Nasal decongestants.

When a patient presents with nasal obstruction a correct diagnosis must be made. Treatment may not always be required, and the need for intervention should be balanced against the risks of therapy. If the patient has an acute problem, such as the common cold or sinusitis, a topical decongestant may be the most immediately effective remedy, but this should never be continued for more than a few days and the patient should be warned against buying similar proprietary products. In more chronic cases, such as allergic or vasomotor rhinitis, or when decongestion of mucosa away from the nose (Eustac

Do topical nasal decongestants affect polyps?

CONCLUSIONS: The hypothesis that decongestants reduce the size of nasal polyps could not be verified. Decongestion is therefore recommended prior to nasal endoscopy, especially in polyp diagnosis, as it does not artificially change the size of the polyps. OBJECTIVE: The role of nasal decongestants in nasal stuffiness is well established and their action is well known. Decongestants are also used prior to nasal endoscopy to achieve a better view of the nasal cavity in order to diagnose polyps. The question is whether this decongestion invalidates the estimation of polyp size in clinical and sc

alpha2-adrenoceptor agonists as nasal decongestants.

Nasal congestion, one of the major disease features of rhinitis, is induced by the filling of venous sinusoids causing mucosal engorgement with resultant obstruction of nasal airflow. The only available drugs that directly target the underlying vascular features driving nasal obstruction are the sympathomimetic alpha-adrenoceptor agonists due to their vasoconstrictor action. However, standard decongestants are nonselective alpha-adrenoceptor agonists, which have the potential for side-effects liabilities such as hypertension, stroke, insomnia and nervousness. In the present study, the effects

Underwater application of nasal decongestants: method for special operations.

A simple method of emergency underwater application of a nasal decongestant in divers to prevent diving-related accidents or even fatalities attributable to sequelae of middle-ear and sinus barotrauma of ascent was evaluated. Eleven military divers had to inject 1 mL of 0.02% methylene blue into a central venous catheter after having inserted the tip between their upper lip and the mask at 1 m depth in a pool. After injection, the head had to be reclined. Blue liquid flowing from a diver's nostril and a "bitter" taste sensation reported immediately after surfacing indicated successful applicat

Rise and fall of decongestants in treating nasal congestion related diseases.

INTRODUCTION: Decongestants are commonly used drugs in clinical practice, and they can relieve nasal congestion caused by factors like influenza, rhinitis, and acute upper respiratory tract infection. AREAS COVERED: In this article, we review the research outcomes about decongestants, which aim to provide beneficial information that can guide the clinical application of decongestants for clinicians. EXPERT OPINION: Although the use of nasal decongestants is increasingly limited, caution rather than prohibition is now advocated. Scientific and accurate use of nasal decongestants can achieve s

A rationale for the selection of nasal decongestants in lacrimal drainage surgery.

Adequate hemostasis during lacrimal drainage surgery affects the success of the operation. Nasal decongestants, which are sympathomimetric agents including sympathomimetic amines and imidazoline derivatives, help to decrease bleeding. Certain of the imidazoline derivatives--oxymetazoline and xylometazoline--are potent and long-acting agents that have many of the same adrenergic effects as cocaine. Their use as an alternative to cocaine provide adequate hemostasis with less adverse reactions than cocaine. The authors recommend premedication of the nasal mucosa with oxymetazoline or xylometazoli

Selecting the Correct Dosage Form for Topical Nasal Decongestants: An Expert Opinion.

Nasal congestion is a bothersome symptom of allergic rhinitis (AR), common cold, and sinusitis, and can significantly impact quality of life (QoL). In options available for managing nasal congestion, oral decongestants have a slower onset of action compared with topical nasal decongestants like oxymetazoline and xylometazoline, which have an onset of action within 2 minutes. It is important to select the correct dose form for topical drug delivery. A panel discussion was conducted with eight ear, nose, and throat (ENT) specialists in India to discuss the burden of nasal congestion and its impa

The effect of decongestion on nasal airway patency and airflow.

Nasal decongestant reduces blood flow to the nasal turbinates, reducing tissue volume and increasing nasal airway patency. This study maps the changes in nasal anatomy and measures how these changes affect nasal resistance, flow partitioning between superior and inferior cavity, flow patterns and wall shear stress. High-resolution MRI was applied to capture nasal anatomy in 10 healthy subjects before and after application of a topical decongestant. Computational fluid dynamics simulated nasal airflow at steady inspiratory flow rates of 15 L.min[Formula: see text] and 30 L.min[Formula: see text

Exogenous lipoid pneumonia induced by nasal decongestant.

INTRODUCTION: Lipoid pneumonia is a clinical condition that may be initially asymptomatic or confused with an infectious or malignant lung disease. OBJECTIVES: We report four cases of this pathological condition. METHODS: The first case concerned an 85-year old woman with bilateral confluent pulmonary opacities, ground-glass type. Diagnosis was based on the cytology of the bronchoalveolar lavage (BAL) fluid followed by its ultrastructural examination. The second case was a 47-year-old man with an isolated pulmonary nodule, which was surgically removed; the diagnosis of lipoid pneumonia was f

Dose-response relationship of a topical nasal decongestant: phenylpropanolamine.

By means of anterior rhinomanometry, the decongestant effect of topically applied phenylpropanolamine (PPA) was evaluated in 40 patients with nasal obstruction. A dose-response relation was obtained for 0.5, 2.5 and 5.0% PPA solution, with the maximum effect (40% decongestion) found with the 2.5% solution. The decongestant effect was comparable to that induced by physical exercise (50% decongestion). It is concluded that PPA nose-drops are effective and offer an attractive alternative to some of the commonly used nasal decongestants.
