Sixty three cases (male 32, female 31) of supravesical obstruction were treated with ultrasonically guided subcutaneous nephrostomy at our hospitals from October 1980 to October 1984. Their age ranges from 2 months to 84 years old, and median age was 52 years old. The major causes of ureteral obstruction were extended pelvic malignancies, 45 cases (71%) (gynecological 18, urological 16, gastrocolorectal 7 and others 4), and benign disease 18 cases (29%). Objectives of nephrostomy were classified in 4 categories (drainage, manipulation, irrigation and evaluation), and results of nephrostomy wer

A safe and effective method for long-term relief of renal obstruction is presented. This method has been successful in buying time for patients in stiuations in which other approaches are hazardous or more complex. Its use can be modified for short-term therapeutic drainage of the upper urinary tract.

A review was made of 127 percutaneous nephrostomies (PCN) in 106 patients with benign (74 patients) and malignant (32 patients) urinary tract obstruction. Data on underlying aetiology, complications, subsequent management and long-term management are presented. The study confirms that PCN is an effective and safe method of providing temporary or prolonged urinary diversion in obstruction. Its use in pelvic malignancy should be determined by the likelihood of obtaining worthwhile prolonged palliation in individual patients.

Percutaneous nephrostomy is a simple technique for temporary drainage of an obstructed kidney. Under local anaesthesia a ureteric catheter is passed through a Vim-Silverman needle into the renal pelvis and is connected to a drainage bag. Seven cases (six successful) in which this method was used are described and the indications are discussed. It has been free from complications, is acceptable to patients, and is felt to represent a useful addition to the methods available for the treatment of obstruction of the upper urinary tract.

The primary indication for the urgent percutaneous nephrostomy is to relieve an obstructed and infected renal collecting system (pyonephrosis). Percutaneous nephrostomy catheter placement is a safe procedure with a high technical success rate. This article will discuss all aspects regarding the emergent placement of nephrostomy catheters, including the indications, techniques, results, and complications. Differences between emergent and nonemergent placement of percutaneous nephrostomy catheters will also be addressed.

Percutaneous US guided nephrostomy is the simplest and most direct technique to drain an obstructed kidney. The indications are included in two groups: temporary drainage and permanent drainage; the former is indicated in the non endoscopically superable ureteral obstruction, in pyonephrosis, in pregnant women and in transplanted kidneys (due to the easier access), the latter is exclusively reserved to neoplastic obstructions. The only real contraindication to the method, besides a documented allergy to local anaesthetics, is represented by a severe coagulopathy. Positioning techniques are the

Percutaneous nephrostomy is increasingly used for temporary relief of upper urinary tract obstruction. The technique described involves the use of angiographic catheters and guides, and will provide drainage for several weeks or months. The patient is placed in the prone oblique position after intravenous contrast to opacify the kidneys. The oblique position ensures that a needle advanced vertically toward the kidney will pass along a posterior oblique line, entering the outer aspect of the kidney away from the hilum and directing guide wires and catheters toward the ureter. A fine needle is p

A prospective controlled trial of the effectiveness of a cefoxitin-bonded nephrostomy catheter was undertaken to determine the effectiveness of an antibiotic bonded catheter in decreasing the infectious complications of percutaneous nephrostomy. The study concludes that bonding of the antibiotic cefoxitin to percutaneous nephrostomy catheters did not influence the incidence of bacteriuria or urinary tract infection. In addition, observations on the overall incidence of complications from percutaneous nephrostomy are made.

