Background and objective Ultrasound assessment of nuchal cord tightness may help identify fetuses at increased risk of cord accidents. Our objective was to assess ultrasound detection of simulated nuchal cord tension. Methods This was an ex vivo pilot study of human umbilical cords harvested from discarded placentas. The simulator consisted of a plastic bucket with a 5.1-cm-diameter cylindrical crossbar to simulate a fetal neck, filled with water and covered with a membrane. Using ultrasound, we measured deformation of the wrapped cord loop at 0, 2, 5, 7, and 10 lb of tension, using each cord'

When detected in a first trimester scan, an increased thickness of nuchal translucency (NT) may be associated with chromosomal, cardiac or genetic disorders. However, less attention has been devoted to the outcome of those fetuses who have confirmed normal anatomies and karyotyping, but have abnormal first trimester scans. Thus, a challenging new issue is how to counsel such cases of transient increased NT in which the translucency rapidly vanishes with no evidence of other underlying abnormalities. Two cases of transient increased thickness of NT are reported. In both, a nuchal cord was ultra

BACKGROUND: The nuchal cord is often blamed for problems that are encountered during delivery and is often cited as a major cause of fetal distress and perinatal mortality. However, the actual significance of nuchal cord on the outcome of an infant is controversial. A retrospective study was done to investigate the actual frequency of nuchal cords encountered in a hospital setting and to determine its effect on an infant as it passes through the birth canal and immediately after delivery.
METHODS: A retrospective study was carried out in 350 deliveries (out of total 367 deliveries) including

Research based on 182,492 births, wielding statistical power to uncover even mild negative associations, showed that both single and multiple nuchal cords at the time of delivery are not associated with adverse perinatal outcomes. Nuchal cords are associated with higher birth weights and lower cesarean section rates than births without nuchal cords. Color Doppler ultrasound, intended specifically to diagnose nuchal cords hours before delivery, has diagnosed the presence of 35-80% of the nuchal cords found on delivery, and has a false positive rate of 19%. The rate of nuchal cords increases wit

BACKGROUND: To investigate the intrapartum factors related to umbilical cord nuchal loops (nuchal cord) with particular reference to shoulder dystocia.
METHODS: We studied all singleton pregnancies with a vertex presentation and a birth weight of at least 2500 g from 1 January 1978 to 31 March 1997 and the 13,717 pregnancies with nuchal cord were compared to the 44,136 without nuchal cord.
RESULTS: When compared to pregnant women without nuchal cord, a greater proportion of pregnant women with nuchal cord underwent induction of labor (adjusted OR 1.09, 95% CI 1.04-1.15) and augmentation with

BACKGROUND: The effect of a nuchal cord on the outcome of delivery is controversial. The purposes of this study were to investigate the frequency of nuchal cords and determine the effect that nuchal cords have on the neonate.
METHODS: In a retrospective, case-control study, 706 consecutive infant deliveries in a community hospital were evaluated. Sixteen deliveries that were complicated by the umbilical cord entangled around an extremity or by a prolapsed cord were excluded from further analysis. The study group consisted of the 167 deliveries (23.7%) complicated by a nuchal cord. The remaini

BACKGROUND: Nuchal cords are rarely associated with significant neonatal morbidity or mortality.
CASE: A primigravida with a normal term pregnancy presented for diminished fetal movement. Several nuchal cord loops were found on the ultrasound imaging, with normal Doppler findings. Abnormal fetal heart rate pattern appeared after 6 hours of labor. A cesarean delivery was performed. A healthy baby was born with eight nuchal cord loops.
CONCLUSION: The management of nuchal cord with a single loop, suspected before delivery, seems clear: it should not influence the clinical management. However,

INTRODUCTION: Nuchal umbilical cord (NUC) is a possible complication of pregnancy which can be associated with adverse perinatal outcome.
MATERIALS AND METHODS: A retrospective study was done at the County Emergency Hospital Timisoara, Romania, between January 2009 and December 2010 and included cases with NUC at the time of delivery. Outcome variable related to the mothers and newborns were studied.
RESULTS: The incidence of NUC in the studied period was 8%. Most were primiparous. There were no significant statistical differences between vaginal births and cesarean section with one minute A

