Physician understanding of the National Practitioner Data Bank.

National Practitioner Data Bank (NPDB) reports are part of the process for acquiring staff privileges, professional credentials, and licenses throughout a physician's entire career. We surveyed our hospital's 66 residents to assess their understanding of NPDB. Only 9 residents had heard about NPDB. A follow-up survey of the 10 medical schools in Pennsylvania and Maryland showed just 4 schools covered NPDB in their curricula. Finally, we did a third survey--of 1,410 Pennsylvania Medical Society members. Eighty-one percent did not know that denial of initial license application was not a reporta

The National Practitioner Data Bank: coping with the uncertainties.

The National Practitioner Data Bank is a database of adverse events involving physicians and other practitioners. Querying the database is mandatory for hospitals in several situations. So too, hospitals are required to report specified adverse events. Thus, hospitals need to be able to identify incidents that are reportable events, events that require them to update the databases, and any possible liability issues that may surround the hospital's reporting duties. The author argues that the regulations are unclear in addressing these issues. Likewise, he notes that practitioners should be awa

Product and Equipment Coding in National Practitioner Data Bank Malpractice Payment Reports, 2019-2025.

INTRODUCTION: Codes related to products and equipment in the National Practitioner Data Bank (NPDB) may identify only a selected subset of malpractice payment reports involving medical products. Our objective was to compare coded injury severity and payment profiles between physician malpractice payment reports with and without product or equipment coding, and to determine whether any differences reflected allegation-group composition, so that researchers can judge whether these codes are usable as a standalone case definition for product-related malpractice. METHODS: We conducted a retrospec

Associations Over Time Between Paid Medical Malpractice Claims and Imaging Utilization in the United States.

PURPOSE: The aim of this study was to explore state-level relationships between the incidence and payout amounts for medical malpractice claims and Medicare imaging utilization and spending across the United States. METHODS: Using claims data from a 5% sample of Medicare beneficiaries for 2004 to 2016, annual state population-adjusted rates of imaging utilization and spending were calculated. Using National Practitioner Data Bank files, all paid physician malpractice claims were identified, and lagged state population-adjusted rolling averages of paid claim frequencies and payout amounts were

Surgical malpractice claims in the United States.

Despite ongoing reform, there is still significant physician concern regarding the impact of medical claims on their practices. It is important that physicians and healthcare risk management professionals have a good understanding of the outcomes of medical malpractice to participate in its restructuring as needed and to prevent potentially harmful practices. In our study, we reviewed National Practitioner Data Bank (NPDB) paid malpractice claim reports from September 1, 1990, through July 30, 2011, and identified the 10 most common surgery-related allegations against physicians, excluding tho

Time to End Physician Sexual Abuse of Patients: Calling the U.S. Medical Community to Action.

Despite the strict prohibition against all forms of sexual relations between physicians and their patients, some physicians cross this bright line and abuse their patients sexually. The true extent of sexual abuse of patients by physicians in the U.S. health care system is unknown. An analysis of National Practitioner Data Bank reports of adverse disciplinary actions taken by state medical boards, peer-review sanctions by institutions, and malpractice payments shows that a very small number of physicians have faced "reportable" consequences for this unethical behavior. However, physician self-

Counting difficulties: retained instruments, sponges, and needles.

Patients in whom a sponge or instruments is left after surgery may suffer complications including pain, infection, abscess, or intestinal obstruction. Consequences or retained items for surgical team members may include malpractice lawsuits and adverse actions from the National Practitioner Data Bank and state licensing board. Adherence to AORN recommended practices for counting and facility counting policies can protect both patients and practitioners.

Stakeholder Recommendations on Shaping Human Biospecimen Banking Governance in Pakistan.

Standardized biospecimen banking is foundational for biomedical research and eventually underpins population health outcomes. In many low- and middle-income countries (LMICs) biobanking remains fragmented and weakly governed, limiting both scientific value and public trust in the ensuing research. This commentary presents insights from a stakeholder dialogue at a national pathology conference in Pakistan. The discourse identified drivers of human biospecimen banking fragmentation and governance gaps in the country, while mapping out contextually appropriate harmonization opportunities. Partici

The National Children's Study: a 21-year prospective study of 100,000 American children.

Prospective, multiyear epidemiologic studies have proven to be highly effective in discovering preventable risk factors for chronic disease. Investigations such as the Framingham Heart Study have produced blueprints for disease prevention and saved millions of lives and billions of dollars. To discover preventable environmental risk factors for disease in children, the US Congress directed the National Institute of Child Health and Human Development, through the Children's Health Act of 2000, to conduct the National Children's Study. The National Children's Study is hypothesis-driven and will

Fitness, knowledge, progress: assessing physician qualification.

