Intermediate trophoblast in germ cell neoplasms.

One-hundred-twenty-four germ cell tumors and 11 gestational choriocarcinomas were assessed immunohistochemically for the presence of the following placental markers: human chorionic gonadotrophin, human placental lactogen, and pregnancy-specific beta-1 glycoprotein, and for the expression of cytokeratin. A total of 40 cases contained syncytial trophoblastic cells, large mononuclear cells, or both. These cases included all gestational choriocarcinomas and choriocarcinomatous foci in 11 mixed germ cell tumors, 14 embryonal carcinomas, and four seminomas. Syncytial trophoblastic cells reactive fo

Germ cell tumours.

Germ cell tumours form an important group of gonadal neoplasms and are also found in a number of extragonadal sites like the mediastinum, para-pineal and sacrococcygeal regions and retroperitoneum. Although there are considerable differences between germ cell tumours occurring in different anatomical locations they exhibit a remarkable homology, and are considered as a group. In this review germ cell tumours are discussed as a group emphasizing some of the recent developments in this field. In the testis germ cell tumours form the most common group of neoplasms comprising 90% of all testicular

Paratesticular localization of burned out non-seminomatous germ cell tumor--NSGCT: a case report.

Testicular cancer is broadly divided into seminoma and non-seminoma types for treatment planning because seminomas are more sensitive to radiation therapy. Non-seminomatous germ-cell testis tumors represent a majority of all testicular neoplasms and include yolk sac tumor, embryonal carcinoma, choriocarcinoma, teratoma and undifferentiated tumors. Malignant neoplasms account for approximately 25% of neoplasms of the paratesticular tissues and most of them are sarcomas. We report a case of completely undifferentiated germ cell tumor of spermatic cord and discuss its diagnosis.

Germ Cell Tumors of the Female Genital Tract.

Ovarian germ cell tumors are a histologically diverse group of neoplasms with a common origin in the primitive germ cell. The vast majority are represented by mature cystic teratoma. In the minority are malignant germ cell tumors including immature teratoma, dysgerminoma, yolk sac tumor, embryonal cell carcinoma, and choriocarcinoma. This article reviews the histologic and immunohistochemical features of the most common ovarian germ cell tumors. The differential diagnoses for each are discussed.

Germ cell neoplasms of the testis: Update for 2022.

Germ cell neoplasia in situ (GCNIS) is the precursor of both seminomatous and non-seminomatous germ cell tumors. It consists of distended tubules that may have either intratubular seminoma or intratubular embryonal carcinoma cells. Many invasive non-seminomatous tumors contain a mixture of tumor types, which are reviewed here. Morphology, aided by a panel of immunostains, can determine the presence and percent of embryonal carcinoma, yolk sac tumor, choriocarcinoma, or teratoma in such tumors. Use of immunostains, required for diagnosis in perhaps 25% of testicular neoplasms, is reviewed. Chan

A primary germ cell tumor of the anterior mediastinum: a case report and discussion.

The anterior mediastinum is the most common extragonadal location for germ cell tumors and accounts for about 50% to 70% of such neoplasms. Embryonal cell carcinomas are one of the rarest forms and account for less than 2%. We present the case of a 19-year-old, white male who was found to have a primary embryonal cell carcinoma of the anterior mediastinum. This case illustrates the subtle complaints that these patients present with, some of the problems and decisions that go into making the diagnosis, and the response to the appropriate therapy. The following discussion takes a look at the var

Involvement of dysadherin and E-cadherin in the development of testicular tumours.

Testicular neoplasms are comprised of a variety of histologically different forms, and their pathogenesis has not been elucidated. Dysadherin is a recently described cell membrane glycoprotein, which has an anticell-cell adhesion function and downregulates E-cadherin. In this study, we examined immunohistochemically the expression of E-cadherin and dysadherin in 120 testicular neoplasms (37 seminomas-26 classic, five spermatocytic and six anaplastic-, 45 embryonal carcinomas, 10 mixed germ cell tumours, two yolk sac tumours, 10 mature and eight immature teratomas and eight non-Hodgkin B-cell l

Differentiation patterns of testicular germ-cell tumours as revealed by a panel of monoclonal antibodies.

