Nerve conduction and electromyography studies.

Nerve conduction studies (NCS) and electromyography (EMG), often shortened to 'EMGs', are a useful adjunct to clinical examination of the peripheral nervous system and striated skeletal muscle. NCS provide an efficient and rapid method of quantifying nerve conduction velocity (CV) and the amplitude of both sensory nerve action potentials (SNAPs) and compound motor action potentials (cMAPs). The CV reflects speed of propagation of action potentials, by saltatory conduction, along large myelinated axons in a peripheral nerve. The amplitude of SNAPs is in part determined by the number of axons in

Radial motor nerve conduction studies.

The radial motor nerve fibers were studied in 30 subjects using a technique of surface stimulation in the axilla and the antecubital fossa. Surface recording over the extensor digitorum communis 8cm from the distal stimulation site was done. Mean distal latency was 2.6msec (SD = 0.44), amplitude 11.24mV (SD = 3.5), and conduction velocity 68m/sec (SD = 7.0). Side-to-side comparison using the t-test demonstrated no significant differences. Five subjects were tested 75 more times on two successive days and showed a variation in latency of 0.2msec or less in 74 of 75 trials. Amplitude was consist

Electrodiagnosis and nerve conduction studies.

The use of electrodiagnostic techniques in evaluation of complaints in the lower extremities provides an objective method of assessment. A basic understanding of principles of neurophysiology, EMG and NCV methodology, and neuropathology of peripheral nerves greatly enhances physical diagnosis and improves the state of the art in treatment of the lower extremity, especially foot and ankle injuries. Familiarity with the method of reporting electrodiagnostic studies and appreciation of the electromyographer's interpretation of the EMG/NCV studies also reflects an enhanced fund of knowledge, skill

Clinical nerve conduction and needle electromyography studies.

The electrodiagnostic study, consisting of nerve conduction studies and needle electromyography, is a useful adjunct to the clinical examination of the peripheral nervous system. The three types of nerve conduction study are motor, sensory, and mixed, of which motor is the least sensitive. Electromyography records the intrinsic electrical activity of muscle fibers, thus providing the physiologic status of muscle function. To interpret the electrodiagnostic study results, the clinician must understand the anatomic and physiologic basis of the studies. Peripheral nerve entrapment initially resul

Nerve conduction studies and their clinical applications.

Nerve conduction studies provide the most sensitive and accurate account of peripheral neuropathophysiology. Though nerve conduction studies should never be performed in lieu of a clinical examination, the electrodiagnostic evaluation provides an optimal adjuvant to the clinical examination. Indications for nerve conduction studies are many, including evaluation of the nature of the pathophysiology, quantification of the severity of involvement, detection of the level of a neurologic deficit, and determining prognosis. Among other indications, limitations of the study should also be acknowledg

Nerve conduction studies in early tuberculoid leprosy.

CONTEXT: Hansen's disease is a chronic illness; besides involving skin and peripheral nerves, it affects multiple organs. Nerve involvement is always present in leprosy, and it may be present much before the patient manifests clinically. AIMS: To assess nerve conduction parameters in thickened and contralateral non-thickened nerves in early tuberculoid leprosy. MATERIALS AND METHODS: Fifty new untreated male patients with tuberculoid and borderline tuberculoid leprosy in the age group of 15-50 years with thickened peripheral nerves on one side were included in the study. Nerve conduction stu

Sensory nerve conduction studies in neuralgic amyotrophy.

Neuralgic amyotrophy is a painful, episodic peripheral nerve disorder localized to the brachial plexus. Sensory symptoms occur in 80% of the patients. We assessed the frequency of abnormalities in sensory nerve conduction studies of the lateral and medial antebrachial cutaneous, radial sensory, median sensory, and ulnar sensory nerves in 112 patients. Sensory nerve conduction studies showed abnormalities in <20% of nerves, even when the nerve was clinically affected. The lateral and medial antebrachial cutaneous nerves were most often abnormal, in 15% and 17% of nerves. No correlation with the

Correlating Median Nerve Cross-sectional Area With Nerve Conduction Studies.

