Nocturnal enuresis.

Nocturnal enuresis (NE) is increasingly seen as part of a heterogeneous phenomenon that at times will include daytime lower urinary tract symptoms such as urgency, frequency and wetting - with reduced bladder storage, usually due to an overactive bladder. In turn, these may be associated with constipation and/or faecal soiling. This paper discusses these considerations in the management of NE.

Nocturnal enuresis.

Childhood enuresis is a common socially disruptive problem. The possible pathophysiological factors include a disorder of sleep arousal, nocturnal polyuria, and low bladder capacity. The evaluation of a patient with nocturnal enuresis is aimed to exclude any organic pathology, UTI and voiding dysfunction. An approach to management of this common disorder is outlined.

Nocturnal enuresis.

Nocturnal enuresis is common. Although its aetiology remains unclear recent evidence increasingly supports the lack of a normal nocturnal increase in ADH leading to nocturnal polyuria exceeding functional bladder capacity. Sleep patterns are probably normal although an arousal disorder might be a factor. Treatment should follow careful evaluation to determine if one is dealing with 'uncomplicated' or 'complicated' enuresis. Complicated enuresis requires further investigation. 'Uncomplicated enuresis' always requires reassurance and patient and parent education. Resource centres such as ERIC ca

Nocturnal enuresis.

Nocturnal enuresis is a treatable condition. It is defined as functional urinary incontinence during sleep beyond the age (arbitrarily taken to be) of 6 years old at which control should be established. Although it is a physically benign condition, it is unfortunately associated with emotional-behavioural disturbances and developmental delays. Diagnosis is usually straight-forward, and based mainly on history and physical examination. Often, only a microscopic examination of urine is required to exclude urinary tract infection. Treatment begins with a baseline recording of the frequency of enu

Nocturnal enuresis in children.

Nocturnal enuresis (NE) can result from an interaction of unstable detrusor contractions, delayed arousal from sleep, and nocturnal polyuria. Some children with NE can hold urine well for several hours during the day and have isolated nocturnal enuresis, while others manifest diurnal voiding symptoms (DVS) as well, including urinary frequency, urgency, urge incontinence, and pelvic withholding. The pathogenesis of NE in patients with isolated NE may be different than in those with NE and DVS. In children with NE and DVS, detrusor instability may play a major role in the causation of their void

Nocturnal Enuresis in the Adult.

PURPOSE OF REVIEW: The goal of this paper is to describe the pathophysiology of adult nocturnal enuresis and develop a generalized approach for evaluation and treatment. RECENT FINDINGS: Although nocturnal enuresis (NE) impacts a significant proportion of the adult population, research on this topic remains lacking. In the few existing studies, the management strategy is extrapolated from research on pediatric nocturnal enuresis. Furthermore, treatment approaches highlight the importance of identifying risk factors and contributing pathologies. The modern urologist should understand the compl

[Nocturnal enuresis in children].

After allergic disorders, nocturnal enuresis is the most common chronic childhood condition. Recent research has yielded abundant new knowledge about the condition, especially about its aetiology and pathophysiology, and the psychological consequences. A hereditary background has been substantiated by the identification in genetic linkage studies of areas in chromosomes 12 and 13 that are manifestly associated with bedwetting, though genotype expression in the phenotype appears to be complex and heterogeneous. Pathophysiologically, findings in current intensive research suggest three interacti

Nocturnal enuresis in children.

Thousands of Australian children suffer the unhappiness of waking most mornings in a wet bed--almost all can be helped by a sympathetic doctor. Very few of these children have significant organic or psychological problems and little investigation is indicated. Management includes: sympathy and empathy, cessation of unhelpful practices, and conditioning.

Adult onset nocturnal enuresis.

PURPOSE: We determined the etiology and prognostic significance of adult onset enuresis with absent daytime incontinence. Adult onset nocturnal enuresis unassociated with daytime incontinence is uncommon and there is a paucity of information about its incidence, significance, evaluation and treatment. We present a retrospective evaluation of this condition based on a database review of more than 3,000 consecutive patients referred for the evaluation of lower urinary tract symptoms. MATERIALS AND METHODS: A database of 3,277 consecutive patients was searched for adult onset nocturnal enuresis.

Current management of nocturnal enuresis.

PURPOSE OF REVIEW: Nocturnal enuresis is an especially common problem with the potential to have an appreciable negative impact on the emotional health of a child. RECENT FINDINGS: Our understanding of the pathogenesis continues to improve. A disorder of sleep arousal, a low nocturnal bladder capacity, and nocturnal polyuria are the three factors that interrelate to cause nocturnal enuresis. Constipation is a very common and often unrecognized factor that appreciably affects bladder function. SUMMARY: Successful treatment involves interventions that simultaneously improve these factors. Diuretic treatment of nocturnal enuresis.

