Syringe and needle exchange programs: Part I.

Syringe and needle exchange (SANE) programs appear to be a pragmatic approach to reducing the rising incidence of human immunodeficiency virus (HIV) infection among intravenous drug users (IVDUs). One purported indication of the effectiveness of SANE programs is lower prevalence rates of cases of AIDS or of HIV seropositivity in cities with SANE programs than in cities without such programs. In each positive comparison cited, however, the lower levels of HIV seropositivity were already present before the establishment of SANE programs. A second indicator of SANE effectiveness is a reduction in

Effectiveness of needle/syringe exchange program in Tbilisi.

IDUs are under the high risk of HIV and other blood born diseases. In Georgia injecting drug use is associated with two third of registered HIV/AIDS cases. Majority of them are also infected with B and C Hepatitis. One of the main components of HIV/AIDS prevention among drug users is considered to be harm reduction programs, among them syringe exchange program. We conducted observational cohort study and performed interviewing participants of syringe exchange program using structured questionnaire. The interviewing was conducted at intake, after 3 months and at the end of the program. During i

Depression among needle exchange program and methadone maintenance clients.

The objective of this study was to compare the prevalence of major depression in two cohorts of injection drug users, those enrolled in a Rhode Island Methadone Maintenance Treatment Program (MMTP) and those enrolled in a Rhode Island Needle Exchange Program (NEP) using cross-sectional interviews. Symptomatic and duration criteria for major depression in the last 6 months were identified using the Structured Clinical Interview for DSM-III-R (SCID). Among 528 persons interviewed, 54% of those in NEP and 42% of those in MMTP met criteria for major depression. Using multivariate logistic regressi

HIV/AIDS prevention: problems and approaches to establishing needle exchange programs in rural areas.

Needle and Syringe Exchange Programs (NSEP) were established to prevent the spread of HIV/AIDS amongst intravenous drug users (IVDU). The first rural NSEP in Australia was developed in the North Coast Region of NSW. The aims and objectives of the NSW NSEP are outlined. The problems and approaches faced by health services managers in developing a needle exchange program in a conservative rural area are presented with the emphasis on 4 major groups: intravenous drug users; the media; the general community and the public health system. A brief description of the North Coast NSEP, which is a combi

Pushing the boundaries of outreach work: the case of needle exchange outreach programs in Canada.

In this paper, we examine the challenges of defining the boundaries of outreach work using the example of needle exchange programs. In particular, we examine the multiple and inter-related factors that extend needle exchange outreach work beyond its official mandate. Using semi-structured interviews, 59 workers at 15 programs in Ontario, Canada were asked questions about operational policies and routines. An iterative and inductive analytic process was used. Over time, most outreach workers develop a well-defined sense of the activities they consider to be consistent with a harm reduction appr

Injecting substance use in prisons in the United States: a case for needle exchange programs.

Despite efforts to prevent illicit substance use in prisons, inmates in many countries still have access to them, along with associated paraphernalia. In the United States, a large proportion of people in prison are there due to drug-related charges. The secretive nature of illicit substance use in prison and the opposition to implementing harm reduction interventions such as needle exchange programs have put prisoners who engage in such practices at high risk for infectious bloodborne diseases. While states have their reasons for opposing the implementation of needle exchange programs in pris

The effect of the needle exchange program on the spread of some sexually transmitted diseases.

In this paper we consider a model for the spread of a sexually transmitted disease considering sexual transmission and spread via infected needles among intravenous drug users. Besides the transmission among drug users, we also consider sexual contacts between intravenous drug users and non-drug users. Furthermore, the needles are considered as a vector population. For several European countries, a sharp increase of sexually transmitted diseases was reported and several others are rated as endangered based on the number of syringes given out per intravenous drug users per year. The main purpos

A bridge to treatment: the needle exchange pilot program in New York City.

The needle exchange pilot program in New York City is one element in a spectrum of drug prevention and treatment modalities under way to slow the spread of the human immunodeficiency virus. The program's aim is to help us learn about the addict behaviors that spread the AIDS virus, including whether needle exchange can help reduce the frequency or eliminate those behaviors. Thus far, its primary value for clients lies in drug counseling and referral to treatment. Where an active IV drug user has taken the first step in changing the direction of his or her life by seeking treatment, needle exch

Qualitative case study of needle exchange programs in the Central Appalachian region of the United States.

BACKGROUND: The Central Appalachian region of the United States is in the midst of a hepatitis C virus epidemic driven by injection of opioids, particularly heroin, with contaminated syringes. In response to this epidemic, several needle exchange programs (NEP) have opened to provide clean needles and other supplies and services to people who inject drugs (PWID). However, no studies have investigated the barriers and facilitators to implementing, operating, and expanding NEPs in less populous areas of the United States. METHODS: This qualitative case study consisted of interviews with program

Exploring stakeholder perceptions of facilitators and barriers to using needle exchange programs in Yunnan Province, China.

