[Neurolymphomatosis].

Neurolymphomatoses (NL) are defined as the direct infiltration of the peripheral nervous system (PNS) by lymphomatous or leukaemic cells. The diagnosis of this rare disease is complex, typically relying on peripheral nerve histology, an invasive examination with a risk of sequelae. This diagnostic entity should be considered in the presence of a painful neuropathy, sometimes severe, whether or not associated with immunohaematological abnormalities, and in cases of resistance to first-line immunomodulatory treatments. NLs are classified into primary, which are limited to or originate from the p

Human neurolymphomatosis.

A patient with a chronically progressive fatal sensorimotor neuropathy showed, at autopsy, extensive and selective lymphocytic infiltration of peripheral and cranial nerves and a segmental demyelinative process. A clinically occult retroperitoneal lymphoma without spread to other systemic organs was also present. The possibility of a selective infiltration of the peripheral nervous system by the retroperitoneal malignancy is rejected as unlikely. Our case bears a strong similarity to 3 other cases previously reported which may be grouped together under the heading "human neurolymphomatosis". T

Neurolymphomatosis in a cat.

A 9-year-old male mixed breed cat showed chronic progressive neurological symptoms, which are represented by ataxia and seizures. At necropsy, spinal roots and spinal ganglions at the level of sixth cervical nerve to second thoracic nerve were bilaterally swollen and replaced by white mass lesions. Right brachial plexus and cranial nerves (III, V and VII) were also swollen. A mass lesion was found in the right frontal lobe of the cerebrum. Histologically, neoplastic lymphocytes extensively involved the peripheral nerves, and they infiltrated into the cerebral and spinal parenchyma according to

[Human peripheral neurolymphomatosis].

Two further cases of human neurolymphomatosis are reported, and clinical, histopathologic and nosologic features of the affection reviewed with respect to findings in these 2 patients and in 7 other reported cases. Clinical manifestations are those of a flaccid ascending paralysis, associated in most cases with an asymmetrical abolition of tendon reflexes, violent muscular pains and sphincter disturbances, frequently preceded by a regressive cranial nerve palsy. Pathology shows infiltration of the peripheral nervous system (nerves, roots, and spinal ganglia) by lymphoid cells, sometimes associ

Diagnosis and management of neurolymphomatosis.

Invasion of cranial nerves and peripheral nerve roots, plexus, or nerves by non-Hodgkin lymphoma is denoted as neurolymphomatosis (NL). Four clinical patterns are recognized. Most commonly, NL presents as a painful polyneuropathy or polyradiculopathy, followed by cranial neuropathy, painless polyneuropathy, and peripheral mononeuropathy. Diagnosis of NL is challenging and requires integration of clinical information, imaging findings, and histopathologic examination of involved nerves or nonneural tissue and cerebrospinal fluid analysis. In the rare cases of primary NL, the diagnosis is often

Ocular neurolymphomatosis: a case report and systematic review of ocular cranial nerve neurolymphomatosis.

Neurolymphomatosis is a rare manifestation of lymphoma. Neurolymphomatosis involving ocular cranial nerves of II, III, IV, and VI remains a poorly characterized disease entity. The authors report a case of cranial nerve III neurolymphomatosis with simultaneous extraocular muscle infiltration which initially mimicked myasthenia gravis with fluctuating diplopia and ptosis. To the authors' best knowledge, this represents the first report where neurolymphomatosis masquerades as myasthenia gravis and is the second case documenting coinciding cranial nerve with extraocular muscle involvement. The ca

The natural history of neurolymphomatosis.

BACKGROUND: Neurolymphomatosis is a lymphoid malignancy of the peripheral nervous system and its natural history is poorly understood. METHODS: We performed PubMed search and extracted clinical data for Kaplan-Meier statistics to determine outcome parameters over time. Kruskal-Wallis test was performed to compare prognostic factors. RESULTS: Our search identified 559 patients and their median age was 61 years. Median overall survival (OS) was 12.0 (range 10.0-15.0) months. Diffuse large B-cell lymphoma was the most frequent histology, involving the brachial plexus, cranial nerves, and sciati

Ocular involvement in neurolymphomatosis.

PURPOSE: To describe the ophthalmic symptoms and histopathological findings in a case of primary neurolymphomatosis (NL). OBSERVATIONS: A man in his 60s with a prior diagnosis of chronic inflammatory demyelinating polyneuropathy developed facial numbness, diplopia, drooling, and difficulty swallowing. Over a 3-month period, he developed total ptosis and ophthalmoplegia of the right eye with a dilated, non-reactive pupil considered secondary to cranial nerve III and VI palsies. His left pupil subsequently became non-reactive to light and accommodation, and extraocular motility of the left eye

Neurolymphomatosis in Waldenström's macroglobulinaemia.

