Negative pressure wound therapy incorporating early exercise therapy in hand surgery: bag-type negative pressure wound therapy.

In the postoperative treatment of hand surgery, it is important to start exercise therapy as early as possible. In conventional negative pressure wound therapy, the fingers are immobilized by the film dressing covering the wound and hand, thereby preventing sufficient rehabilitation. Here, we devised a bag-type negative pressure wound therapy that makes it possible to start finger exercises almost immediately, and we applied it to 4 patients:one each with hand burns, symmetrical peripheral gangrene, a crush injury accompanied by extensive skin defects and a fingertip degloving injury. The dura

Negative pressure wound therapy for treating pressure ulcers.

BACKGROUND: Pressure ulcers, also known as bedsores, decubitus ulcers and pressure injuries, are localised areas of injury to the skin or the underlying tissue, or both. Negative pressure wound therapy (NPWT) is a treatment option for pressure ulcers; a clear, current overview of the evidence is required to facilitate decision-making regarding its use. OBJECTIVES: To assess the effects of negative pressure wound therapy for treating pressure ulcers in any care setting. SEARCH METHODS: For this review, we searched the following databases in May 2015: the Cochrane Wounds Group Specialised Regi

Negative pressure wound therapy for treating pressure ulcers.

BACKGROUND: Pressure ulcers, also known as bedsores, pressure sores, or pressure injuries, are localised damage to the skin and underlying soft tissue, usually caused by intense or long-term pressure, shear, or friction. Negative pressure wound therapy (NPWT) has been widely used in the treatment of pressure ulcers, but its effect needs to be further clarified. This is an update of a Cochrane Review first published in 2015. OBJECTIVES: To evaluate the effectiveness of NPWT for treating adult with pressure ulcers in any care setting. SEARCH METHODS: On 13 January 2022, we searched the Cochran

Dressings, Topical Therapy, and Negative Pressure Wound Therapy.

The current epidemic of diabetes has created a high demand for skilled wound-care professionals. Wound-care treatment begins with an appreciation of the cause of the ulceration and an adherence to the fundamental pillars of wound care. Also critical in the wound management paradigm is the optimization of the wound environment to facilitate the progression through the stages of healing. This can be accomplished through the use of different topical therapies and wound dressings to generate a favorable condition conducive to healing. This article summarizes the updated literature and best practic

Negative pressure wound therapy in orthopaedic surgery.

Negative pressure wound therapy (NPWT) consists in applying subatmospheric pressure to a wound that is sealed off by a specially designed dressing and connected by a tube to a suction pump and drainage collection system. Skin defects are extremely common in orthopaedic and trauma surgery. NPWT is valuable across a range of indications. Proven effects include an increase in blood flow, stimulation of angiogenesis, and a decrease in wound surface area. NPWT can be used to treat post-traumatic and surgical wounds, burns, and chronic wounds such as pressure sores and ulcers. The lower frequency of

Negative pressure wound therapy updates for 2021.

Negative pressure wound therapy (NPWT) has been used in clinical practice for 25 years. Worldwide, it has been used to treat more than 10 million wounds. The repertoire of NPWT procedures is still growing. This originally simple procedure entails a number of pitfalls and limits, and full utilisation of the micro-deformation potential of NPWT depends on many key details. We present the pathophysiology, effects and forms of NPWT use including our own experience, tips and a proposal for the use of NPWT during the COVID-19 pandemic.

Excessive Wound Fluid Discharge during Retroperitoneal Negative Pressure Wound Therapy.

We report a case of a 66-year-old man with an excessive wound fluid discharge during negative pressure wound therapy (NPWT) in the retroperitoneal space. Creatinine concentration of the unclear fluid and a subsequent late-phase computed tomography confirmed the diagnosis of a urinary fistula. NPWT was terminated, and the patient was successfully treated with a nephrostomy combined with a retrograde double J stent and a Foley catheter.

Case study: negative pressure wound therapy in an abdominal wound.

