[Neuropathy and unmyelinated epidermal nerve fibers].

Detection of unmyelinated epidermal nerve fibers allows objective, pathologic, and minimally invasive assessment of small myelinated A-delta and unmyelinated C primary afferent fibers. This procedure is especially valuable for diagnosing small fiber neuropathy; this condition is characterized by sensory symptoms and pain in the lower extremities but standard nerve conduction fails to show abnormalities in this condition. Detection of epidermal nerve fibers is useful not only for confirming the diagnosis of neuropathies affecting small-diameter sensory fibers, but also in advanced clinical care

Simultaneous Quantification of Unmyelinated Nerve Fibers in Sural Nerve and in Skin.

Peripheral polyneuropathies are common and their diagnosis may be challenging. We compared the results from sural-nerve and skin biopsies in 33 patients with a polyneuropathy and neuropathic pain examined in our hospital over a 6-year period. The biopsies were all from the same lower limb of each patient. Intraepidermal nerve fiber (IENF) densities in the skin were determined by fluorescence microscopy; unmyelinated fiber densities in sural-nerve biopsies (UFNB) were determined by electron microscopy. There was no correlation with age or gender in either biopsy type; there was a weak trend to

Microneurographic recording from unmyelinated nerve fibers in neurological disorders: an update.

Microneurography is a unique neurophysiological technique allowing direct recording of unmyelinated postganglionic sympathetic or afferent nociceptive fibers by tungsten needles inserted into a peripheral nerve fascicle. In recent years, microneurography has been used to ascertain autonomic impairments in central neurological disorders such as sleep disorders, Parkinson's disease, amyotrophic lateral sclerosis, or vasovagal syncope. Abnormal resting muscle sympathetic nerve activity (MSNA) and skin sympathetic nerve activity (SSNA) or the abnormal sympathetic response to arousal have been desc

Unmyelinated nerve fibers distributed in the human crural fascia of Thiel-embalmed cadavers.

Muscle fascia is implicated to play a role in musculoskeletal pain; however, the distribution of unmyelinated nerve fibers within human fascia remains to be characterized. Conventional formalin fixation of biological tissues for long periods degrades the antigenicity of protein markers, often resulting in loss of nerve fiber immunoreactivity, limiting immunohistochemical analysis. This study aimed to examine the intrafascial distribution of immunohistochemically labeled unmyelinated nerve fibers in the muscle fascia of cadavers embalmed for 4-21 months using the Thiel-embalming method, which e

Three dimensional analysis of unmyelinated fibers in normal and pathologic autonomic nerves.

The technique of serial-section electron microscopy and diagrammatic three dimensional reconstructions has been used to assess normal and pathological unmyelinated nerve fibers from a peripheral autonomic nerve: the rat cervical sympathetic trunk. Within this predominantly unmyelinated nerve, there is a complex arrangement of axons into longitudinally oriented bundles brought together by chains of Schwann cells. Each bundle is subdivided into smaller components by the cytoplasmic processes of Schwann cells; such subdivisions, which are basal lamina-enclosed masses of Schwann cell cytoplasm, wh

Vulnerability study of myelinated and unmyelinated nerve fibers in acute ocular hypertension in rabbit.

In the current study, it was aimed to evaluate the changes in myelinated and unmyelinated nerve fibers in retinal ischemia‑reperfusion injuries caused by acute ocular hypertension and to determine the sequence of these changes. Adult healthy New Zealand white rabbits were randomized to the hemodynamic group [n=12; used to determine the optimal intraocular pressure (IOP) for the subsequent experiments] and the hypertension group (n=6; 70‑mmHg hypertension induced in one eye). IOP was adjusted using a cannula and saline. Doppler ultrasound was used to measure the velocity of the optic artery und

Myelinated and unmyelinated fibers in sural nerve biopsy of a case with lepromatous leprosy--a quantitative approach.

A 64-year-old Cambodian male with lepromatous leprosy of several months' duration, with only brief previous treatment, had symmetrical patchy loss of pain and thermal sensibility including the lateral foot regions. Quantitative assessment by light and electron microscopy revealed normal counts of myelinated fibers (MF) and an increased number of unmyelinated fibers (UF). The respective abnormal histograms and the qualitative differentiation of UF-associated Schwann cells suggested a combination of segmental demyelination with Wallerian degeneration of MF, and a loss of genuine UF, concomitant

Peripheral nerve in type III glycogenosis: selective involvement of unmyelinated fiber Schwann cells.

Electron microscopy of intramuscular nerves in biopsy material from a child with glycogenosis type III showed massive glycogen accumulation in the Schwann cells of unmyelinated nerve fibers. Other cells, including Schwann cells of myelinated fibers, perineurial cells, endoneurial fibroblasts, endothelial cells, and pericytes, were not similarly affected.

Anodally focused polarization of peripheral nerve allows discrimination of myelinated and unmyelinated fiber input to brainstem nuclei.

We investigated the ability of a novel direct current (DC) polarization technique to block selectively the conduction in peripheral myelinated nerve fibers and allowing propagation in only unmyelinated fibers. In anesthetized adult rats, distal branches of the sciatic nerve (caudal cutaneous sural and tibial nerves) were exposed for electrical stimulation of A- and C-fibers. Two specially fabricated trough electrodes of different size and surface area were placed onto the sciatic nerve. Through these proximal electrodes a controlled ramped DC was timed to coincide with the arrival of A- and C-

Distinct activation thresholds of unmyelinated C-fiber afferents by dorsal root ganglion and peripheral nerve stimulation.