The informed and empowered consumer is an ideal invoked by many would-be health care reformers. An actual consumer wishing to don the mantle of power may be hindered by the scarcity of information available, particularly with respect to choosing among physicians. How is one to know who is best qualified? This issue brief looks at the basics of physician qualification and the processes by which physicians are licensed, credentialed, and board-certified. It examines how the evolution of these processes (for example, the move from lifetime certification to ongoing maintenance of certification) af

[Evidence-based psychotherapy of addictive disorders].

BACKGROUND: Addictive disorders form the group of the most common mental disorders. A wide range of psychotherapeutic treatment interventions exists; however, the proportion of patients receiving evidence-based interventions or psychotherapeutic treatment in outpatient care is very low. OBJECTIVE: The aim of the present review was a systematic reassessment of the empirical evidence for the efficacy of the different forms of psychotherapeutic treatment, identification of new effective interventions and derivation of recommendations for treatment practitioners. Incidence of Scaphoid Fractures and Associated Injuries at US Trauma Centers.

Backgound  The scaphoid is the most commonly fractured carpal bone and often presents as a diagnostic challenge. Fractures can often go unnoticed on initial radiographic evaluation and clinical presentation can vary significantly among patients. Awareness of high-risk cohorts assists practitioners in making the appropriate clinical diagnosis of acute scaphoid fracture. Materials and Methods  The National Trauma Data Bank is the world's largest trauma data repository. Utilizing the 2016 public use file, we isolated scaphoid fractures by anatomic fracture location by utilizing International Clas

Characterization of Age-Related Injury Patterns and Surgical Treatment of Pediatric Facial Fractures: Analysis of the National Trauma Data Bank.

BACKGROUND: Presentation of pediatric facial fractures varies widely and many injuries are encountered infrequently by most practitioners. This study summarizes injury patterns in a large cohort of facial fractures and their subsequent surgical management. METHODS: Demographic and clinical characteristics of patients 18 years of age or younger admitted between 2009 and 2015 to trauma centers participating in the National Trauma Data Bank were examined. Craniofacial fractures and reconstructive procedures performed at index admission were selected based on ICD-9 and AIS codes. A multivariable

Litigations following spinal neurosurgery in France: "out-of-court system," therapeutic hazard, and welfare state.

OBJECTIVE: Spinal surgeries carry risks of malpractice litigation due to the random nature of their functional results, which may not meet patient expectations, and the hazards associated with these complex procedures. Claims are frequent and costly. In France, since 2002, a new law, the Patients' Rights Law of March 4, 2002, has created an alternative, out-of-court scheme, which established a simplified, rapid, free-of-charge procedure (Commission for Conciliation and Compensation [CCI]). Moreover, this law has optimized the compensation provided to patients for therapeutic hazards by use of

Paediatric fracture complications following traditional bone setting in low- and middle-income countries: a systematic review and meta-analysis protocol.

INTRODUCTION AND AIMS: Traditional bone setters (TBS)-lay practitioners who manage fractures without formal medical qualification-treat an estimated 10%-85% of fracture patients in low- and middle-income countries (LMICs). Children are disproportionately vulnerable due to physeal immaturity and lower diastolic blood pressure thresholds that increase susceptibility to compartment syndrome at lower absolute intracompartmental pressures than in adults. No prior systematic review has restricted analysis exclusively to the paediatric population or generated pooled meta-analytic estimates of complic

Association Between US Physician Malpractice Claims Rates and Hospital Admission Rates Among Patients With Lower-Risk Syncope.

IMPORTANCE: The US Government Accountability Office has changed its estimate of the annual costs of defensive medicine, largely because it has been difficult to objectively measure its impact. Evaluating the association of malpractice claims rates with hospital admission rates and the costs of admitting patients with low-risk conditions would help to document the impact of defensive medicine. Although syncope is a concerning symptom, most patients with syncope have a low risk of adverse outcomes. However, many low-risk patients are still admitted to the hospital, with associated costs of more

Canadian Assessment of Physical Literacy Second Edition: a streamlined assessment of the capacity for physical activity among children 8 to 12 years of age.

BACKGROUND: The Canadian Assessment of Physical Literacy (CAPL) assesses the capacity of children to lead a physically active lifestyle. It is comprised of a battery of standardized assessment protocols that reflect the Canadian consensus definition of physical literacy. The Royal Bank of Canada Learn to Play - Canadian Assessment of Physical Literacy study implemented the CAPL with 10,034 Canadian children (50.1% female), 8 to 12 years of age. Feedback during data collection, necessary changes identified by the coordinating centre, and recent data analyses suggested that a streamlined, second
