Alzheimer's & dementia : the journal of the Alzheimer's Association2026eng
The Alzheimer's Disease Research Centers (ADRC) program, funded by the National Institute on Aging (NIA), is a national network for conducting cutting-edge basic, clinical, and translational Alzheimer's disease (AD) and AD-related dementias (ADRD) research. ADRCs collect clinical, biomarker, neuroimaging, and autopsy data on thousands of longitudinally followed research participants and the well-characterized data and samples generated by the ADRCs are readily available to the research community. Beyond conducting research, the primary objectives of the ADRC program are to train the next gener

New criteria for Parkinson's disease-associated psychosis (PDAP) were recently proposed by a NINDS-NIMH working group. We assessed 116 consecutive unselected outpatients with PD for the existence of psychotic symptoms during the previous month, using a structured questionnaire covering the whole spectrum of PDAP symptoms. Hallucinations occurred in 42% of the patients (visual: 16%; nonvisual: 35%), delusions in 4%, and minor symptoms in 45% (sense of presence, visual illusions, or passage hallucinations). The prevalence of PDAP was 43% when the usual definition was used (hallucinations and/or

Since the groundbreaking work of Alois Alzheimer in the early 1900s, Alzheimer's disease research has evolved immensely, most notably with the establishment of the Alzheimer's Disease Research Centers (ADRC). Fueled by the vision and funding of the National Institute on Aging (NIA), ADRCs have created a legacy through their work standardizing criteria for Alzheimer's disease diagnosis. Leveraging the experience of the Consortium to Establish a Registry for Alzheimer's Disease, the ADRCs advanced standardized assessment tools, including the Clinical and Neuropsychological Batteries, as well as

The Dominantly Inherited Alzheimer Network (DIAN) initially was funded by the National Institute on Aging (NIA) in 2008 and thus was able to adopt and incorporate the protocols developed by the Alzheimer's Disease Neuroimaging Initiative (ADNI) that had been established by the NIA in 2004. The use of ADNI protocols for DIAN neuroimaging studies and assays of biological fluids for Alzheimer disease (AD) biomarkers permitted examination of the hypothesis that autosomal dominant AD (ADAD), studied by DIAN, and "sporadic" late-onset AD (LOAD), studied by ADNI, shared the same pathobiological const

Alzheimer's disease (AD) accounts for the majority of dementia cases and leaves clinicians, patients, family members, caregivers, and researchers faced with numerous ethical issues that vary and evolve as a function of disease stage and severity. While the disclosure of a diagnosis of AD dementia is difficult enough, advances in the neurobiology of AD--embodied in the recent revisions to the AD diagnostic guidelines--have translated into an increasing shift toward the diagnosis being made in its pre-dementia stages, when patients have full insight into their prognosis. Genetic issues in AD are

Chronic traumatic encephalopathy (CTE) is a neurodegenerative disorder associated with exposure to head trauma. In 2015, a panel of neuropathologists funded by the NINDS/NIBIB defined preliminary consensus neuropathological criteria for CTE, including the pathognomonic lesion of CTE as "an accumulation of abnormal hyperphosphorylated tau (p-tau) in neurons and astroglia distributed around small blood vessels at the depths of cortical sulci and in an irregular pattern," based on review of 25 tauopathy cases. In 2016, the consensus panel met again to review and refine the preliminary criteria, w

Populations at higher risk for Alzheimer's disease and related dementias (ADRD) are often not proportionally represented in research. These populations include but are not limited to individuals from wide-ranging ethnic and racial backgrounds, those with lower education, and those with limited health-care access. Addressing disparities in research participation will be essential to improving our understanding of the varied causal mechanisms, risks and protective factors, and responses to interventions to find generalizable solutions to improve health outcomes in ADRD. The goals of this perspec

In this issue of Alzheimer's & Dementia, Mashour et al. propose the intriguing hypothesis that some manifestations of late-stage dementia are reversible, albeit transiently. Calling this phenomenon paradoxical lucidity, their paper follows a 2018 workshop sponsored by the National Institute on Aging that assessed the state of knowledge on lucidity in dementia and identified areas ripe for further study. The National Institute on Aging has since released two funding opportunity announcements (RFA-AG-20-016 and RFA-AG-20-017) to establish the building blocks of such a research program. The poten

INTRODUCTION: The National Institute on Aging Late-Onset Alzheimer's Disease Family Based Study (NIA-LOAD FBS) was established to study the genetic etiology of Alzheimer's disease (AD).
METHODS: Recruitment focused on families with two living affected siblings and a third first-degree relative similar in age with or without dementia. Uniform assessments were completed, DNA was obtained, as was neuropathology, when possible. Apolipoprotein E (APOE) genotypes, genome-wide single nucleotide polymorphism (SNP) arrays, and sequencing was completed in most families.
RESULTS: APOE genotype modified

BACKGROUND AND PURPOSE: The scale of stroke impairment characteristics by Brott and associates, the National Institutes of Health (NIH) Stroke Scale, has been used widely in various studies of stroke outcome; however, the measurement properties of the items applied to patients during medical rehabilitation have not been evaluated thoroughly. This study evaluated the extent to which scale items cohere to define a unidimensional construct and have a useful range for application to patients during medical rehabilitation.
METHODS: Rating scale (or Rasch) analysis of the 15 NIH Stroke Scale items

BACKGROUND: During the course of a previous study, our laboratory made a serendipitous finding that just thinking about genital stimulation resulted in brain activations that overlapped with, and differed from, those generated by physical genital stimulation.
OBJECTIVE: This study extends our previous findings by further characterizing how the brain differentially processes physical 'touch' stimulation and 'imagined' stimulation.
DESIGN: Eleven healthy women (age range 29-74) participated in an fMRI study of the brain response to imagined or actual tactile stimulation of the nipple and clito

Background: The National Institute on Alcohol Abuse and Alcoholism (NIAAA) finalized a definition of alcohol use disorder (AUD) recovery requiring (1) remission from DSM-5 AUD symptoms except craving and (2) cessation of heavy drinking. The literature suggests that recovery capital, which encapsulates the resources that influence substance use recovery, is an important determinant of AUD recovery outcomes. However, little research has examined potential relationships between recovery capital and the NIAAA recovery definition. Objectives: This study examined associations between recovery capita

The National Institute on Aging and the Alzheimer's Association convened three separate work groups in 2011 and single work groups in 2012 and 2018 to create recommendations for the diagnosis and characterization of Alzheimer's disease (AD). The present document updates the 2018 research framework in response to several recent developments. Defining diseases biologically, rather than based on syndromic presentation, has long been standard in many areas of medicine (e.g., oncology), and is becoming a unifying concept common to all neurodegenerative diseases, not just AD. The present document is