OBJECTIVES: Sensitization of C-fiber nociceptors plays a critical role in spontaneous and ongoing pain in patients with chronic pain. We recently demonstrated that C-fiber afferents can be reversibly blocked through activity-dependent conduction slowing, suggesting that selective activation of C-fiber afferents may represent a novel strategy for pain relief. We hypothesized that electrical peripheral nerve stimulation (ePNS) and dorsal root ganglion (DRG) stimulation exhibit distinct activation thresholds for unmyelinated C-fiber afferents. MATERIALS AND METHODS: We characterized the activati

Pressing Intervention Alleviates Pain in Rats With Myofascial Trigger Points via the TRPV1 Channel in Unmyelinated C-Type Sensory Nerve Fibers.

BACKGROUND: Although pressing intervention is widely used clinically to alleviate pain associated with chronic myofascial trigger points (MTrPs), the mechanisms by which it modulates pain via sensory nerves remain unclear. This study aimed to investigate the effects of pressing intervention on transient receptor potential vanilloid 1 (TRPV1) channels in sensory nerves and to explore its potential analgesic mechanisms. METHODS: Twenty-six male Sprague-Dawley rats were randomly divided into a blank group (n = 6) and a model establishment group (n = 20). Chronic MTrPs were induced in the model g

Human organ donor-derived vagus nerve biopsies allow for well-preserved ultrastructure and high-resolution mapping of myelinated and unmyelinated fibers.

The vagus nerve provides motor, sensory, and autonomic innervation of multiple organs, and electrical vagus nerve stimulation (VNS) provides an adjunctive treatment option for e.g. medication-refractory epilepsy and treatment-resistant depression. The mechanisms of action for VNS are not known, and high-resolution anatomical mapping of the human vagus nerve is needed to better understand its functional organization. Electron microscopy (EM) is required for the detection of both myelinated and unmyelinated axons, but access to well-preserved human vagus nerves for ultrastructural studies is spa

Selective fiber vulnerability in acute ischemic neuropathy.

To study the relationship between fiber size and vulnerability to ischemia, we produced partial infarction of the proximal posterior tibial nerve of Wistar rats by injecting arachidonic acid into the ipsilateral femoral artery. In the resulting lesion degenerating fibers were concentrated in the center of the nerve. Distal to the infarct, surviving myelinated and unmyelinated fibers were evenly distributed throughout both fascicles. Smaller myelinated fibers (less than 6 micrometers in diameter) were more severely depleted than larger ones ( greater than 6 micrometer). Unmyelinated fibers were

[Role of transthyretin in posttraumatic regeneration of the sciatic nerve in mouse].

Although thyroid hormones are known to have a significant influence on the development of nervous system, the absence of changes in the brain of mice deficient in transthyretin--a protein providing thyroid hormone transport across the blood-brain and blood-nerve barrier--remains unexplained. Therefore, the aim of this study was to determine the effect of transthyretin on the formation of myelinated and unmyelinated nerve fibers in sciatic nerve of mice. The myelinated and unmyelinated nerve fibers were counted in sciatic nerve of 3-months-old normal and transthyretin-knockout (transthyretin(-/

Photochemically-induced ischemic injury of the rat sciatic nerve: a light- and electron microscopic study.

Lesion of presumably ischemic origin of the rat sciatic nerve was induced photochemically by laser irradiation combined with systemic administration of a photosensitizing organic dye, erythrosin B. We have studied the pathologic features of the nerve after the photochemical insult with light- and electronmicroscopy and related them to behavioral signs of neuropathic pain. At the irradiated nerve site, occlusion of blood vessels was seen and the vessels were packed with aggregated thrombocytes, fibrins and deformed erythrocytes, supporting the notion that photochemical reaction caused intraneur

Unmyelinated axons in a muscle nerve. Electron microscopic morphometry of the sternomastoid nerve in normal and sympathectomized rats.

In continuation of earlier studies on the innervation of the sternomastoid muscle of the rat, a detailed morphometric analysis was performed on the unmyelinated axons of the nerve, in normal rats and after extensive cervical sympathectomy. In 4 normal rats an average of 314 myelinated and 319 unmyelinated axons were present. 42 days after surgery, the 3 animals showed Horner's syndrome and a highly significant 40% loss of unmyelinated axons. We therefore suggest that 40% of the C-fibers in this nerve are postganglionic sympathetic efferents and that the remaining 60% are type IV fibers, i.e.,

Effects of 4-methylcatechol on skin reinnervation: promotion of cutaneous nerve regeneration after crush injury.

We assessed the effects of treatment with 4-methylcatechol (4MC), a known inducer of nerve growth factor, on peripheral nerve regeneration by analyzing cutaneous and muscular reinnervation in mice after sciatic nerve crush injury. At 3 months postinjury, the skin innervation index was significantly higher in the 4MC group than the control group (p=0.0002); there was also increased unmyelinated fiber density (p=0.0042) and unmyelinated fibers/Remak bundle (p = 0.001) in sural nerves, indicating unmyelinated nerve fiber regeneration. These changes were accompanied by increases of transcripts for
