[An outline of the history of neuroendoscopy].

Almost 200 years ago, German doctor Philipp Bozzini introduced the idea of endoscopy as a method of permitting visualisation of body cavities through their natural openings. First experiences with the use of this new method in different fields of medicine began in the early 20th century. Development of neuroendoscopy was related to the treatment of hydrocephalus. In 1910 Victor Lespinasse conducted the first neuroendoscopic intervention. He executed coagulation of choroid plexus in two children diagnosed with communicating hydrocephalus. The method was further developed by Walter Dandy who imp

[Neuroendoscopy and endoscopic neurosurgery].

The trend in recent neurosurgery is going toward further reduction of invasiveness and avoidance of traumatization of the brain, spinal cord, and peripheral nerves to reduce the risk of neurological and mental deficit. In this way, the duration of hospital-stay and disablement can be shortened. An important "minimally invasive" technique is neuroendoscopy. The ventricular system and the subarachnoid space of the brain give suitable conditions for the use of an endoscope. Non-communicating hydrocephalus is the most frequent indication for an endoscopic approach. However, arachnoid cysts, cystic

Neuroendoscopy: history, endoscopes, and instrumentation.

INTRODUCTION: Endoscopy was first employed in the surgical treatment of neurosurgical diseases early in the twentieth century, but did not become an established practice for a long time, mainly because of poor technology and clinical results. After a slow re-appearance in the 1980s, the 1990s saw an explosion of techniques and instrumentation. Continuing technological improvement has led to further expansion of surgical techniques and indications for use of neuroendoscopy. DISCUSSION: The expansion of ventricular endoscopy has led to significant understanding of CSF disorders. Aqueduct stenos

Neuroendoscopy: Current and Future Perspectives.

Neuroendoscopic surgery is performed because it causes minimal damage to normal structures, carries a lower rate of complications, and achieves excellent outcomes. Surgeons using an endoscope and related instruments can perform complex operations through very small incisions, which is especially useful for minimally invasive procedures for the brain and spine. Neuroendoscopic surgery is now performed in cases of obstructive hydrocephalus, various intraventricular lesions, hypothalamic hamartomas, craniosynostosis, skull base tumors, and spinal lesions. This review discusses the brief history o

Experience with an ultrasonic aspirator in neuroendoscopy.

This report describes the clinical application of an ultrasonic aspirator for endoscopic neurosurgery. Eight patients with intraventricular hematoma with complete tamponade of the ventricular system and marked hydrocephalus were treated with the ultrasonic aspirator for endoscopic neurosurgery. Clinical evaluation and neuroimaging studies were obtained for one month. Removal of hematoma was confirmed on follow-up imaging. The ultrasonic aspirator is operated by electrostriction transducer on low generating power (15-30 watts). Its tube has an outer diameter of 1.8 mm. This aspirator is designe

The authors report a series of 40 patients treated by endoscopic neurosurgery. It includes 31 cases of obstructive hydrocephalus, 4 paraventricular or intraventricular CSF cysts, 3 cases of multiloculated hydrocephalus, one suprasellar arachnoid cyst and one cystic astrocytoma with mural tumor nodule. Third ventriculostomy is the most frequent indication of the endoscopic neurosurgery, which is very useful also for performing fenestration of CSF cysts and multiloculated hydrocephalus. The surgical endoscopic techniques in the different above mentioned pathologies are exposed. The criteria for

Intraventricular neuroendoscopy: complication avoidance and management.

In the modern era, neuroendoscopy has had an increasingly prominent role in neurosurgery. As attention has focused the development of minimally invasive surgical methods, neuroendoscopy has advanced both as an independent treatment modality for various neurologic disorders and as an adjunct to microneurosurgery. Neuroendoscopy is distinct from traditional surgery, and a thorough understanding of its unique attributes is required to attain maximal benefit. In addition to its advantages, neuroendoscopy is associated with unique obstacles that must be anticipated, appreciated, and accounted for t

Electromagnetic-guided neuroendoscopy in the pediatric population.

OBJECT: Image-guided neuroendoscopy is being increasingly used in an attempt to reduce the morbidity associated with surgery and to make navigation easier. It has a particularly useful application in the pediatric population for the treatment of conditions such as complex hydrocephalus and arachnoid cysts. However, its use has been limited by the requirement for rigid head fixation, which may be difficult in infants because of the immaturity of the skull. In addition there can be line-of-sight issues, which can be a problem with optical-based systems. Electromagnetic navigation has eliminated

Neuroendoscopy for spinal disorders: a brief review.

Neuroendoscopy has grown rapidly in the last 20 years as a therapeutic modality for treating a variety of spinal disorders. Spinal endoscopy has been widely used to treat patients with cervical, thoracic, and lumbosacral disorders safely and effectively. Although it is most commonly used with minimally invasive lumbar spine surgery, endoscopy has gained widespread acceptance for the treatment of thoracic disc herniations and for anterior release and rod implantation in the correction of thoracic spinal deformity. The authors review the use of endoscopy in spine surgery and in the treatment of

Value of high-definition imaging in neuroendoscopy.

To compare the image quality of a standard definition (SD) three-chip camera with a new high-definition (HD) three-chip camera. In five neurosurgical interventions, an SD three-chip camera and an HD three-chip camera were used with the same endoscopic equipment. Both cameras were used while performing one endoscopic third ventriculostomy, one endoscope-assisted microvascular decompression, one endoscope-assisted removal of a vestibular schwannoma, and two endonasal pituitary surgeries. To provide comparable conditions, the outputs of both cameras were displayed on the same flat screen and were

Air encephalography for hydrocephalus in the era of neuroendoscopy.

INTRODUCTION: There is often uncertainty regarding the site of the cerebrospinal fluid (CSF) block in individual patients with hydrocephalus, leading to a significant failure rate for endoscopic third ventriculostomy (ETV) when performed for unconventional pathologies such as postmeningitic and posthaemorrhagic hydrocephalus. We describe the use of lumbar air encephalography (AEG) to refine the indications for ETV in such circumstances. METHODS: Data from AEG studies used to guide indications for ETV were collected prospectively. The technique and protocol for AEG have been modified from the

[Postoperative respiratory alkalosis as a complication of neuroendoscopy].

Endoscopic neurosurgery is a minimally invasive technique that has been developing rapidly. It is mainly indicated for the treatment of hydrocephaly due to ventriculocisternostomy, biopsies of cerebral ventricular lesions, evacuation of cerebral hematomas and spinal surgery. Hemorrhage, infection and spinal fluid fistula are known complications. We report the appearance of symptomatic postoperative respiratory alkalosis in a patient who underwent ventriculocisternostomy by endoscopic neurosurgery. The underlying disease was obstructive hydrocephaly secondary to partial stenosis of the Silvius

A new method for transcutaneous coaxial neuroendoscopy.

A new method of direct endoscopy of the subarachnoid space and a major part of the ventricles of the human central nervous system is presented. The technique was developed on more than 100 human bodies with the help of a bronchoscope. Percutaneous entry into the subarachnoidal space is performed from the dorsal side between vertebrae L5 and S1. The endoscope can be moved along the spinal cord on both the dorsal and the ventral side. From the dorsal side of spinal cord the cerebello-medullary cistern can be reached. The fourth ventricle is entered through the median aperture and then the third
