The platysma myocutaneous flap.

Reconstructing defects of the oral mucosa or skin of the lower one-third of the face can be accomplished by a variety of techniques. This article presents two versions of the platysma myocutaneous flap, which is a reliable, axial pattern, pedicled flap capable of providing excellent one-stage reconstruction of such defects. As discussed herein, the superiorly based and posteriorly based versions of the flap have wide application in the oral and facial region. Also provided is a review of other uses of this flap in head and neck surgery.

Platysma myocutaneous flap revisited.

The platysma myocutaneous flap has seen limited application in the reconstruction of head and neck defects over the past decade. There are anatomical and physiological reasons for this. There has been an emphasis on the anatomical vascular supply and method of preparation of the platysma myocutaneous flap and its use in the oral cavity. This article emphasizes the use of this flap for reconstruction of defects of the base of tongue, hypopharynx, lateral pharyngeal wall, and supraglottis. The criteria for selection of the platysma myocutaneous flap as a method of reconstruction, and the limitat

The brachioradialis myocutaneous flap.

The vascular anatomy and the cutaneous territorial definition of the brachioradialis myocutaneous flap has been studied by cadaver dissection and intra-arterial injection of methylene blue eye. A case is presented in which a proximal brachioradialis myocutanenous flap, based on the vascular pedicle arising from the radial recurrent artery just distal to the elbow level, was used to cover an exposed open elbow joint. The advantages and disadvantages of this flap are briefly discussed.

The scrotal myocutaneous flap.

The scrotum is a thermoregulatory, well-vascularized structure formed by skin and nonstriated muscle with unique elastic properties. This makes it an ideal source of tissue coverage for problem wounds in its vicinity. Two patients in which scrotal musculocutaneous flaps were used are reported: one, a paraplegic, with a recurrent ischioperineal decubitus ulcer, and another with an ulcer of the penis with exposed Dacron graft previously placed to treat Peyronie's disease. After reviewing the anatomy of the scrotum and the existent literature, we studied scrotal vascularity in a fresh specimen by

Composite myocutaneous flaps.

In this past few years myocutaneous (MC) flaps have, to a large extent, replaced other flaps. Because of axial blood supply, they may be used without delay. The MC flaps, useful in head and neck reconstruction, are based on the latissimus dorsi, the pectoralis major, the sternomastoid, and the trapezius. All have discrete independent vasculature that acts as a pedicle for the muscle and skin flap. The sternomastoid MC flaps have limited range and only special areas of usefulness. The latissimus dorsi flap, because it requires special positioning of the patient, has not been as popular as the p

Refinements of nasalis myocutaneous flap.

During the last 3 years, we have applied the nasalis myocutaneous island flap for reconstruction of nasal defects following excision of skin basal cell carcinoma and melanoma in 19 patients. By refinements with Z plasty, early dermabrasion, and placement of a bolster, we could minimize the trapdoor deformity, obvious scars, and blunting of the alar groove. No patients required revisional surgery, and flap survival was 100%. On the basis of our experience, the nasalis myocutaneous island flap is the solution for reconstruction of moderate-sized distal nasal defects in terms of reliability, aest

Split pectoralis major myocutaneous flap.

A variation of the pectoralis major myocutaneous flap in head and neck reconstruction is presented. The defect to be covered was a large, intraoral, U-shaped defect extending from the left tonsillar region around the alveolar ridge and floor of mouth to the right soft palate and buccal mucosa. Nearly the entire tongue was intact, and a 4 cm left mandibular segmental defect existed. A single left pectoralis major myocutaneous island flap was split longitudinally to resurface the defect. Implications of vertical splitting of a myocutaneous flap are discussed with respect to blood supply and limi

Vastus medialis myocutaneous and myocutaneous-tendinous composite flaps.

This report describes a new myocutaneous territory and introduces a new myocutaneous flap based on the vastus medialis muscle. Dissection, arterial dye perfusion, and specimen angiography of 32 fresh cadaver legs defined a vastus medialis myocutaneous territory directly overlying the muscle and also defined a segment of quadriceps femoris tendon vascularized by the muscle. This vascular unit provides a myocutaneous flap for closure of anterior knee defects. This flap can also be designed as a composite myocutaneous-tendinous flap by inclusion of the vascularized quadriceps femoris tendon segme

The pectoralis major myocutaneous flap.

INTRODUCTION: The pectoralis major myocutaneous flap (PMMF) is a workhorse flap for head and neck reconstruction and a flap of choice for anterior chest wall reconstruction. METHODS: A thorough review of the literature was undertaken, exploring the full range of designs, variations, indications, and outcomes of the PMMF. RESULTS: Numerous modifications have been documented since it was first described in 1968. Complication rates vary greatly in the literature, but most institutions describe significant success with the PMMF. It is fast, versatile, reliable, and a good alternative when micros

Mammography of autologous myocutaneous flaps.