OBJECTIVE: The best practices for the care of a neonate born after a tight nuchal cord have not been defined. As a step toward this, we compared the outcomes of neonates born after a tight nuchal cord vs those born after a loose nuchal cord vs those born after no nuchal cord.
STUDY DESIGN: This was a retrospective comparison using electronic data of all deliveries during a 6-year period (2005 to 2010) in a multihospital healthcare system in the western United States. At the time of delivery, each birth was recorded as having a tight nuchal cord, a loose nuchal cord or no nuchal cord. Nuchal c

Nuchal cord, or cord around the neck of an infant at birth, is a common finding that has implications for labor, management at birth, and subsequent neonatal status. A nuchal cord occurs in 20% to 30% of births. All obstetric providers need to learn management techniques to handle the birth of an infant with a nuchal cord. Management of a nuchal cord can vary from clamping the cord immediately after the birth of the head and before the shoulders to not clamping at all, depending on the provider's learned practices. Evidence for specific management techniques is lacking. Cutting the umbilical c

OBJECTIVE: Our purpose was to evaluate the outcomes of pregnancies complicated by a multiple (double, triple, or quadruple) nuchal cord entanglement.
STUDY DESIGN: Computerized data from our University Hospital perinatal database were reviewed between 1990 and 1994. Only singleton, vertex, and term pregnancies undergoing labor were analyzed. Patients with active perinatal complications were eliminated to reduce bias. Pregnancies with infants with either a single or no nuchal cord entanglement served as comparison groups. A comparison of frequencies in the three groups was by chi 2 testing and

Two neonates who went into acute hypovolaemic shock due to a tight nuchal cord were successfully resuscitated. The occurrence of this life threatening complication in two low risk pregnancies emphasises the importance of having staff trained in resuscitation immediately available in the delivery unit.

OBJECTIVE: To evaluate the relationship between entanglement of umbilical cord around the fetal neck and adverse perinatal outcomes.
METHODS: In this prospective cohort study, perinatal outcomes of 218 pregnancies complicated with nuchal cord (NC) (study group) were compared with 190 uncomplicated pregnancies (control group). Main outcome measures were umbilical cord pH values, APGAR scores and cesarean section (C/S) rates. Fetal distress was stated as an abnormal heart rate pattern on electronic fetal heart monitorization.
RESULTS: There were no statistically significant differences in mate

OBJECTIVE: To identify nuchal cord risk factors.
METHODS: The present case-control study was carried out between December 2016 and April 2017 at two hospitals in Yaoundé, Cameroon. Singletons with a nuchal cord at delivery (case group) as well as the two neonates without a nuchal cord delivered immediately after each case (control group), all in cephalic presentation, were included. Undated pregnancies were excluded. The main variables studied were maternal age, parity, pregnancy duration at delivery, cord insertion site, cord length, delivery weight, and fetal sex.
RESULTS: A nuchal cord wa

PURPOSE: Birth asphyxia leading to acidosis comprises 20-60 % of perinatal mortality. Nuchal cord (NC) is one of the possible causes of birth asphyxia. Majority of fetuses who are antenatally detected to have nuchal cord are able to achieve successful vaginal birth. The purpose of this study was to analyze the effect of nuchal cord on fetal acid base status and perinatal outcome in vaginal deliveries.
STUDY DESIGN: 150 parturients were equally divided into three groups after vaginal delivery based on no NC, single and multiple loops. Umbilical cord arterial blood was analyzed for biochemical

The perinatal data of 550 fetuses with a nuchal cord at delivery were compared with those of a control group matched for gestational age, maternal age, and parity. The study group was subsequently subdivided into those fetuses with either single or multiple coils of nuchal cord. No significant differences were found in the comparison of routine ultrasound data between the two groups. The perinatal mortality rate and the prevalence of arterial pH less than 7.16, venous pH less than 7.20, and Apgar scores less than 7 at 5 and 10 minutes were similar in both groups. There was a significantly high
