5), PR15 (7), and carbachol (9) as lead compounds, we developed the covalent muscarinic receptor agonists 8 and 10 functionalized by a mustard-type reactive group. Muscarinic Acetylcholine Receptor Agonists as Novel Treatments for Schizophrenia.

Schizophrenia remains a challenging disease to treat effectively with current antipsychotic medications due to their limited efficacy across the entire spectrum of core symptoms as well as their often burdensome side-effect profiles and poor tolerability. An unmet need remains for novel, mechanistically unique, and better tolerated therapeutic agents for treating schizophrenia, especially those that treat not only positive symptoms but also the negative and cognitive symptoms of the disease. Almost 25 years ago, the muscarinic acetylcholine receptor (mAChR) agonist xanomeline was reported to r

Muscarinic receptor agonists and antagonists: effects on ocular function.

Muscarinic agonists act mainly via muscarinic M₃ cholinoceptors to cause contraction of the iris sphincter, ciliary muscle and trabecular meshwork as well as increase outflow facility of aqueous humour. In the iris dilator, the effect of muscarinic agonists is species dependent but is predominantly relaxation via muscarinic M₃ receptors. In the conjunctiva, muscarinic agonists stimulate goblet cell secretion which contributes to the protective tear film. Muscarinic M₂ and M₃ receptors appear mainly involved. In the lens muscarinic agonists act via muscarinic M₁ receptors to produce depolarizat

Muscarinic receptor agonists and antagonists: effects on keratinocyte functions.

The stratified epithelium enveloping the skin and lining the surfaces of oral and vaginal mucosa is comprised by keratinocytes that synthesize, secrete, degrade, and respond to acetylcholine via muscarinic and nicotinic receptors. The two pathways may compete or synergize with one another, so that net biologic effect represents the biologic sum of the effects of distinct acetylcholine receptors expressed by a keratinocyte at a particular stage of its development. Keratinocytes express a unique combination of muscarinic receptor subtypes at each stage of their development. Experimental results

Effects of muscarinic agonists in the guinea-pig prostate.

1. The contractile response to transmural stimulation of the guinea-pig prostate is largely due to activation of noradrenergic neurons but there is a small contribution from cholinergic neurons. Carbachol and acetylcholine have been reported to act via muscarinic M(1) cholinoceptors to facilitate contractions produced by neuronal stimulation of the tissue. This action of cholinomimetics was further investigated in isolated ventral lobes of the prostate. 2. Oxotremorine-M, bethanecol, pilocarpine, xanomeline and McN-A-343 produced facilitation of the response to transmural stimulation of the pr

Determination of dissociation constants and relative efficacies of some potent muscarinic agonists at postjunctional muscarinic receptors.

This study was undertaken to determine dissociation constants (KA and relative efficacies (er) of seven muscarinic agonists (methylfurtrethonium; dioxolane, oxathiolane, carbachol, muscarine, muscarone and oxotremorine) in three isolated tissues (guinea-pig ileum and atria and rat urinary bladder). The rank order of affinities (-log KA) of the various compounds varied depending on the tissue used. er values for the different agonists did not differ significantly from each other in any of the three tissues, except that the er of muscarine in the guinea-pig ileum was higher than those of the oth

Is muscarinic receptor agonist effective and tolerant for schizophrenia?

BACKGROUND: Several randomized clinical trials (RCTs) have recently examined the efficacy and tolerability of muscarinic receptor agonists in schizophrenia. However, whether therapeutics targeting muscarinic receptors improve symptom management and reduce side effects remains systemically unexplored. METHODS: Embase, PubMed, and Web of Science were searched from inception until Jan 9, 2025. Altogether, the efficacy and safety outcomes of four RCTs (397 individuals in the muscarinic receptor agonists group, and 374 in the placebo control group) were meta-analyzed. To compare scores of positive

Muscarinic agonists for schizophrenia: Bridging preclinical evidence and clinical enthusiasm.

Antipsychotic drugs targeting dopamine receptors have been used to treat schizophrenia for decades, primarily alleviating positive symptoms such as hallucinations and delusions. However, they provide limited benefit for cognitive and negative symptoms-major contributors to poor outcomes-and are frequently associated with significant side effects. The recent approval of KarXT, the first muscarinic-targeting antipsychotic drug, marks a potentially important shift in therapeutic strategy. Here, we review the mechanism of action of xanomeline (the centrally active component of KarXT) and related c

Effect of muscarinic receptor agonists on animal models of psychosis.

The purpose of this study was to determine the effects of muscarinic receptor agonists on models of psychosis in rats. A battery of three tests, catalepsy (haloperidol-induced) augmentation, amphetamine-induced stereotypy antagonism and conditioned avoidance response were used. Both muscarinic receptor agonists physostigmine and pilocarpine produced similar results. They dose-dependently augmented the haloperidol-induced catalepsy, inhibited amphetamine-induced stereotypy behavior and inhibited the conditioned avoidance response. Results demonstrate that muscarinic receptor agonists have actio

Differences in Ca2+ mobilization by muscarinic agonists in tracheal smooth muscle.