PURPOSE: To determine whether there is a correlation between the cross-sectional area (CSA) of the median nerve, as measured using ultrasound, and the distal motor and/or sensory latencies as measured on nerve conduction studies. METHODS: Patients with clinical signs and symptoms of carpal tunnel syndrome were prospectively enrolled in this study. Subjects underwent ultrasound measurement of the CSA of the median nerve at the carpal tunnel inlet by a fellowship-trained hand surgeon, followed by nerve conduction studies (NCS) by a certified electrodiagnostic technician who was blinded to the r

Normal values of sensory conduction velocities (CV) in four routinely examinated peripheral nerves are presented median, ulnar, radial and sural were gathered from the intact side of children with unilateral brachial plexus or different peripheral nerve lesions aged from two weeks to 15.5 years. The data were computer treated using an exponential function as the one which best reflects the dynamics of CV age-related changes i.e. its neurophysiological maturation. Approximated exponential curves and standard error, 95 per cent of confidence limits were computer-fitted to CV values and computer-

Carpal tunnel syndrome with normal nerve conduction studies.

The utility of electrodiagnostic testing in the evaluation of carpal tunnel syndrome (CTS) has been questioned. We studied patients who met the clinical criteria for CTS and compared patients who had normal nerve conduction studies (NCS) with patients who had abnormal NCS. We found that 25% of the CTS patients without confounding neurologic disorders had normal NCS with median palmar nerve stimulation. Patients with abnormal NCS were older and heavier and had more clinical features of CTS. NCS results could not be predicted accurately from clinical features by use of logistic regression models

Endoscopic carpal tunnel release and nerve conduction studies.

We investigated the outcome of endoscopic carpal tunnel release (ECTR) for patients with carpal tunnel syndrome (CTS) in comparison with the results of preoperative nerve conduction studies. The compound muscle action potential (CMAP) of the abductor pollicis brevis muscle (APB) and the second lumbrical muscle (L2) was recorded following median nerve stimulation at the wrist. A total of 38 hands in 35 patients were classified into four categories. Hands with a similarly prolonged distal motor latency for the APB and L2 were classified as type I (n=25), while those with a more prolonged distal

Peripheral nerve conduction studies in passively addicted neonates.

Neurologic signs dominate the manifestations of the neonatal abstinence syndrome (NAS). To help delineate this dysfunction, peripheral nerve conduction studies (NCS) were made in 25 neonates born to methadone-maintained mothers; 12 of the mothers abused other controlled substances concomitantly. Median and common peroneal motor nerve conduction velocities (NCV) in these infants were normal, both at three to seven days and three to four weeks of age, and were unaffected by maternal drug intake pattern, severity of neonatal abstinence symptoms, treatment with either camphorated tincture of opium

Antidromic vs orthodromic sensory median nerve conduction studies.

OBJECTIVE: Median sensory nerve conduction studies are arguably the most often performed electrodiagnostic tests worldwide. Routine tests in clinical practice are done using either antidromic or orthodromic techniques type of stimulation, with no universal agreement on the use of one or the other technique. METHODS: We review the advantages and drawbacks of antidromic and orthodromic as well as their particularities for clinical application and research. RESULTS: The two techniques differ on how physical and physiological changes affect the action potential. Near-nerve recording is better su

[Nerve conduction studies of suprascapular nerve for site-of-lesion diagnosis].

Shoulder pain is a common symptom, resulting not only from bone and shoulder joint diseases, but also from neurogenic lesions. Entrapment neuropathy of the suprascapular nerve also causes shoulder symptoms. Conduction of the suprascapular nerve was studied in 12 healthy control subjects and 25 patients suffering from shoulder pain and/or dysfunction. Surface stimulation was performed at Erb's point, and compound muscle action potentials(M waves) were recorded from the supraspinatus and the infraspinatus muscles with concentric needle electrodes. To determine the optimal site for recording M wa

Use of nerve conduction studies in carpal tunnel syndrome.

This review article examines the use of nerve conduction studies in the management of carpal tunnel syndrome. These studies should be understood not as a test that determines the diagnosis but as a measure of impaired nerve function. They are sensitive indicators of local demyelination and axonal loss that can detect and quantify these changes before the appearance of clinical signs, providing information that cannot be obtained with the unaided senses of the physician, nor by any other investigation. They are the best available indicator of overall disease severity, correlating with symptoms