OBJECTIVE: Nocturnal polyuria is considered a major pathogenetic factor in nocturnal enuresis, and the antidiuretic drug desmopressin, given at bed-time, is consequently recognized as a first-line treatment alternative. The aim of this open, non-randomized study was to see whether diuretic medication, given in the afternoon, could give similar therapeutic benefit. MATERIAL AND METHODS: Sixty-three children suffering from primary, monosymptomatic, nocturnal enuresis were included in the study by their primary care paediatrician. After 14 days without any treatment and 14 days for which 0.4 mg

Nocturnal enuresis in older adults.

BACKGROUND: Nocturnal enuresis is uncommon in older adults. The paucity of literature about this problem prompts us to review our cases to determine the management strategy. METHODS: Six older adults, including 2 females and 4 males, were evaluated for refractory nocturnal enuresis. Only 2 of them had minor daytime urge symptom. Most of them had failed in the treatment using anticholinergics and/or alpha-adrenergic blocker. Evaluation consisted of detailed medical history, voiding diary, and urodynamic studies. Clinical follow-up persisted for 12 months. We define nocturnal polyuria as nightt

Hereditary phenotypes in nocturnal enuresis.

OBJECTIVE: To identify phenotypic characteristics in three large families with autosomal dominant nocturnal enuresis (NE), and to elucidate whether such characteristics persist after cessation of symptoms. SUBJECTS AND METHODS: From three unrelated NE kindreds (A-C) we included 98 living members of whom 34 either had active NE (>one wet night/month after the age of 5 years) or a history of NE. The family members were interviewed to identify NE type and severity. Subsequently, night-time urine production was recorded for 2 weeks at home and 4 days of frequency-volume charts were completed. Neuromotor development in nocturnal enuresis.

In children with nocturnal enuresis, a higher rate of minor neurological dysfunction has been found. The aim of this study was to assess timed performance (a measure of motor performance speed) and associated movements using a standardized and reliable instrument. The motor function of 37 children with nocturnal enuresis (27 males, 10 females; mean age 10y 7mo [SD 1y 10mo]; age range 8y-14y 8mo) and 40 comparison children without enuresis (17 males, 23 females; mean age 10y 7mo [SD 1y 6mo]; age range 8y-14y 8mo) was assessed using the Zurich Neuromotor Assessment. Nocturnal enuresis: systems for assessment.

Childhood nocturnal enuresis, or bedwetting, is a common childhood condition that causes children and their families stress and embarrassment and which can radically affect the lives of these children. Penny Dobson and Anne Weaver outline how nurses can inform and support these families, and describe a research-based approach, the three systems, which provides a useful framework for assessing the problem before choosing the most appropriate treatment strategy. Specific help that could be offered teenagers is also outlined.

Psychosocial implications of nocturnal enuresis.

Primary nocturnal enuresis (PNE), a biobehavioral problem affecting approximately 5,000,000 children in the United States, is among the most bothersome and frustrating disorders of childhood. Negative psychosocial consequences are common, secondary to the impact of enuresis on family members and others. The enuretic child may be at increased risk for emotional or even physical abuse from family members and may experience stress related to fear of detection by peers. These factors contribute to the loss of self-esteem that the enuretic child often experiences. Fortunately, a number of treatment

Desmopressin for nocturnal enuresis in children.

BACKGROUND: Enuresis (bedwetting) affects up to 20% of five-year-old children and 2% of adults. Although spontaneous remission often occurs, the social, emotional and psychological costs can be great. Desmopressin has been used to treat bedwetting since the 1970s. This is an update of a Cochrane review first published in 2000 and last updated in 2006. OBJECTIVES: To assess the effects of desmopressin on treating nocturnal enuresis in children. SEARCH METHODS: We searched Cochrane Incontinence Specialised Register, which contains trials identified from the Cochrane Central Register of Control

Acupuncture as a treatment for nocturnal enuresis.

Acupuncture can influence spinal micturition centers and parasympathetic innervation to the urinary tract and is known to modulate brain function via the descending serotonergic system. There are numerous difficulties in evaluating the efficacy of acupuncture in patients with nocturnal enuresis (NE), since the patient group is frequently heterogenous and the intervention is commonly given in association with other modalities. Until recently incomplete reporting of the quality aspects of studies has also limited evaluation of treatment effects. The aim of this study was to systematically evalua
