[Norovirus infections].

Noroviruses infect persons of all ages, often causing epidemic outbreaks of acute gastroenteritis as well as sporadic cases. The application of novel molecular methods to the diagnosis of norovirus infections is now revealing their real impact. Molecular epidemiology studies have identified the most common viral genotypes responsible for human infections. Norovirus gastroenteritis is usually mild and of short duration, although the disease can also be severe, especially in the elderly, or may become chronic, as occurred in the immunocompromised patients. Several factors have been identified re

Norovirus gastroenteritis.

Recent epidemiologic studies have shown that norovirus is one of the most frequent causes of acute nonbacterial gastroenteritis. Reverse-transcription polymerase chain reaction and nucleotide sequencing are the means by which the hundreds of norovirus strains have been identified, named, and classified into genogroups and genetic clusters. They are also the means by which a particular strain is traced from the source of an outbreak throughout its spread. These molecular techniques have been combined with classic epidemiology to investigate norovirus outbreaks in diverse settings, including hos

Evolution of norovirus.

Norovirus (NoV) is now the dominant aetiological agent of acute gastroenteritis, and, with the recent introduction of rotavirus vaccines in many countries, this is likely to remain the case. NoV has a significant impact on human wellbeing in terms of morbidity, economic costs and mortality in developing countries. NoVs are divided into six genogroups (GI-GVI), but only GI, GII and GIV are known to infect humans, with GII being the most prevalent, causing >95% of human infections. The immune system is thought to drive selection of emerging pandemic NoVs through both antigenic drift and shift. T

[Norovirus (infants/children)].

Human norovirus (NoV) is an important cause of the epidemic acute gastroenteritis in children and adults. This review mentions on the required knowledge to understand morphology, genetics, transmission, pathogenesis, and control. The development of molecular diagnostic techniques is clarified the epidemiological impact that is now recognized not only as the major cause of foodborne gastroenteritis outbreaks but also as a cause of sporadic gastroenteritis. It was recognized that the combination pattern to an intestinal epithelium of NoV was prescribed in organization, blood type antigen (histob

Pediatric norovirus infection.

Noroviruses (NoVs) are among the most frequent causes of acute pediatric gastroenteritis. Although the disease is often self-limiting and recovery is the rule, it constitutes an important health problem because of its highly contagious nature and the high rate of morbidity. NoVs are responsible for 47-96 % of outbreaks of acute pediatric gastroenteritis, and 5-36 % of sporadic cases. NoV-induced gastroenteritis is a frequent cause of hospitalization, and severe and sometimes fatal cases have been reported in immunocompromised children. The increasing recognition of NoVs as the cause of pediatr

Heat-Denatured Lysozyme Inactivates Murine Norovirus as a Surrogate Human Norovirus.

Human norovirus infects humans through the consumption of contaminated food, contact with the excrement or vomit of an infected person, and through airborne droplets that scatter the virus through the air. Being highly infectious and highly viable in the environment, inactivation of the norovirus requires a highly effective inactivating agent. In this study, we have discovered the thermal denaturing capacity of a lysozyme with known antimicrobial activity against gram-positive bacteria, as well as its inactivating effect on murine norovirus. This study is the first report on the norovirus-inac

Norovirus detection from sera of young children with acute norovirus gastroenteritis.

BACKGROUND: Antigenemia and viremia are common in rotavirus infection but only few studies have shown norovirus (NoV) RNA in the blood circulation. OBJECTIVES: To detect NoV RNA from serum of NoV-infected children and study if NoV RNAemia correlates with clinical severity of acute gastroenteritis. STUDY DESIGN: Serum specimens were collected from 176 Finnish children with acute NoV gastroenteritis. Semi-nested PCR was optimized to detect NoV capsid RNA from sera. NoV positive samples were further analyzed by sequencing. RESULTS: NoV RNA was found in 11/176 (6.3%) of serum specimens. NoV GII

Aerosol Transmission of Norovirus.

Norovirus (NoV) is a major cause of acute gastroenteritis outbreaks worldwide. A comprehensive understanding of the transmission mode is of great significance for the prevention and control of the NoV infection. Currently, the transmission modes of NoV include contact, food-borne, water-borne and aerosol transmission. The first three modes are more common, while aerosol transmission is seldom reported. In this paper, the source, generation mechanism, infectivity, sampling and related outbreaks of NoV aerosol are summarized and discussed.

Infection control for norovirus.