Ultrasonically guided percutaneous nephrostomy was planned in 98 patients with a total of 128 nephrostomies. The success rate was over 90%. The catheters were maintained from a 1/2 day to 150 days with a median duration of 13 days. 26% of the patients had the catheter replaced, mainly because of displacement. Two major complications were seen, one perforation of the gallbladder and one case of major haemorrhage resulting in a nephrectomy. Two of the 26 patients infected at the puncture died from sepsis. Among cases with low ureteral obstruction untreated prostatic carcinoma and ureteral tumors

In the authors department in the course of six years 38 percutaneous nephrostomis were performed. The most frequent indication was renal failure and infection of the urinary pathways in obstruction of the ureter. Puncture of the hollow renal system was performed in the majority of patients under ultrasonographic control from a posterolateral approach and dilatation of the nephrostomic canal under intermittent X-ray control. After dilatation into the renal pelvis a nephrostomic catheter was inserted. The success rate of the procedure was 95%. In two patients percutaneous nephrostomy failed and

A series of 76 pyonephrotic kidneys in 73 patients were drained by percutaneous nephrostomy (PN) tube and examined to evaluate the contribution of this technique to the treatment of pyonephrosis. In 71 patients, clinical symptoms disappeared 24-48 h after the procedure. Two patients died from sepsis subsequent to anuria and underlying malignancy. Once the acute phase had remitted, interventional procedures were carried out in 39 cases, and constituted the definitive therapy in 36. In 32 cases, elective surgery was the definitive therapy, including the 3 cases not resolved after interventional

Percutaneous full-thickness incision and stenting of the ureteropelvic junction (endopyelotomy) relieved obstruction in 33 (87%) of 38 patients treated over a 2-year period. Proper placement of the percutaneous nephrostomy tract through a posterior middle calyx and of a guidewire across the ureteropelvic junction is necessary in order to gain access to the narrowed area with a rigid cutting instrument. Except in patients with long lesions, high insertion of the ureter, or an enormously redundant renal pelvis, endopyelotomy gives excellent results with less morbidity and a shorter recovery time

INTRODUCTION: Percutaneous nephrostomy has traditionally been performed with the patient in the prone position, probably to reduce the risk of injury to adjacent visceral organs, particularly the colon. The prone position, however, is associated with disadvantages such as patient discomfort and circulatory and ventilatory difficulties, particularly in obese patients. We describe a technique of percutaneous nephrostomy with the patient in the supine anterolateral position using local anesthesia, ultrasound-guided puncture, and fluoroscopy-controlled placement.
TECHNICAL CONSIDERATIONS: The sup

Percutaneous nephrostomy may serve as an alternative to an open operation for urinary diversion. Difficulties with poor drainage and premature catheter dislodgement have prompted the development of a technique for dilation of the needle tract and placement of a large, self-retaining catheter at the time of the initial puncture.

Percutaneous nephrostomy is a useful method of supravesical diversion when ureteral obstruction occurs at initial diagnosis of gynecologic cancer or recurrence of disease. Although survival is prolonged, the attendant quality of life would indicate that percutaneous nephrostomy should be performed only in those cases in which there is a reasonable chance for palliation or a hope for cure or when temporization is needed to define adequately the etiology of ureteral obstruction following therapy.

Percutaneous nephrostomy procedures generally are safe. The associated mortality rate is approximately 0.04%, and the incidence of important complications is 5% (2-4). To minimize complications, certain precautions always should be followed. First, radiologists should perform a preprocedural evaluation of the patient, with correction of marked coagulopathy or thrombocytopenia before all but the most emergent procedures. Second, antibiotics should be administered routinely before nephrostomy drainage; the choice of antibiotics can be based on the specific patient's risk factors for bacteriuria.

OBJECTIVE: To assess survival length and quality of life in patients with a percutaneous nephrostomy (PN) for malignant tumour and postrenal anuria.
PATIENTS AND METHODS: Case records were analysed of 50 patients (39 men, eleven women; mean age 65.6 years) with postrenal anuria due to malignancy who had undergone ultrasound-directed PN under local anaesthesia for urinary drainage. Particular attention was paid to the course of the disease from nephrostomy until death.
RESULTS: Survival time after PN ranged from 4 days to 16 months, averaging 4.3 months. 26 patients (52%) had died after 3 mon
