Sarcoidosis involving the musculoskeletal system.

Although sarcoidosis can affect virtually any organ, musculoskeletal involvement is infrequent. Early in the course of the disease, acute polyarthritis may be observed in up to 40% of patients, but this is often self-limited. Chronic or recurrent sarcoid arthritis is rare, affecting only 1 to 4% of patients. Asymptomatic involvement of muscles has been reported in 25 to 75% of patients in small series, but symptomatic involvement of muscles is rare. Although prospective studies assessing the prevalence of muscle involvement in sarcoidosis have not been done, several large retrospective studies

[Sonography of the musculoskeletal system].

Beside standard radiographs, bone scintigraphy and MR imaging, ultrasonography plays an important role in assessing abnormalities of the musculoskeletal system. Ultrasonography is widely available, inexpensive and provides high-contrast resolution; however, interobserver variability is a major problem. Ultrasonography should only be employed by experienced examiners and for proven indications in order to prevent inadequate results. Most peer-reviewed publications have evaluated the role of ultrasonography for abnormalities of the rotator cuff and the achilles tendon. Ultrasonography also commo

Non-Musculoskeletal Benefits of Vitamin D beyond the Musculoskeletal System.

Vitamin D, a fat-soluble prohormone, is endogenously synthesized in response to sunlight or taken from dietary supplements. Since vitamin D receptors are present in most tissues and cells in the body, the mounting understanding of the role of vitamin D in humans indicates that it does not only play an important role in the musculoskeletal system, but has beneficial effects elsewhere as well. This review summarizes the metabolism of vitamin D, the research regarding the possible risk factors leading to vitamin D deficiency, and the relationships between vitamin D deficiency and numerous illness

Collagen VI in the Musculoskeletal System.

Collagen VI exerts several functions in the tissues in which it is expressed, including mechanical roles, cytoprotective functions with the inhibition of apoptosis and oxidative damage, and the promotion of tumor growth and progression by the regulation of cell differentiation and autophagic mechanisms. Mutations in the genes encoding collagen VI main chains, COL6A1, COL6A2 and COL6A3, are responsible for a spectrum of congenital muscular disorders, namely Ullrich congenital muscular dystrophy (UCMD), Bethlem myopathy (BM) and myosclerosis myopathy (MM), which show a variable combination of mu

Functional imaging of the musculoskeletal system.

Functional imaging, which provides information of how tissues function rather than structural information, is well established in neuro- and cardiac imaging. Many musculoskeletal structures, such as ligaments, fascia and mineralized bone, have by definition a mainly structural role and clearly don't have the same functional capacity as the brain, heart, liver or kidney. The main functionally responsive musculoskeletal tissues are the bone marrow, muscle and nerve and, as such, magnetic resonance (MR) functional imaging has primarily addressed these areas. Proton or phosphorus spectroscopy, oth

Purinergic signalling in the musculoskeletal system.

It is now widely recognised that extracellular nucleotides, signalling via purinergic receptors, participate in numerous biological processes in most tissues. It has become evident that extracellular nucleotides have significant regulatory effects in the musculoskeletal system. In early development, ATP released from motor nerves along with acetylcholine acts as a cotransmitter in neuromuscular transmission; in mature animals, ATP functions as a neuromodulator. Purinergic receptors expressed by skeletal muscle and satellite cells play important pathophysiological roles in their development or

Computed tomography of the musculoskeletal system.

To demonstrate the potential impact of computed tomography on orthopaedics, a brief review was done of four generations of scanners and of one new development (assay of bone mineral in the spine) that is presently being researched. To assess the utility of the method in treating orthopaedic problems, the cases of 140 patients, including patients with congenital anomalies, tumors, infections, and traumatic lesions, were analyzed. In the majority of the patients the technique provided useful information, often unobtainable with any other technique.

Musculoskeletal involvement in systemic sclerosis.