This article explores the history and management of large abdominal wounds and the importance of a multi-disciplinary team approach to wound healing. It identifies the difficulties of using negative pressure wound therapy (NPWT) in a patient with a large abdominal wound, which is further complicated by an ileostomy. NPWT is increasingly used for large wounds with positive outcomes, however, the case study demonstrates the many difficulties involved and how they were overcome. Cavity wounds can take weeks, or even months to heal and many patients become frustrated. This can have a significant i

Negative pressure wound therapy for open traumatic wounds.

BACKGROUND: Traumatic wounds (wounds caused by injury) range from abrasions and minor skin incisions or tears, to wounds with extensive tissue damage or loss as well as damage to bone and internal organs. Two key types of traumatic wounds considered in this review are those that damage soft tissue only and those that involve a broken bone, that is, open fractures. In some cases these wounds are left open and negative pressure wound therapy (NPWT) is used as a treatment. This medical device involves the application of a wound dressing through which negative pressure is applied and tissue fluid

Guidelines for managing pressure ulcers with negative pressure wound therapy.

Pressure ulcers are a serious health issue, leading to clinical, financial, and emotional challenges. Numerous treatment modalities are available to promote wound healing, yet clinicians may be unsure how to incorporate these treatment options into an overall plan of care for the patient with a pressure ulcer. A consensus panel of experienced wound care clinicians convened in July 2004 to review the mechanisms of action and research basis for one such treatment modality: negative pressure wound therapy. After answering key questions about this modality, they developed an algorithm to assist th

Negative pressure wound therapy reduces surgical site infections.

OBJECTIVE: Surgical site infection (SSI) with lower extremity incisions represents a modifiable source of major morbidity. Our institutional bundled care protocol to decrease SSI includes optimization of perioperative risk factors, dedicated wound closure tray, and voluntary use of a closed surface negative pressure wound therapy (cNPWT) device applied over closed incisions in the operating room. This study examined the individual effect of cNPWT on SSI reduction and other perioperative outcomes. METHODS: All patients with lower extremity or infrainguinal incisions between January 2016 and De

Negative-pressure Wound Therapy for Periocular Necrotizing Fasciitis.

Periocular necrotizing fasciitis is a rare, but potentially blinding, or even fatal disease. The authors report a case of a 44-year-old man who presented with quiescent bilateral periocular and facial necrotizing fasciitis. The patient was treated with antibiotics and surgical debridement, followed by negative-pressure wound therapy (NPWT), until the wound bed was thought to be healthy enough to support bilateral upper eyelid full-thickness skin grafts. NPWT appeared to decrease local edema; speed reperfusion and granulation tissue formation; and served to stabilize the skin grafts against the

An overview of negative pressure wound therapy (NPWT).

Negative pressure wound therapy (NPWT) is often called topical negative pressure (TNP) or by the trade name: 'VAC' (Vacuum assisted closure, KCI). NPWT is a technique that uses negative pressure applied to wounds of diverse aetiology to promote healing. Over the last 50 years clinicians have described how certain wounds respond to sub-atmospheric pressure used within a closed dynamic delivery system. Several companies are now able to offer devices that deliver NPWT, giving greater choice to decision-makers in Healthcare authorities. This article provides an overview of NPWT.

Negative Pressure Wound Therapy in Pyoderma Gangrenosum Treatment.

BACKGROUND Pyoderma gangrenosum (PG) is a rare, non-infectious, fulminant dermatosis of the skin. The lack of objective diagnostic criteria requires differential diagnosis and exclusion of extensive ulcerative skin diseases. Currently, treatment includes a combination of systemic steroids, immunosuppressants, and topical agents, but after decades of research, no clear scientific evidence exists for a criterion standard treatment. The use of NPWT (negative pressure wound therapy) effectively reduces the wound area, eliminates exudate, and reduces bacterial titer, thereby stimulating neoangiogen

Negative pressure wound therapy in spinal fusion patients.

Post-operative wound complications are some of the most common acute complications following spine surgery. These surgical site infections (SSI) contribute to increased healthcare related costs. Negative pressure wound therapy (NPWT) has long been used for treatment of soft tissue injury or defects. NPWT may reduce the incident of SSI following spinal fusion procedures; however, its potential applications need further clarification. Thus, we conducted a retrospective analysis of two cohorts to compare NPWT to traditional sterile dressings following spinal fusions in regards to post-operative o
