Neurofibromatoses.

The neurofibromatoses are a group of genetic disorders that cause development of nervous system tumors as well as various other tumor and systemic manifestations. Neurofibromatosis type 1 is the most prevalent of these conditions and has the most variable phenotype and highest risk of malignant tumor formation. Neurofibromatosis type 2 has no associated malignant tumors but does carry significant morbidity, including deafness, facial weakness, and physical disability. Schwannomatosis is the least prevalent of these disorders and is characterized primarily by nonvestibular schwannomas and pain.

[Neurofibromatoses].

Neurofibromatosis type 1 (NF1) and type 2 (NF2) are hereditary autosomal dominant neurocutaneous disorders, the phacomatoses, characterized by the development of tumors derived from the cells of the peripheral nerve sheath and also includes schwannomatosis. Regarding the clinical and genetic aspects they are, however, two distinct entities which are described separately in the following review. In addition to the typical clinical presentation and diagnostic criteria, characteristic imaging findings are presented especially in terms of the role of imaging in the diagnosis, follow-up and assessm

Neurofibromatoses.

The studies of familial tumor predisposition syndromes have contributed immensely to our understanding of oncogenesis. Neurofibromatosis 1, neurofibromatosis 2 and schwannomatosis are inherited autosomal dominant neurocutaneous disorders with complete penetrance. They are clinically and genetically distinct and considerable knowledge has been gathered about their pathogenesis. In this chapter, the genetics, molecular mechanism of disease, as well as clinical features, diagnosis and treatment are discussed.

The Neurofibromatoses.

BACKGROUND: Neurofibromatosis of types 1 and 2 (NF1, NF2) and schwannomatosis are the diseases that make up the neurofibromatosis spectrum. With respective incidences of 1 in 3000, 1 in 33 000, and 1 in 60 000 births, they form part of the group of rare tumor-suppressor syndromes. They give rise to a greater tumor burden for the nervous system than any other type of neoplastic disease. New approaches to symptomatic treatment are emerging. METHODS: This review is based on articles retrieved by a selective literature search on the pathogenesis, diagnosis, and treatment of the neurofibromatoses.

The neurofibromatoses. An overview.

The last two decades have seen clinical and molecular delineation of the different forms of neurofibromatosis. Differentiation of these forms is not just an academic exercise: their natural history, management and genetic counselling are quite different. Of the numerical classifications of neurofibromatosis proposed in the past, only neurofibromatosis type 1 (Nf1) and neurofibromatosis type 2 (Nf2) are now well delineated clinically and have been shown to be distinct at the molecular level. For both forms of neurofibromatosis, patients with clinical generalised disease have been demonstrated t

New insights into the neurofibromatoses.

The two neurofibromatoses, neurofibromatosis type 1 (NF1) and type 2 (NF2), have been greatly advanced by the cloning of their respective disease genes. Although NF1 and NF2 are clinically distinct disorders, they represent diseases caused by disruption of tumor suppressor genes. Tumor suppressor genes encode proteins involved in the proper regulation of cell differentiation and proliferation and their disruption presumably predisposes to tumor formation. This year it was demonstrated that the NF1 gene fulfills the genetic criteria for a tumor suppressor gene and that its expression is altered

Ophthalmological issues in the neurofibromatoses.

The neurofibromatoses are a set of at least two distinct disorders characterized by the development of nerve sheath tumors and diverse other features. Both are genetically determined as autosomal dominant traits, and both genes have been identified, with resultant major insights into pathogenesis. The neurofibromatoses have prominent effects on the visual system, which are important both for diagnosis and clinical management. This review focuses on clinical aspects of the neurofibromatoses, highlighting ophthalmological issues and recent advances in molecular genetics.

Molecular biology of the neurofibromatoses.

The molecular biology of the neurofibromatoses is fascinating, both because the process of obtaining the data has been instructive in investigating other genetic disorders and because those data will be so informative about physiological and pathological phenomena well beyond the neurofibromatoses themselves. In this brief overview, the highlights of the molecular biology of NF-1 and NF-2 are noted and their significance considered in terms of key clinical facts.