A panel of monoclonal antibodies against different keratin polypeptides, epithelial glycoproteins, placental alkaline phosphatase and collagen type IV was used to evaluate immunohistochemically the expression of the target antigens in 30 different human testicular germ-cell tumours of various types. Antikeratin antibodies detecting markers of different routes of epithelial differentiation revealed remarkable similarity of differential expression of various keratins in epithelial structures of teratomas and combined tumours as compared with normal human epithelial tissues. Mediastinal tumors.

Mediastinal tumors represent a wide spectrum of epithelial, mesenchymal, neuroectodermal, embryonal, germ cell, and mixed neoplasms. Nevertheless, their incidence is low, accounting for approximately 1% of all neoplasms, and clinical presentation is often nonspecific usually reflecting advanced-stage disease. Primary mediastinal tumors must be distinguished from secondary tumors and so-called pseudotumors. From the perspective of their classification, diagnostics, and therapy, the anatomical--topographical division of the mediastinum into compartments is essential, as it may facilitate identif

Embryonal carcinoma of the ovary: a clinicopathologic entity distinct from endodermal sinus tumor resembling embryonal carcinoma of the adult testis.

The clinical and pathologic features of 15 examples of a hitherto undescribed germ cell tumor of the ovary are delineated. This tumor resembles the embryonal carcinoma of the adult testis and may be distinguished from the endodermal sinus tumor on the basis of its histologic and immunohistochemical characteristics. An indirect immunoperoxidase method for the localization of human chorionic gonadotropin (HCG) and alpha-fetoprotein (AFP) was done on formalin-fixed paraffin-embedded tissue from 10 neoplasms; HCG was present in all 10 neoplasms, and AFP was found in seven. HCG was indentified only

Medulloblastoma arising from an immature teratoma of the posterior fossa. Case report.

Teratomas, a group of nongerminomatous germ cell tumors, account for 3% of all childhood tumors. These unusual lesions reproduce the cellular and structural phenotypic traits associated with the three classic germ layers in inappropriate places in the body and often retain an embryonal character. These immature cells can differentiate into more malignant neoplasms. An intracranial location for this lesion type is rare. The authors describe the case of a 2-year-old boy with a teratoma of the posterior fossa that had partially differentiated into a medulloblastoma.

Pathology of pineal region tumors.

Various histological types of tumors arise in the pineal region. The most common tumors are pineal parenchymal tumors and germ cell tumors. Pineal parenchymal tumors are divided into pineocytoma, pineal parenchymal tumor with intermediate differentiation and pineoblastoma. Pineocytomas are well-differentiated tumors and retain the morphological and immunohistochemical features of pineal parenchymal cells. Lobular architectures and pineocytomatous rosettes are also typical features. In contrast, pineoblastomas are embryonal tumors resembling primitive neuroectodermal tumors (PNET). However, pin

Embryoid Bodies and Related Proliferations in Ovarian Germ Cell Tumors.

We investigated the frequency and associated pathology of embryoid bodies in ovarian tumors by evaluating neoplasms in which they are known to occur: 100 immature teratomas, 125 malignant mixed germ cell tumors, and 6 polyembryomas. Three immature teratomas contained a single relatively well-formed embryoid body, whereas these and 11 others showed foci we categorized as embryoid body remnants consisting of microscopic aggregates of embryonal or yolk sac-type epithelium associated with spaces consistent with yolk sac or amniotic cavity but lacking a classic embryoid body structure. Teratomas wi

From the archives of the AFIP: tumors and tumorlike lesions of the testis: radiologic-pathologic correlation.

