This preliminary report discusses a method which unites the advantages of the surgical procedures introduced by Gil Vernet and Boyce for the transparenchymal removal of staghorn calculi. The significance of the length of the incision involving the parenchyma and the reduced thickness of the incised parenchyma are stressed. The urine collecting system (pyelocalyceal system) is explored through the renal pelvis making all the calyces well visible. This case report describes the use of computed tomography for the diagnosis and surgical planning in a horse with bilateral nephrolithiasis. An adult Warmblood stallion (460 kg) was presented with chronic weight loss, hypercalcemia and azotemia. The results of the operative treatment of 21 large longitudinal nephrotomies on account of the existence of cast calculi of the renal calyx are reported and discussed. Dioctophymosis is a zoonotic disease caused by Dioctophyme renale, a nematode that can reach up to 100 cm in length and is commonly known as the "giant kidney worm." A technique for performing antegrade pyelography during nephrotomy is described--puncture with a 21-gauge, 20-cm Cope needle, placement of a 0.018-inch SMG mandril "coat-hanger"-shaped guidewire, with the needle exchanged for a 3-French multi-sidehole dilator. Use of the 3-French dilator provides more security and less trauma to the renal pelvis; nephrotomy time is also curtailed. This paper reports on two cases of laparoscopic nephrotomy employed in the treatment of canine dioctophymosis, which is considered a unusual procedure and a new treatment proposal heretofore not performed in veterinary medicine. The authors report a study of 37 kidneys in 30 patients with staghorn calculi undergoing isotopic investigation of renal function by sequential scintigraphy and isotopic nephrogram by gamma-camera. 7 of the patients had bilateral staghorn calculi. Open surgery for stones of the upper urinary tract has very few indications, failures or complications of other techniques, greater than 2cm stones, hard stones, anatomical abnormalities and complex stones. Open surgery for stone may be difficult and need specific tools. For the kidney, the anatrophic nephrotomy is an effective procedure which spares the renal function. For the ureter, the mini-ureterotomy is no more invasive than laparoscopy. The stone-free rate of open surgery is over 90%. The main complications are stenosis, fistula and infections.

INTRODUCTION: The management of caliceal diverticular calculi has changed from an open surgical approach to endoscopic management.
TECHNICAL CONSIDERATIONS: Different minimally invasive treatment modalities, such as extracorporeal shock wave lithotripsy, ureteroscopy, percutaneous nephrolithotomy, laparoscopy, and retroperitoneoscopy, can be offered to the patient. We report on a retroperitoneoscopic operative technique using endosonography for location and performing nephrotomy with complete excision of the caliceal diverticulum and ligation of the diverticular neck with an Endo-loop.
