Auditory neurophysiology.

The anatomy and the physiology of the ear and the auditory system are reviewed. The differences between the anatomy and physiology in humans and animals commonly used in auditory research are discussed. The clinical importance of brainstem auditory evoked potentials and their neural generators are discussed as well as the anatomy and physiology of the acoustic middle ear reflex. The possible clinical relevance of the extralemniscal auditory system is also addressed.

Neurophysiology of synesthesia.

Synesthesia is an experience in which stimulation in one sensory or cognitive stream leads to associated experiences in a second, unstimulated stream. Although synesthesia is often referred to as a "neurological condition," it is not listed in the DSM IV or the ICD classifications, as it generally does not interfere with normal daily functioning. However, its high prevalence rate (one in 23) means that synesthesia may be reported by patients who present with other psychiatric symptoms. In this review, I focus on recent research examining the neural basis of the two most intensively studied for

Neurophysiology of spasms.

Spasms are a form of epileptic seizure typical of infancy. From a clinical point of view, the child presents a flexor-extensor movement involving the trunk and limbs and lasting about 1s. Although asymmetry can be present, the seizure involves both sides of the body. The ictal discharge most frequently associated with spasms in West syndrome (WS) is a diffuse triphasic slow high-amplitude wave and less frequently a low-amplitude brief rapid rhythm. The origin of the spasm in WS and classification as either partial or generalized seizure are the subject of much discussion. Factors supporting pa

Neurophysiology of myoclonus.

Myoclonus may be generated by any area in the central nervous system. Finding its generator is helpful in the diagnostic process. Although clinical features have to be carefully analyzed as they may give a first idea, neurophysiologic study of myoclonus provides the most important clues for the determination of the generator. Surface electromyography (EMG) allows analyzing the recruitment order in generalized myoclonus, thereby suggesting either a cortical, brainstem, or spinal origin. It also reveals whether myoclonus is positive (jerks that are caused by muscle activation) or negative (jerks

Neurophysiology of pain.

The transmission of pain-related information from the periphery to the cortex depends on signal integration at three levels of the nervous system: the spinal medulla, brainstem and telencephalon. In fulfilling its task of safeguarding human health, pain may develop as a result of damaged or altered primary afferent neurons (stimulus-dependent) or arise spontaneously without any apparent causal stimulus (stimulus-independent). Hyperalgesia (i.e. an exaggerated perception of pain after a painful stimulus) is due to an anomaly in the processing of nociceptive inputs in the central and peripheral

Clinical neurophysiology of apnea.

Understanding the clinical neurophysiology of apnea generation encompasses discussion of the neuroanatomic aspects of central respiratory rhythm and pattern generation, including the central respiratory control networks, central and peripheral chemoreceptors, mechanisms of respiratory muscles, and sleep state dependent differences. Anatomical and functional links to apnea also involve central respiratory motor output recruited from the hypoglossal nerve, which has led to novel treatments for obstructive sleep apnea. Autonomic fluctuations occur in relation to sleep-wake and sleep states (i.e.,

Updated neurophysiology of itch.

The unique physiological features of histamine-sensitive C-fibers and spinothalamic tract neurons support the hypothesis of itch specific pathway, whereas subsequent studies on cowhage-induced itch have provided evidence against it, suggesting the presence of multiple neural pathways for itch. Not only peripheral pruritogens but also spinal neural receptors are involved in the control of itch, and will be the target of treatment. Itch sensitization in chronic pruritus is another crucial factor that needs to be considered in the treatment. Neuropathic itch is the type of itch that occurs when n

Clinical neurophysiology in ALS.

Amyotrophic lateral sclerosis (ALS) belongs to a group of disorders known as motor neuron diseases. Despite being one of the most devastating diseases known, there is little evidence for diagnosing and managing patients with ALS. Clinical neurophysiologic tests are essential, when no biological marker exists to aid early diagnosis, not only in relation to diagnosis, but also in the development of disease progression, and perhaps, in the future, in measuring patients' response to therapy. The electrophysiological features used in the diagnosis of ALS are based on Awaji-shima consensus recommend

[Neurophysiology of corticobasal degeneration].