Musculoskeletal (MSK) involvement is a very frequent manifestation of patients with systemic sclerosis (SSc). There are several reports about clinical trials assessing musculoskeletal involvement in SSc. However, only few controlled studies have been conducted. The prevalence of musculoskeletal symptoms, clinical and radiographic findings has been assessed. The most important articular (arthralgia, synovitis, contractures), tendon (tendon friction rubs, tenosynovitis) and muscular manifestations (myalgia, muscle weakness, myositis) should be carefully evaluated during the assessment of SSc pat

Musculoskeletal involvement in systemic sclerosis.

Systemic sclerosis (SSc) is a multi-organ, systemic connective tissue disease, which affects the lungs, heart, gastrointestinal tract, kidneys, skin, and musculoskeletal system. Musculoskeletal involvement is observed in 40-90% of patients with SSc. During the disease, any structure of the musculoskeletal system, such as bones, joints, tendon sheaths, tendons, and muscles, may be affected. The most common symptoms include joint pain, arthritis, tendinitis leading to tendon rupture, acro-osteolysis, calcinosis, myalgia, and myositis. Osteo-articular complications and changes in the soft tissues

Musculoskeletal Manifestations of Systemic Sclerosis.

Systemic sclerosis (SSc; scleroderma) is a chronic autoimmune connective tissue disease characterized by microvascular obliteration and sclerosis of the skin and internal organs. Although the clinical hallmark of the disease is the appearance of taut tethering of the skin, one of the earliest manifestations of SSc is a painful symmetrical arthropathy ranging from minimal arthralgia to overt polyarthritis. Musculoskeletal (MSK) involvement in SSc occurs more frequently than expected. Arthralgia is the most commonly reported manifestation. Some of the existing composite and organ-specific indice

[Postoperative imaging of the musculoskeletal system - spine].

Imaging of the postoperative spine intends to answer two main questions: It is used to assess the surgical success and to identify complications. To this end, conventional X‑ray, computer tomography (CT), myelography, and magnetic resonance imaging (MRI) are available. The radiologist has to select the appropriate modality for sufficient diagnosis considering the preoperative situation, the performed operation, and existing postoperative symptoms. Particularly, the implantation of foreign material represents a technical challenge in the context of image acquisition. In the radiologist's report

Dual-Energy CT for the Musculoskeletal System.

The principal advantages of dual-energy computed tomography (CT) over conventional CT in the musculoskeletal setting relate to the additional information provided regarding tissue composition, artifact reduction, and image optimization. This article discusses the manifestations of these in clinical practice-urate and bone marrow edema detection, metal artifact reduction, and tendon analysis, with potential in arthrography, bone densitometry, and metastases surveillance. The basic principles of dual-energy CT physics and scanner design will also be discussed. © RSNA, 2016.

Sports injury of the pediatric musculoskeletal system.

Sports related injuries are common in children and adolescents, with a reported incidence of around one in ten children each year. Boys incur more and severer sports injuries than girls, and chance for injury is greater with contact or jumping sports. Sports injuries seen in children under 10-years of age are non-specific, including contusions, mild sprains, and extremity fractures, usually Salter fractures of the physes (growth plate) or plastic fractures. In the very young athlete, sports injury of the ligaments or muscle is rare as are spine or head injuries. With growth and adolescence, th

Musculoskeletal system. Joint and vertebral column diseases.

Owner complaints that refer to the musculoskeletal system are common in older dogs and cats. When the veterinarian is presented with these types of complaints, the differential lists include chronic intervertebral disk disease, diskospondylitis, degenerative joint disease, spondylosis with nerve root compression, joint/ ligament instability, and/or cancer. The diagnosis and management of some of these conditions is presented in detail with the general goal in mind that the older dog or cat is provided the best quality of life possible through good mobility along with being pain free.

Drug-induced lesions of the musculoskeletal system.

Drugs, whether prescribed by physicians or taken without the knowledge of the physician, may result in profound changes in the musculoskeletal system. Some drugs may produce teratogenic changes in the developing fetus frequently, but not necessarily, in association with anomalies of other systems. Other drugs may have their effects on the child or adult, whereas a number of drugs result in either teratogenic or nonteratogenic abnormalities. In the course of evaluating any diagnostic imaging study, it is essential that the radiologist be aware of any drugs that the patient may be taking and be
