Effects of non-depolarizing neuromuscular blocking agents on the cardiac vagus nerve in the guineapig.

The ability of gallamine, metocurine, pancuronium, tubocurarine and atropine to block the response to postganglionic stimulation of the vagus nerve were measured in isolated, spontaneously beating guineapig atria. From analysis of the results in conjunction with those of earlier assays of concentrations that block effects of preganglionic vagal stimulation, it was concluded that only tubocurarine possessed ganglionic blocking activity. From calculation of the extent of block of muscarinic receptors at drug concentrations which blocked the response to postganglionic vagal stimulation, it was co

Supramolecular therapeutics to treat the side effects induced by a depolarizing neuromuscular blocking agent.

Succinylcholine (Sch) is the only depolarizing neuromuscular blocking agent widely used for rapid sequence induction in emergency rooms. Unfortunately, a variety of (sometimes lethal) adverse effects, such as hyperkalemia and cardiac arrest, are associated with its use, and currently there are no specific antidotes to reverse Sch or to treat these side-effects. Methods: The binding behaviors of Sch and several synthetic receptors, including cucurbit[7]uril, sulfo-calix[4]arene and water-soluble carboxylatopillar[6]arene (WP[6]), were first investigated. With a mouse model, a leathal dose of Sc

Comparison of the cardiovascular actions of ORG NC 45 with those produced by other non-depolarizing neuromuscular blocking agents in experimental animals.

The cardiovascular actions of 1- [(2 beta,3 alpha, 16 beta,17 beta)-3,17-bis(acetyloxy)-2-(1-piperidinyl)-androstan-16-yl]-1-methyl-piperidinium bromide (Org NC 45) are reviewed and compared with those of other non-depolarizing neuromuscular blocking drugs. Results obtained in anaesthetized cats and dogs have demonstrated that, in contrast to other neuromuscular blocking drugs, Org NC 45, even in doses 20 times greater than those required for neuromuscular block, has no effects on heart rate, arterial pressure, autonomic ganglia, adrenoceptors or baroreceptors activity. Anesthetic Considerations in Congenital Myasthenic Syndrome: A Case Report.

Congenital myasthenic syndrome poses unique anesthetic challenges. Most notably, this rare disease is heterogeneous. Affected individuals' responses to depolarizing and non-depolarizing neuromuscular blocking agents are poorly understood. Additionally, while neuraxial techniques may confer respiratory benefits due to their opioid-sparing profiles, it is unclear to what extent local anesthetics may impair signal transmission at the neuromuscular junction. This case report raises concern for heightened sensitivity to non-depolarizing neuromuscular blocking agents in patients with congenital myas

A Case of Rocuronium-Induced Anaphylaxis Managed With Sugammadex.

Anaphylaxis is a severe, life-threatening complication that can result in complete cardiovascular and respiratory collapse. Peri-operatively, this complication is most often seen after the intravenous (IV) induction of general anesthesia, and the most common responsible agents are non-depolarizing neuromuscular blocking agents. In this report, we describe a case of perioperative anaphylaxis induced by the non-depolarizing neuromuscular blocker rocuronium. Sugammadex, a reversal agent for the non-depolarizing muscle relaxants rocuronium and vecuronium, was administered shortly after induction.

Prolonged paralysis after neuromuscular blockade.

Non-depolarizing neuromuscular blocking agents have been used with increasing frequency in critically ill patients. Recently, numerous reports have described patients with prolonged muscle weakness after use of these agents for more than two days. Brief weakness lasting several hours to several days is likely the result of prolonged neuromuscular blockade, while more prolonged weakness lasting several weeks to months is likely caused by a myopathy. Specific features of this myopathic disorder are reviewed. Clinically, patients have flaccid paralysis with intact sensation and cognition. Electro

Development and potential clinical impairment of ultra-short-acting neuromuscular blocking agents.

Developing a non-depolarizing neuromuscular blocking agent that, like succinylcholine, has a rapid onset and a short duration of effect remains a goal of ongoing research. While rocuronium fills a portion of this need, the large doses required for rapid intubation render it a much longer-acting neuromuscular blocking agent. Postoperative residual neuromuscular block (NMB) is an increasingly recognized complication of non-depolarizing neuromuscular blocking agents. This occurs because of dosing choices for neuromuscular blocking agents and anticholinesterases as well as insensitivity of typical

Use of Muscle Relaxants in Emergency Medicine: A Review.

Modern anesthesia, intensive care, and emergency medicine rely heavily on neuromuscular blocking agents (NMBAs), first introduced in 1942. These agents not only facilitate endotracheal intubation but also improve surgical conditions by suppressing muscle responses to stimuli. NMBAs function via depolarizing (eg, succinylcholine) or non-depolarizing mechanisms. Non-depolarizing agents are further classified by chemical structure into aminosteroids (eg, rocuronium, vecuronium), benzylisoquinolinium compounds (eg, atracurium, cisatracurium), and asymmetric mixed-onium chlorofumarates (gantacurium

Extravascular leakage of induction doses of rocuronium: four cases in which both depth of neuromuscular block and plasma concentration of rocuronium were assessed.