Injection drug use is an ongoing urban health crisis in China and one of the largest drivers of the transmission of HIV/AIDS. Sentinel surveillance sites in Yunnan province show upwards of 20% of injection drug users (IDUs) are HIV positive. Though the Ministry of Health has scaled-up needle exchange programs (NEPs), they have not received official government recognition nor have they been extensively evaluated to explore factors influencing their acceptability and feasibility. Using in-depth qualitative interviews conducted from February to July 2008 with 35 participants consisting of IDUs an

Securitizing carceral health: a realist review of Canada's prison needle exchange program.

BACKGROUND: In 2018, in response to a lawsuit and years of civil society advocacy for prison-based syringe distribution due to elevated rates of injection drug use, HIV, and hepatitis C virus among incarcerated people, the Correctional Service of Canada (CSC) developed a Prison Needle Exchange Program (PNEP). Implementation of the PNEP has been slow and has faced significant critiques and challenges. As of early 2025, the PNEP is only available at eleven of forty-three federal prisons in Canada. Employing realist review methodology, an iterative process for synthesizing evidence concerning com

Navigating harm reduction and security mandates: Prison leadership perspectives on implementing prison needle exchange programs in Canada.

BACKGROUND: Prison needle exchange programs (PNEPs) reduce bloodborne virus transmission among people who inject drugs, yet global implementation remains limited. Canada introduced PNEPs in 2018, but uptake is low, and barriers to implementation persist. While perspectives of correctional officers, healthcare workers, and incarcerated individuals have been examined, little is known about how prison leadership shapes program delivery. Drawing on interviews informed by the Exploration, Preparation, Implementation, and Sustainment (EPIS) framework, we sought to identify the organisational, system

A Meta-analysis of the Association Between Needle Exchange Programs and HIV Seroconversion Among Injection Drug Users.

We assessed the association between different levels of needle exchange program (NEP) use and human immunodeficiency virus (HIV) seroconversion in the injecting drug user (IDU) population using meta-analysis to aggregate risk estimates from any reported cohort studies or randomized controlled trials (RCTs) in the literature. We searched the literature for articles published from January 1990 to August 2014 using Medical Subject Headings and other terms from MEDLINE® (using Ovid), Embase, ProQuest, the International Aids Society Abstract database, and the European AIDS clinical society database

Community support for needle exchange programs and pharmacy sale of syringes: a household survey in Baltimore, Maryland.

In October 1995, community attitudes toward needle exchange programs were assessed in Baltimore, Maryland. Household interviews were conducted with a random sample of residents living within six contiguous census tracts. Multivariate logistic regression was used to determine which factors were independently associated with acceptance of needle exchange programs. Of 274 eligible respondents contacted, 138 (50%) completed the interview. Respondents were statistically similar to the 1990 census population by income but were more likely to be female, black, between the ages of 35 and 44 years, and

Predictors of needle exchange program utilization during its implementation and expansion in Tijuana, Mexico.

OBJECTIVE: Until the early 2000s, there was only one needle exchange program (NEP) offered in Mexico. In 2004, the second Mexican NEP opened in Tijuana, but its utilization has not been studied. We studied predictors of initiating NEP during its early expansion in Tijuana, Mexico. METHODS: From April 2006 to April 2007, people who inject drugs (PWID) residing in Tijuana who had injected within the last month were recruited using respondent-driven sampling. Weighted Poisson regression incorporating generalized estimating equations was used to identify predictors of initiating NEP, while accoun

A Senior Management Perspective on the Policy Debate of Needle and Syringe Exchange Program Provision in Irish Prisons.

This study examined from an Irish perspective the contentious policy debate about providing clean needles to injecting drug users within prison systems; specifically, it examined the views of six senior managers who were prison health staff and security management. Research participants were generally opposed to the introduction of prison-based needle and syringe exchange programs (PNSP) in Ireland. They argued that (1) PNSP were unnecessary since injecting drug use within Irish prisons has declined significantly, (2) PNSP, by making needles freely available to prisoners, would make prisons ri

Factors associated with frequent needle exchange program attendance in injection drug users in Vancouver, Canada.

The objective of this study was to identify factors associated with frequent needle exchange program (NEP) attendance by injection drug users (IDUs) in Vancouver, Canada. Data were examined from a case control study of recent HIV infection. IDUs with documented HIV seroconversion after January 1, 1994 (n = 89) and seronegative controls with two documented HIV-negative test results in the same period (n = 192) were asked about demographic and social information, drug injection and sexual behavior, and NEP attendance. Logistic regression was used to examine the effect of multiple variables on NE

Transition to injecting 3,4-methylene-dioxy-pyrovalerone (MDPV) among needle exchange program participants in Hungary.

In 2011, anecdotal data indicated that 3,4-methylene-dioxy-pyrovalerone (MDPV) might become popular among needle exchange program (NEP) clients in Hungary as a possible substitute for formerly used substances such as amphetamines and heroin. The aim of the study reported here was to examine how the emergence of MDPV influenced the choice of the injecting substance among NEP clients. A total of 183 injecting drug users (IDUs) participating in the largest NEP in Budapest agreed to participate in the study and report on their drug use habits. During 2011, remarkable changes occurred in the struct