Testicular carcinoma represents only 1% of all neoplasms in men, but it is the most common malignancy in the 15-34-year-old age group. Germ cell tumors constitute 95% of all testicular tumors. Germ cell tumors are a varied group of neoplasms whose imaging features reflect their underlying histologic characteristics. Seminomas are generally well-defined homogeneous lesions, whereas the nonseminomatous tumors (embryonal carcinoma, yolk sac tumor, choriocarcinoma, teratoma, and mixed germ cell tumor) have a much more varied appearance. Germ cell tumors follow a predictable pattern of spread via t

Immunohistochemical demonstration of placental alkaline phosphatase in various states of testicular development and in germ cell tumours.

Immunohistochemical localization of placental alkaline phosphatase (PlAP) has been performed on eighty-two samples of normal (embryonic, fetal, infantile or adult), cryptorchid or tumorous testicular tissue. The isoenzyme could be demonstrated at the cell membrane of primitive, embryonic germ cells but not in other normal tissues. In-situ carcinomas and seminomas were positively stained in 93% and 94% of cases respectively. The percentage of positivity decreased with tumour differentiation. It is suggested that PlAP is a marker of primitive germ cells and that it reappears in germ cell neoplas

Germ cell tumors of the gonads: a selective review emphasizing problems in differential diagnosis, newly appreciated, and controversial issues.

Gonadal germ cell tumors continue to be the cause of diverse, diagnostically challenging issues for the pathologist, and their correct resolution often has major important therapeutic and prognostic implications. They are academically interesting because of the biological diversity exhibited in the two gonads and variation in frequency of certain neoplasms. The most dramatic examples of the latter are the frequency of dermoid cyst in the ovary compared to the testis and the reverse pertaining to embryonal carcinoma. Within the teratoma group, there is strong evidence that ovarian and prepubert

Immature teratoma at umbilicus region presenting as exomphalos: A case report with review of literature.

Umbilical cord neoplasms are rare. Teratomas, which are accepted as the only true neoplasms at this site, are exceptional. These tumors are polymorphic in their presentation and are often associated with other abnormalities, with exomphalos being the most common abnormality. Histologically these tumors show mature tissues from all the three-germ layers. Immature elements as seen in our case are rare. Germ cells which have gone astray during embryonic development are thought to be the cells of origin of these tumors.

Malignant mixed germ cell tumors of the ovary. A clinical and pathologic analysis of 30 cases.

The clinical and pathologic features of 30 ovarian mixed germ cell tumors (neoplasms containing combinations of malignant germ cell elements) were studied to determine their behavior and to compare them with pure forms of malignant germ cell tumors. Dysgerminoma was the most common constituent, found in 24 (80%), followed by endodermal sinus tumor in 21 (70%), teratoma in 16 (53%), choriocarcinoma in 6 (20%), and embryonal carcinoma in 5 (16%). The actuarial survival for the entire group was 46%, and for patients with Stage I tumors it was 50%. The most important factors in predicting the prog

Diagnosis and management of a rare bilateral ovarian mixed germ cell tumor: a case report.

BACKGROUND: Mixed ovarian malignant germ cell tumors (MOGCTs) are rare neoplasms composed of two or more malignant germ cell components, representing less than 1% of all ovarian germ cell tumors. They primarily affect adolescents and young women, presenting a clinical challenge due to their histologic heterogeneity, potential for recurrence, and the need to balance oncologic safety with fertility preservation. CASE PRESENTATION: We reported a 22-year-old woman diagnosed with a four-component MOGCT in the right ovary-comprising yolk sac tumor, immature teratoma, embryonal carcinoma, and dysger

Germ cell neoplasms of the testis.

Testicular germ cell neoplasia, a disease predominantly of young men, is, for unknown reasons, increasing in incidence. Cryptorchidism, a prior testicular germ cell tumor, a family history of testicular germ cell tumors, and somatosexual ambiguity syndromes remain well-established risk factors. Intratubular germ cell neoplasia of the unclassified type represents the common precursor to the great majority of testicular germ cell tumors, and its identification in testicular biopsies reliably identifies those patients who will often progress to an invasive lesion. Seminoma appears to represent th
