Nasoalveolar Molding.

Nasoalveolar molding (NAM) is a presurgical infant orthopedic therapy that brings the alveolar segments in closer opposition and realigns the soft tissue and cartilaginous tissues of the lip and nose in patients with cleft lip and palate. NAM offers clinical and nonclinical benefits with improvements to various palatal, alveolar, and nasolabial parameters; possible reduction in revision surgery and financial burden; and psychosocial benefits to caregivers. Many variations have been made to NAM appliances and protocols, with emerging digital workflows holding great promise for improving patient

Nasoalveolar Molding: Assessing the Burden of Care.

Over the last 3 years a shift at our institution has taken place in which patients who would have been offered nasoalveolar molding (NAM) as an adjunct to cleft lip repair (repair after 3 months) have instead undergone early cleft lip repair (ECLR) (2-5 weeks of life) without NAM. This study sought to examine the financial and social impact of the transition away from NAM to ECLR. The efficacy of NAM is limited by patient compliance to a rigorous treatment schedule requiring weekly visits for appliance adjustments. Nasoalveolar molding patients required an average of 11 dental visits, accounti

A Comparative Cephalometric Study of Nasoalveolar Molding- and Non-Nasoalveolar Molding-Treated Bilateral Cleft Patients at Early Mixed Dentition Period.

OBJECTIVE: To evaluate and compare early maxillary growth in 2 groups of patients with repaired bilateral cleft lip and palate (BCLP) who had and had not received nasoalveolar molding (NAM) therapy in infancy. HYPOTHESIS: Nasoalveolar molding does not have any effect on maxillomandibular growth at the early mixed dentition period. DESIGN: Retrospective study. PATIENTS: The study group consisted of forty-two 7-year-old patients with BCLP-20 in the NAM group and 22 in the no-NAM group. The control group consisted of nineteen 7-year-old noncleft, normal patients. INTERVENTIONS: Nasoalveolar m

Presurgical Infant Orthopedics: Sustainable Alternatives to Nasoalveolar Molding.

The conventional management of cleft patients involves presurgical infant orthopedics (PSIO), a strategy designed to align maxillary segments and improve nasal shape before definitive reparative surgery. Nasoalveolar molding (NAM) has been widely adopted for this purpose, but its drawbacks are threefold: it is costly and unavailable in many low-income countries and requires frequent follow-up. The aim of this article is to present practical PSIO alternatives that offer greater accessibility and sustainability while yielding comparable preoperative outcomes. The authors propose that the suggest

Pre-surgical nasoalveolar molding technique: a literature review.

Cleft lip and palate patients have many defects particularly nasal deformities. The nasoalveolar molding (NAM) technique is an adjunctive treatment, which not only corrects deviated alveolar segments but also addresses nasal deformity before cheiloplasty. NAM technique is claimed to facilitate primary surgical correction and to provide favorable esthetic outcomes. However, there is limited evidence to confirm the special benefits ofNAM technique since so far there have been no truly long term controlled clinical trials to evaluate outcomes of treatment based on pre-surgical NAM technique. NAM

Nasoalveolar molding in cleft care: is it efficacious?

In the era of evidence-based medicine, new treatment protocols and interventions should be routinely evaluated for their efficacy by reviewing the available evidence. In the cleft literature, nasoalveolar molding has garnered attention over the last decade as a new option for improving nasal form and symmetry before primary surgical repair. Systematic review of the evidence is, however, currently lacking. This review evaluates whether nasoalveolar molding can improve nasal symmetry and form toward the norm, as well as whether nasoalveolar molding demonstrates advantages over other protocols in

Individualized Digital Nasoalveolar Molding: A Proof of Concept.

Nasoalveolar molding (NAM) is a recognized adjunctive therapy to primary cheiloplasty in patients with complete cleft lip and palate (CLP). The conventional technique can be time-consuming, costly, and requires experienced clinicians. Over the past decade, CAD/CAM technology has been employed to facilitate NAM therapy, showing promising results. This study introduces an individualized Digital NAM (iDNAM), a digital approach revolutionizing NAM treatment using CAD/CAM technology, and evaluates treatment outcomes. A 57-day-old infant with complete UCLP underwent intraoral and nasolabial scans, a

Long-Term Comparison of the Aesthetic Outcomes between Nasoalveolar Molding- and Non-Nasoalveolar Molding-Treated Patients with Unilateral Cleft Lip and Palate.

BACKGROUND: The cleft lip-nose deformity in unilateral cleft lip and palate is one of the most challenging problem for surgeons to correct. Although nasoalveolar molding has been shown to be effective in improving presurgical symmetry in patients with complete unilateral cleft lip and palate, there is need for better evidence regarding the long-term nasolabial aesthetics of patients who have received this therapy. METHODS: Thirty-eight patients treated with nasoalveolar molding and 48 patients not treated with nasoalveolar molding (but otherwise treated similarly) with unilateral cleft lip an

Recent Advances in Nasoalveolar Molding Therapy Using 3D Technology.

Presurgical Nasoalveolar Molding (NAM) is an adjunctive treatment modality designed to reorient misaligned tissue structures and nasal cartilage in cleft lip and/or palate (CL/P) patients. Recent advances in NAM therapy focus on modifications to the intraoral molding plate or nasal stent intended to improve treatment outcomes, ease of use, compliance, and cost-effectiveness. Notably, 3D technological advancements have been employed to design NAM devices more efficiently and create objective, standardized means of measuring progressive morphological changes during therapy. These advances are de

Nasoalveolar Molding for Unilateral and Bilateral Cleft Lip Repair.

Nasoalveolar molding (NAM) is a powerful tool in the treatment of patients with unilateral or bilateral cleft lip and palate. The primary goal of NAM is to improve alignment of critical anatomic elements before surgical repair of the unilateral or bilateral cleft lip. Modifications of the position of the alveolar segments and their associated lip elements, the lower lateral cartilages, and the columella achieved with NAM are helpful for creating a suitable platform for tension-free lip repair.

Cleft Lip Repair, Nasoalveolar Molding, and Primary Cleft Rhinoplasty.

Cleft lip and palate are the fourth most common congenital birth defect. Management requires multidisciplinary care owing to the complexity of these clefts on midface growth, dentition, Eustachian tube function, and lip and nasal cosmesis. Repair requires planning, but can be performed systematically to reduce variability of outcomes. The use of primary rhinoplasty at the time of cleft lip repair can improve nose symmetry and reduce nasal deformity. Use of nasoalveolar molding ranging from lip taping to the use of preoperative infant orthopedics has played an important role in improving functi

Montreal Children's Hospital formula for nasoalveolar molding cleft therapy.

BACKGROUND: For cleft teams that use nasoalveolar molding for presurgical treatment of cleft lips, the determination of desired cleft-nasal height correction is a subjective assessment. The latter, however, is complicated by a noncleft nasal height that itself is depressed by the shifted nasal pyramid native to the deformity. The authors introduce a simple formula based on the Pythagorean theorem to estimate the corrected height of the nose as an objective guide for the endpoint of nasoalveolar molding therapy. METHODS: Nasal impressions of 20 consecutive patients with unilateral cleft lips w