The muscarinic agonists acetylcholine (ACh) and McN-A-343 activate a homogeneous population of M3 receptors in canine tracheal smooth muscle. However, ACh is much more efficacious than McN-A-343 both at stimulating force development and active shortening and at antagonizing relaxation induced by isoproterenol. In other tissues, the same muscarinic receptor may be coupled to multiple subcellular pathways, but activate different pathways depending on the efficacy of the agonist. The present study investigated mechanisms of excitation-contraction coupling in canine tracheal smooth muscle by musca

5-Methyl-2-pyrrolidone analogues of oxotremorine as selective muscarinic agonists.

A series of N-(4-amino-2-butynyl)-5-methyl-2-pyrrolidones modified only in the amino group was synthesized. The compounds were agonists, partial agonists, and antagonists on the isolated guinea pig ileum. They had greater affinity and lower intrinsic efficacy at ileal muscarinic receptors than the identically modified N-(4-amino-2-butynyl)-2-pyrrolidones and N-(4-amino-2-butynyl)succinimides. Dissociation constants in the three series were correlated, suggesting that the compounds had similar mode of binding to muscarinic receptors. The 5-methyl-2-pyrrolidones were 10- to 20-fold less potent a

Recognition of cholinergic agonists by the muscarinic receptor. 1. Acetylcholine and other agonists with the NCCOCC backbone.

A theoretical model is used to deduce the pharmacologically active conformation of acetylcholine and other agonists interacting with the muscarinic receptor of the parasympathetic and central nervous systems. This is accomplished by replacing the usual dihedral angles tau 1 and tau 2, which define the conformations of cholinergic drugs, with two new geometric parameters more suitable for describing the muscarinic pharmacophore: a characteristic distance, [PQ], and a dihedral angle, PNOQ. Values for these parameters are determined by conformational analysis on semirigid muscarinic agonists usin

Agonist-induced desensitization and phosphorylation of m1-muscarinic receptors.

Pre-stimulation of Chinese hamster ovary (CHO) cells expressing the human m1-muscarinic receptor (CHO-m1 cells) with a maximally effective concentration of the muscarinic agonist methacholine resulted in desensitization of Ins(1,4,5)P3 accumulation, apparent as a approximately 4-fold shift in the agonist dose-response curve. Agonist-induced desensitization was rapid (detectable by 10 s) and concentration dependent (EC50=8.2+/-2.2 microM) and resulted in a complete loss of receptor reserve for the agonist-stimulated Ins(1,4, 5)P3 response. An investigation of the possible mechanisms involved in

The influence of the muscarinic agonist RS 86 on the cortisol system.

To test the assumption that activation of the hypothalamic-pituitary-adrenocortical (HPA) axis is mediated by cholinergic neurons, the cortisol response to the administration of the muscarinic agonist RS 86 was investigated in 12 healthy volunteers. In addition, prolactin and growth hormone secretion patterns were assessed. No significant increase in plasma cortisol or growth hormone was observed after the administration of 1.5 and 3.0 mg RS 86, respectively. However, there was a slight increase in prolactin serum concentration after 3.0 mg RS 86. As RS 86, when used in the same dose range, ca

Temperature-dependence and heterogeneity of muscarinic agonist and antagonist binding.

The binding parameters of muscarinic agonists and antagonists in rat central (brain, cerebellum and striatum) and peripheral (heart and lung) tissues were determined at 2 degrees and 37 degrees from competitive binding experiments with (3H)-quinuclidinylbenzylate (QNB). Muscarinic ligands binding affinities for cerebellum, heart and lung were tightly correlated. These tissues were also characterized by their low concentration of muscarinic receptors with high affinity for agonists. In contrast, brain and striatum contained a higher concentration of muscarinic receptors with a lower affinity fo

Muscarinic Cholinergic Receptor Agonist and Peripheral Antagonist for Schizophrenia.

BACKGROUND: The muscarinic receptor agonist xanomeline has antipsychotic properties and is devoid of dopamine receptor-blocking activity but causes cholinergic adverse events. Trospium is a peripherally restricted muscarinic receptor antagonist that reduces peripheral cholinergic effects of xanomeline. The efficacy and safety of combined xanomeline and trospium in patients with schizophrenia are unknown. METHODS: In this double-blind, phase 2 trial, we randomly assigned patients with schizophrenia in a 1:1 ratio to receive twice-daily xanomeline-trospium (increased to a maximum of 125 mg of x

Clozapine is a potent and selective muscarinic M4 receptor agonist.

Clozapine was studied in functional assays at human muscarinic M1-M5 receptors expressed in Chinese hamster ovary cells. Clozapine was a full agonist at the muscarinic M4 receptor (EC50 = 11 nM), producing inhibition of forskolin-stimulated cAMP accumulation. In contrast, clozapine potently antagonized agonist-induced responses at the other four muscarinic receptor subtypes. Selective stimulation of M4 receptors may, in part, explain the hypersalivation observed clinically with clozapine. Moreover, the unique overall muscarinic profile of clozapine may contribute to its atypical antipsychotic