Autologous myocutaneous flaps (AMFs) are used increasingly as a method of breast reconstruction after mastectomy for breast cancer. Autogenous breast reconstruction may be performed with a rectus abdominis, latissimus dorsi, or gluteus maximus myocutaneous flap. Mammographic imaging of AMFs is controversial but has been recommended by some authors because mammographic detection of nonpalpable local recurrences in AMFs continues to be reported. At mammography, AMFs have a predominantly fatty appearance with variable density due to the muscle component and postoperative scarring. Normal mammogra

The free latissimus dorsi myocutaneous flap.

The general anatomy and the detailed vascular anatomy of the latissimus dorsi myocutaneous flap is described, and its use as a free flap is illustrated. The speed and ease with which it can be raised, its long vascular pedicle, the large diameter of the donor vessels, and the potentially large size of the flap are emphasized.

Infrahyoid myocutaneous flap: A systematic review.

INTRODUCTION: The infrahyoid myocutaneous flap (IHMCF) is an often-overlooked flap of the anterior neck used for reconstruction of oral cavity and laryngopharyngeal defects. The primary goal of this systematic review is to evaluate the postoperative outcomes and efficacy of this flap. METHODS: A comprehensive search of PubMed, Biological Abstracts, CINAHL Plus, and Web of Science was conducted. Two researchers independently scrutinized the studies to determine inclusions based on relevance, sample size, and English language publications. RESULTS: Twenty-eight studies containing 1027 IHMCF ca

[The superior pedicled platysma-myocutaneous flap].

INTRODUCTION: The platysma-myocutaneous flap is a surgical procedure little used in oral and oropharyngeal reconstruction after malignant tumor resection. However, it provides a fast solution of reconstruction in specific indications, with functional and esthetic results comparable to other methods. ANATOMY: The platysma covers the anterolateral area of the neck and constitutes a part of the superficial musculoaponevrotic system of the head and neck. The arterial vascularization relies on the external carotid network and venous drainage on the external and anterior jugular veins. SURGICAL TE

Postauricular myocutaneous flap in otologic surgery.

Management of a large mastoid defect resulting from skull base operations or extensive surgical procedures because of chronic ear disease continues to challenge the otologic surgeon. Various local muscle or periosteal rotation flaps have been used to help reduce the size of the postoperative mastoid cavity. With these techniques there are problems with flap retraction and epithelization that may result in delayed healing or chronic drainage. Closure of the ear canal and tissue obliteration of the mastoid results in a maximal conductive hearing loss. A postauricular myocutaneous flap based on t

Postoperative outcomes of free myocutaneous flap and pedicled myocutaneous flap for reconstruction in locally invasive thyroid carcinoma.

Locally invasive thyroid carcinoma (TC) often involves trachea. In such patients, the trachea needs to be reconstructed after surgery. We discuss the postoperative outcome and complications after trachea reconstruction by free myocutaneous flap (FMF) and pedicled myocutaneous flap (PMF).From January 2009 to September 2019, the clinical data of 38 patients with TC were retrospectively analyzed. Demographics, pathologic results, neoplasm staging, surgical protocol, decannulation, subjective speech function, and complications were documented.A total of 38 patients were analyzed (22 FMFs and 16 PM

Lymphoscintigraphy in pectoralis major myocutaneous flaps.

Myocutaneous flaps are used widely in the surgical treatment of advanced cancers, which in the past had been thought to be inoperable. Tumor recurrences are expected more frequently in these patients. Recurrent tumors may spread locally and to the regional areas via lymphatics and vessels. However, the lymphatic spread may differ in cases where myocutaneous flaps are used for reconstruction. This study is based on five patients with head and neck cancer who underwent reconstruction with myocutaneous flaps. Technetium Tc99m-labeled dextran was used to demonstrate the lymphatic flow, and technet

The platysma myocutaneous flap. Indications and caveats.

The platysma myocutaneous flap has enjoyed limited popularity despite its versatility, dependability, and ease of harvesting. In this article, we describe 12 consecutive patients who underwent platysma flap reconstruction of various oral cavity and oropharyngeal defects. Complications included loss of one skin paddle, one pharyngocutaneous fistula, and one intraoral wound dehiscence. In all patients, bare bone was covered or appropriate spacing between the tongue and other structures was maintained to avoid ankyloglossia. We discuss specific indications and caveats.