We report four patients with Waldenström's macroglobulinaemia with an unusual neurologic complication, neurolymphomatosis, characterized by meningeal and root nerve infiltration by lymphoplasmacytic cells. Patients presented with rapidly progressive leg proximal weakness. Examination of cerebrospinal fluid disclosed lymphoplasmacytic cells. Magnetic resonance imaging of the lumbar spine was suggestive of a tissular infiltration of leptomeninges and nerve roots. Chemotherapy and irradiation of involved tissues led to a remarkable and sustained neurological improvement.

Neurolymphomatosis presenting as mononeuritis multiplex.

We report the case of a 60-year-old man who presented with a mononeuritis multiplex-type illness, beginning cranially and then sequentially involving peripheral nerves in all four limbs and torso. Despite extensive investigation, including multiple lumbar punctures and tissue biopsies, the diagnosis was not made until his death 10 months after presentation. Post mortem revealed lymphocytic infiltration of multiple peripheral nerves and radicules with minimal central nervous system involvement, consistent with neurolymphomatosis. Except for the pericardium, there was no other evidence of system

Neurolymphomatosis: The Sinister Face of Lymphoma.

Neurolymphomatosis (NL) is a rare clinical condition characterized by the infiltration of malignant lymphocytes into the cranial or peripheral nerves, nerve roots, or plexus. Diagnosis can be clinically challenging due to its variable presentation. It usually occurs in B cell lymphoma; however, a few cases of extranodal killer/T cell lymphoma. Most cases present at a secondary site in patients with primary site in remission. 18Fluorine fluorodeoxyglucose positron emission tomography/computed tomography plays an important role in the early detection of NL, resulting in timely treatment. Primary neurolymphomatosis of the trigeminal nerve.

We report a case of a primary malignant lymphoma of the trigeminal nerve that was associated with facial pain. A 65-year-old man was examined at another hospital for unilateral facial pain. Carbamazepine was prescribed, but his symptoms did not improve. Magnetic resonance imaging (MRI) revealed swelling of the trigeminal nerve and a mass lesion in Meckel's cave. The patient was referred to our hospital at this point. Gadolinium-enhanced MRI and F18-Fluorodeoxyglucose-position emission tomography suggested a likely malignant tumour and a biopsy was performed. Histopathological examination showe

Primary Neurolymphomatosis: A Literature Review.

BACKGROUND: Primary neurolymphomatosis (PNL) is a rare clinical entity resulting from direct lymphomatous infiltration into the peripheral nervous system. Its diagnosis is challenging as the hematological condition is unknown at the onset of neurological symptoms. METHODS: We report two of our own cases and the first extensive review of published cases of PNL to delineate its clinical features, paraclinical investigation results, progression, and treatment response more precisely. We extracted demographic data, clinical presentation, results of the investigations performed, type and number of

Lymphoma relapse presenting as neurolymphomatosis.

Neurolymphomatosis (NL) is a rare neurological manifestation of lymphoma characterized by malignant lymphoma cells infiltrating cranial or peripheral nerve, or their roots. We present the first reported Australian case of a patient whose initial presentation of relapsed mantle cell lymphoma was NL. Our case highlights that clinical and imaging findings of NL often mimic other neuropathies, and hence presents unique challenges that may lead to delayed diagnosis and management. We emphasize the importance of considering NL in the differential diagnosis and combining imaging with other diagnostic

Fluorothymidine PET/CT in neurolymphomatosis.

Neurolymphomatosis, a rare extranodal lymphoma, may involve the cranial nerves, peripheral nerves, spinal nerve roots, or nerve plexus. F-FDG PET/CT however shows increased FDG uptake in the nerves but is not specific for malignancy. We report a patient with neurolymphomatosis who underwent F-FDG PET/CT and F-fluorothymidine PET/CT where both the scans showed increased tracer uptake in the right sciatic nerve, proven later as neurolymphomatosis on nerve biopsy. Furthermore, the F-FDG avid mediastinal lymph nodes did not show F-fluorothymidine avidity, suggesting an inflammatory etiology.

Neurolymphomatosis associated with Sézary syndrome.

BACKGROUND: Mycosis fungoides and Sézary syndrome are cutaneous T-cell lymphomas characterized by the epidermotropism of tumor cells. Neuropathic disease is rare during mycosis fungoides and Sézary syndrome and usually results from a central nervous system involvement in late stages. Neurolymphomatosis is defined as the infiltration of the peripheral nerves by tumor lymphocytes. It has been described in patients with aggressive systemic lymphomas but, to our knowledge, not in patients with mycosis fungoides or Sézary syndrome. We report the first case of neurolymphomatosis in a patient with Sé

[Human peripheral neurolymphomatosis (reticuloendothelial polyradiculoneuritis)].

This is the clinico-pathological report of a case of reticulo-endotheliosis of the nervous system in a 53-year-old white female. She had a history of transient facial palsy followed 6 months later by progressive weakness, paresthesiae and severe pain in both lower limbs. Clinical examination revealed signs of sensory and motor polyradiculoneuritis with involvement of the oculomotor, facial and trigeminal cranial nerves. She died 10 months after the onset because of bronchopneumonia. Pathological findings in the nervous system were limited to the cranial and spinal roots and ganglia. Histopatho