The duration of action of extravasated rocuronium varies depending on the patient's comorbidities. In patients who receive high doses of non-depolarizing neuromuscular blocking agents subcutaneously, anesthesiologists should be aware of unexpected prolongation of the progress and recovery of neuromuscular block. In such cases, the depth and recovery of neuromuscular block should be objectively monitored to avoid residual neuromuscular block and recurarization.

Use of Neuromuscular Blocking Agents in Elderly Patients: A Narrative Review in Geriatric Medicine.

With the growing number of elderly patients undergoing surgical procedures, the use of neuromuscular blocking agents (NMBAs) in geriatric anesthesia has become increasingly relevant. Aging is associated with physiological and pathological changes that affect drug pharmacokinetics and pharmacodynamics, including reduced renal and hepatic function, altered body composition, and decreased plasma protein levels. These changes influence the onset, duration, and recovery from neuromuscular blockade, increasing the risk of postoperative residual curarization (PORC) and respiratory complications. This

[Update: Neuromuscular Blockade during General Anesthesia].

The correct use of muscle relaxants and neuromuscular monitoring during anesthesia has been subject of controversial discussions for decades. Particularly important in clinical practice are identification and management of residual neuromuscular blockages and avoidance of associated complications. Despite the differences in the molecular mechanisms of action between depolarizing and non-depolarizing muscle relaxants the blockade of the postsynaptic nicotinic acetylcholine receptor remains a common ending pathway. Due to its unfavorable side effect profile, succinylcholine should only be used i

Age-Specific Pharmacology of Neuromuscular Blocking Agents: A Comprehensive Review.

Neuromuscular blocking agents (NMBAs) are essential in anesthesia, used primarily for intubation and synchronizing mechanical ventilation. They can be divided into depolarizing and non-depolarizing agents with an addition of reversal agents, which can be used to avoid residual muscle blockade. Their pharmacological properties vary among age groups due to physiological differences. In children, immature liver and kidney function, body composition, and immature neuromuscular junction influence drug properties. Elderly patients tend to have reduced organ function, more comorbidities, and greater

Neuromuscular blockade: what was, is and will be.

Non-depolarizing neuromuscular blocking agents (NMBAs) produce neuromuscular blockade by competing with acetylcholine at the neuromuscular junction, whereas depolarizing NMBAs open receptor channels in a manner similar to that of acetylcholine. Problems with NMBAs include malignant hyperthermia caused by succinylcholine, anaphylaxis with the highest incidence for succinylcholine and rocuronium, and residual neuromuscular blockade. To reverse these blocks, anticholinesterases can act indirectly by increasing the amount of acetylcholine in the neuromuscular junction; sugammadex is the only selec

Comparison of Phonomyography Prototype With Train-of-Four Watch SX for Neuromuscular Monitoring: A Prospective Observational Study.

BACKGROUND: Quantitative neuromuscular monitoring has been advocated to deal with residual neuromuscular block. Phonomyography (PMG) is a technology that captures the acoustic signals yielded by muscle contraction, which is easy to use, anti-interference, and has multiple alternative monitoring sites. Our previous study has developed a PMG prototype for neuromuscular monitoring, and its feasibility was preliminarily verified, but further study on its clinical reliability with different neuromuscular blocking agents (NMBs) is needed. METHODS: This single-center, prospective, observational stud

the selective relaxant binding agents. Sugammadex can reverse residual paralysis by encapsulating free circulating non depolarizing muscle relaxants. The mains advantages of sugammadex when compared with conventional anticholinesterase agents are a much faster recovery time and the unique ability, for the first time, to reverse rapidly and efficiently deep levels of neuromuscular blockade. However it only works for reversal of rocuronium or vecuronium-induced neuromuscular blockade. When administered 3 min after rocuronium the use of a large dose (16 mg/kg) can even reverse rocuronium signific

In-depth exploration of sugammadex and novel neuromuscular blockade antagonists: a narrative review.

Sugammadex is a modified γ-cyclodextrin widely used in clinical practice to reverse neuromuscular blockade induced by non-depolarizing neuromuscular blockade agents, notably rocuronium and vecuronium. It has been proven to selectively reverse rocuronium-induced neuromuscular blockade within 3 min of administration, representing a revolutionary breakthrough in anesthesiology. Although severe adverse events have been observed in a very small subset of patients, these events have not compromised its overall clinical utility or widespread adoption. This review synthesizes key aspects of sugammadex