The Neurofibromatoses. Part 1: NF1.

The neurofibromatoses, including neurofibromatosis 1 (NF1), neurofibromatosis 2 (NF2), and schwannomatosis, comprise a group of genetically distinct disorders of the nervous system unified by the predisposition to nerve sheath tumors. NF1 is the most common neurogenetic disorder, with a birth incidence of 1 in 3000. NF1 is inherited in auto-somal dominant fashion with full penetrance and variable expressivity. The hallmark lesion of NF1 is the neurofibroma, a benign tumor derived from the nerve sheath and composed of a mixture of proliferating Schwann cells, fibroblasts, mast cells, and pericy

The neurofibromatoses: when less is more.

The study of cancer predisposition syndromes presents unique opportunities to gain insights into the genetic events associated with tumor pathogenesis. Individuals with two inherited cancer syndromes, neurofibromatosis 1 (NF1) and neurofibromatosis 2 (NF2), develop both benign and malignant tumors. The corresponding genes mutated in these two disorders encode tumor suppressor proteins, termed neurofibromin (NF1) and merlin (NF2), which function in novel ways to regulate cell growth and differentiation. Neurofibromin inhibits cell proliferation, at least in part, by modulating mitogenic pathway

[Neurofibromatoses. Types, classification and diagnostic criteria].

According to the worldwide-accepted classification of the neurofibromatoses only neurofibromatosis type 1 (NF1) and neurofibromatosis type 2 (NF2) are well defined. Other described forms of neurofibromatosis are rare. The aim of this paper is to review the contemporary classification and diagnostic criteria of the different types of neurofibromatoses.

The clinical and diagnostic implications of mosaicism in the neurofibromatoses.

Neurofibromatosis type 1 and type 2 both occur in mosaic forms. Mosaicism results from somatic mutations. Early somatic mutations cause generalized disease, clinically indistinguishable from nonmosaic forms. Later somatic mutation gives rise to localized disease often described as segmental. In individuals with mosaic or localized manifestations of neurofibromatosis type 1 (segmental neurofibromatosis type 1), disease features are limited to the affected area, which varies from a narrow strip to one quadrant and occasionally to one half of the body. Distribution is usually unilateral but can b

Neurofibromatoses: part 1 - diagnosis and differential diagnosis.

Neurofibromatoses (NF) are a group of genetic multiple tumor growing predisposition diseases: neurofibromatosis type 1 (NF1), neurofibromatosis type 2 (NF2) and schwannomatosis (SCH), which have in common the neural origin of tumors and cutaneous signs. They affect nearly 80 thousand of Brazilians. In recent years, the increased scientific knowledge on NF has allowed better clinical management and reduced complication morbidity, resulting in higher quality of life for NF patients. In most cases, neurology, psychiatry, dermatology, clinical geneticists, oncology and internal medicine specialist

Intramedullary Spinal Cord Tumors Associated with the Neurofibromatoses.

The neurofibromatoses (NF) comprise several tumor predisposition conditions, including neurofibromatosis type 1 (NF1) and neurofibromatosis type 2 schwannomatosis (NF2-SWN). Although paraspinal peripheral nerve sheath tumors (including neurofibromas and schwannomas) occur frequently in the NF conditions, there is also a risk of intramedullary spinal cord tumors in people with NF1 and NF2-SWN. Here, we discuss the presentation and diagnosis of the various forms of NF, the intramedullary spinal cord tumors that occur in NF1 and NF2-SWN, and their diagnostic and treatment considerations.

The neurofibromatoses: hereditary predisposition to multiple peripheral nerve tumors.

Neurosurgeons must be aware of the neuro-fibromatoses because they frequently present with nervous system tumors. Such patients develop numerous peripheral nerve sheath tumors; fortunately, only a few of these tumors are symptomatic or malignant. The tumor burden in individual patients can be remarkable, however, producing significant morbidity and requiring extensive or repeated care by a skilled neurosurgeon. Because it is impossible to predict which of the deep neurofibromas will undergo malignant transformation, it behooves clinicians and patients to be aware of any change in symptoms or g