INTRODUCTION: Corticobasal degeneration (CBD) is a neurodegenerative disorder of mid- to late-adult life. From a clinical standpoint, CBD is characterized by (i) an insidious onset and a slowly progressing, unilateral, levodopa-unresponsive parkinsonian syndrome with dystonia or myoclonus and (ii) cerebral features such as apraxia, alien limb phenomena and cortical sensory loss. Decisive clinical diagnostic criteria are not available and thus a neuropathological study remains essential for accurate CBD diagnosis. Consequently, additional non-clinical criteria must be identified in order to imp

[The neurophysiology of cataplexy].

Cataplexy and excessive daytime sleepiness are the leading symptoms of narcolepsy. Electrophysiological studies in humans do not show a clear association between cataplexy and rapid eye movement (REM) sleep. Even a decrement of the H reflex is not specific for cataplexy and may be caused by unspecific triggers such as coughing. Cholinomimetics, which may induce status cataplecticus, do not influence REM sleep, thus evidencing a REM-independent mechanism. Recent studies demonstrate a lack of the neuropeptide hypocretin in the CSF of narcoleptics. Hypocretin controls wakefulness and the motor an

Neurophysiology of Rett syndrome.

Rett syndrome is a neurodevelopmental disorder that in most cases is consequent to a mutation in the MECP2 gene. The central nervous system is the primary organ system involved in Rett syndrome. Neurophysiologic evaluations provide information concerning the developmental aspects of Rett syndrome and the character and extent of involvement of the central, peripheral, and autonomic nervous system pathways. Evoked potentials typically demonstrate intactness of peripheral auditory and visual pathways and suggest dysfunction of central or "higher" cortical pathways. Somatosensory evoked potentials

Neurophysiology of Rett syndrome.

Neurophysiological studies on Rett syndrome (RTT) are reviewed, and pathophysiology of RTT is discussed. The electroencephalography (EEG), sensory evoked potentials (SEP), sleep-wake rhythm study and polysomnography (PSG) study showed age-dependent characteristics. PSG revealed the brainstem and midbrain monoaminergic systems are deranged from early developmental stage, that is serotonin and noradrenaline systems seem to be hypoactive and dopaminergic system is also hypoactive associated with receptor supersensitivity. These monoaminergic systems are known to influence the maturation of the hi

Neurophysiology of gastrointestinal pain.

The only non-general sensation that can be evoked from the gastrointestinal (GI) tract is that of pain ranging from mild discomfort to intense pain. However, in certain regions of the gut, such as the rectum and gastro-oesophagus, the feeling of pain can be preceded by non-painful sensations of distension at lower stimulus intensities. GI pain is often dull, aching, ill-defined and badly localized. In some cases, GI pain is projected to areas of the body away from the originating viscus ('referred' pain). These properties indicate that the representation of internal organs within the central n

Neurophysiology of movement disorders.

The relation of the cortex, cerebellum, and basal ganglia and their participation in movement disorders is presented. Studies have shown that lesions to the cortex will produce contralateral paralysis while damage to the basal ganglia or cerebellum will result in movement abnormalities. These data provide support for the view that subcortical but not cortical structures initiate the control movement activity. It would appear that the basal ganglia and cerebellum receive signals from the auditory, visual, and somatosensory cortex integrate this information, and relay signals back to the motor c

Clinical neurophysiology of tics.

Clinical neurophysiology constitutes a relevant tool for clinicians in movement disorders since it expands their characterization beyond clinically observable features and supports their diagnosis. Although the field has yet to identify a specific neurophysiologic marker for the diagnosis of tics, neurophysiology serves as a valuable tool for clinicians in differentiating tics from other hyperkinetic disorders. Beside diagnosis, recent years have witnessed the emergence of two primary domains in which neurophysiology plays a pivotal role in the realm of tic disorders: investigating the pathoph

Meet the brain neurophysiology.

The central nervous system is composed of two main types of cells: the most numerous are glia, which have a supportive, protective and regulatory role, and neurons, which are the primary computing element. Neurons transmit information as a pulsed electrical code which is conducted down a specialized process (axon) that connects with other neurons. Each neuron can connect with many others, and each neuron can receive input from many others. At the sites of connection (synapses), information is transmitted across a small gap; small molecules (neurotransmitters) are released from the end of the a