Norovirus infections are notoriously difficult to prevent and control, owing to their low infectious dose, high shedding titre, and environmental stability. The virus can spread through multiple transmission routes, of which person-to-person and foodborne are the most important. Recent advances in molecular diagnostics have helped to establish norovirus as the most common cause of sporadic gastroenteritis and the most common cause of outbreaks of acute gastroenteritis across all ages. In this article, we review the epidemiology and virology of noroviruses, and prevention and control guidelines

[Hospital hygiene. Norovirus infections].

Outbreaks of gastroenteritis caused by noroviruses have become an increasing problem for institutions in the health-care system over the past years. Staff members are also afflicted by the outbreaks of infection due to the highly contagious nature of noroviruses and this can lead to bottlenecks in health-care management and to economic losses. An acute gastroenteritis due to norovirus usually begins with severe nausea, heavy often projectile vomiting and a pronounced feeling of unwellness. In addition, there can be diarrhoea and abdominal cramps. The incubation time amounts to around one day.

Control of norovirus infection.

PURPOSE OF REVIEW: The purpose of the review is to provide an update on control measures for norovirus (NoV), which is the most commonly implicated pathogen in acute gastroenteritis and outbreaks, causing major disruption in nurseries, schools, hospitals and care homes. RECENT FINDINGS: Important developments include the discovery that virus particles, previously considered to be the infectious unit, also occur in clusters, which appear to be more virulent than individual virus particles; a working culture system using human stem-cell derived enteroids; promising results from early phase clin

[Norovirus gastroenteritis in adult].

Noroviruses cause epidemic and sporadic acute gastroenteritis in both children and adults. There is a wide range of genetic diversity among the noroviruses, which are subdivided into genogroups and further classified into genotypes. The onset of illness is typically abrupt and often includes vomiting and diarrhea. Fever is present in approximately half of patients. Although short duration of illness is characteristic, it can be severe and fatal among the patient with underlying illness. RT-PCR is currently the mainstay of diagnosis, but not clinical routine. Infectious dose is relatively low a

[Norovirus infections in Germany].

Noroviruses are responsible for the majority of acute viral gastroenteritis infections worldwide. Transmission may be faecal-oral or through contaminated food and water or airborne by virus-containing aerosols. Characteristics of noroviruses that facilitate their spread are their high concentration in stool and vomitus, their extreme environmental stability, their low infectious dose as well as the lack of long-lasting immunity. The majority of norovirus infections occur in large outbreaks among persons living in institutional settings, such as hospitals and nursing homes, although sporadic ca

Norovirus vaccines under development.

Noroviruses (NoVs) are one of the leading causes of acute gastroenteritis, including both outbreaks and endemic infections. The development of preventive strategies, including vaccines, for the most susceptible groups (children <5years of age, the elderly and individuals suffering crowding, such as military personnel and travelers) is desirable. However, NoV vaccine development has faced many difficulties, including genetic/antigenic diversity, limited knowledge on NoV immunology and viral cycle, lack of a permissive cell line for cultivation and lack of a widely available and successful anima

Impact of an emergent norovirus variant in 2009 on norovirus outbreak activity in the United States.

In October 2009, a new genogroup II, type 4 (GII.4) norovirus variant was identified in the United States. We collected norovirus outbreak data from 30 states to assess whether this new strain was associated with increased acute gastroenteritis activity. No increase in norovirus outbreaks was observed during the 2009-2010 winter.

Prolonged norovirus infection after pancreas transplantation: a case report and review of chronic norovirus.

Norovirus is a major cause of self-limited gastroenteritis worldwide. Prevention and treatment are thwarted by rapid viral evolution, and thus supportive care remains the mainstay of therapy. Chronic infection in immunocompromised hosts is increasingly described. We report a case of norovirus infection lasting 2543 days in a pancreas transplant recipient. Serial fecal specimens were obtained, from which a map of genetic relatedness was derived. The clinical course was complicated by renal failure that progressed to end-stage renal disease. Minimization of immunosuppression was associated with

Outbreak of norovirus illness in a college summer camp: impact of cleaning on occurrence of norovirus on fomites.

During the summer of 2005 an outbreak of norovirus acute gastroenteritis occurred in a residential college summer camp and was reported to the local health department. The outbreak spread rapidly to several other groups concurrently sharing the same facilities. During the investigation, fomites were sampled at different times in dorm rooms and tested for norovirus. The number of norovirus-positive rooms increased after the first room cleaning, from 40% to 73%. After the initial cleaning, the staff was instructed on proper cleaning and disinfection procedures and provided with disposable disinf
