Quality management of nuchal translucency measurement in residents.

PURPOSE: The measurement of the nuchal translucency (NT) in the 1st trimester is a sensitive, reliable method to assess the risk of specific fetal chromosomal and other defects. Training, however, is an issue not only among experienced sonographers, but especially for ObGyn residents, since all NT measurements in a true screening setting should fulfil the quality standards. The aim of this study was therefore the evaluation of the learning curve of residents and determination of the number of measurements necessary to achieve acceptable results. MATERIALS AND METHODS: Between 30th June, 1997

Nuchal translucency measurement in fetuses with spinal muscular atrophy.

OBJECTIVE: Spinal muscular atrophy (SMA) is an autosomal recessive neurodegenerative disorder with a high carrier frequency in the general population. The severity of this disorder indicates the importance of early prenatal detection. In medical literature, there are a few published case reports of enlarged nuchal translucency (NT) measurement in association with a diagnosis of SMA in the fetus. Our goal is to determine whether SMA in infants is associated with a history of an increased NT measurement during pregnancy. METHODS: Using contact information obtained through SMA family support gro

Nuchal translucency measurement in first trimester Down syndrome screening.

(1) Approximately three in every four fetuses with Down syndrome have increased nuchal translucency (NT), which is a larger than normal build-up of fluid at the back of the neck. (2) The ultrasound measurement of NT between 11 and 14 weeks' gestation, in combination with the mother's age and the levels of placental biochemical markers in her blood, can be used to detect approximately 84% of fetuses with Down syndrome. (3) The accuracy of NT measurement is affected by fetal position, measurement technique, the type of risk-calculation software used, and the sonographer's experience and technica

Optimal crown-rump length for measuring the nuchal translucency.

PURPOSE: To determine the optimal crown-rump length (CRL) for obtaining nuchal translucency (NT) measurements. METHODS: Women undergoing NT measurements by NT-certified sonographers within a combined screening program over an 18-month period were included in the study if they had a living fetus with a CRL of 45-84 mm. NT measurement success and screen-positive rates, transvaginal sonography (TVUS) use, and reasons for failed NT measurements were compared in 3 groups by CRL corresponding to 11-0/7 to 11-6/7 weeks (45-54 mm), 12-0/7 to 12-6/7 weeks (55-66 mm), and 13-0/7 to 13-6/7 weeks (67-84

Ultrasonographic Fetal Nuchal Translucency Measurements and Cytogenetic Outcomes.

IMPORTANCE: Ultrasonographic measurement of fetal nuchal translucency is used in prenatal screening for trisomies 21 and 18 and other conditions. A cutoff of 3.5 mm or greater is commonly used to offer follow-up investigations, such as prenatal cell-free DNA (cfDNA) screening or cytogenetic testing. Recent studies showed a possible association with chromosomal anomalies for levels less than 3.5 mm, but extant evidence has limitations. OBJECTIVE: To evaluate the association between different nuchal translucency measurements and cytogenetic outcomes on a population level. DESIGN, SETTING, AND

Assessment of sonographer nuchal translucency measurement performance--central tendency and dispersion.

OBJECTIVE: To assess center and sonographer nuchal translucency (NT) measurements that were performed as part of routine prenatal screening for Down syndrome. METHODS: Sonographers were assessed for measures of central tendency and dispersion by comparing them with expected population median and dispersion parameters. NT measurements were converted to multiples of the expected NT values according to CRL (NTMoM) and transformed to their log(10) equivalent (log(10) NTMoM). Central tendency and measurement were assessed by checking whether the median of the NTMoM distribution and SD of the log(1

Impact of tissue harmonic imaging on measurement of nuchal translucency thickness.

OBJECTIVES: To evaluate the effect of tissue harmonic imaging (THI) on the measurement of fetal nuchal translucency thickness (NT). METHODS: One-hundred and three pregnant women underwent first-trimester NT measurement according to The Fetal Medicine Foundation criteria. NT was evaluated using conventional ultrasonography (CUS) and THI without modifying any other ultrasound parameter (e.g. gain). For each patient three images with CUS and three images with THI were stored and then measured independently on the ultrasound system by two different operators. The maximum measurements using CUS an

Pregnancy outcome and nuchal translucency measurements in fetuses with a normal karyotype.

The aim of this study was to examine the relationship between nuchal translucency thickness and pregnancy and fetal outcome in fetuses with a normal karyotype and without structural malformations. Fetal nuchal translucency measurements were performed in 2088 chromosomally and structurally normal fetuses. In all fetuses the karyotype and pregnancy outcome were known. Likelihood ratios for different outcome measures were calculated. Spontaneous abortion, intra-uterine and neonatal death occurred in 2.4, 1.1 and 0.5 per cent respectively. The incidence of immature delivery was 1.1 per cent and of

Automatic measurement of fetal anterior neck lower jaw angle in nuchal translucency scans.

This study aims at suggesting an end-to-end algorithm based on a U-net-optimized generative adversarial network to predict anterior neck lower jaw angles (ANLJA), which are employed to define fetal head posture (FHP) during nuchal translucency (NT) measurement. We prospectively collected 720 FHP images (half hyperextension and half normal posture) and regarded manual measurement as the gold standard. Seventy percent of the FHP images (half hyperextension and half normal posture) were used to fit models, and the rest to evaluate them in the hyperextension group, normal posture group (NPG), and

Screening for fetal chromosomal abnormalities with nuchal translucency measurement in the first trimester.

OBJECTIVE: To describe the detection rate of first-trimester chromosomal abnormality screening with nuchal translucency (NT) measurement and maternal age in our population. METHODS: We have screened the fetuses between 11 to 14 weeks' gestation according to the Fetal Medicine Foundation's (London) instructions and used the FMF's software to assess the risk based on maternal age, crown-rump length (CRL) and NT. Fetal karyotyping was offered when screening for Down syndrome identified a risk greater than 1 in 300. Sensitivity and false-positive rates were calculated for different cut-offs. RES

Nuchal translucency measurements in consecutive normal pregnancies. Is there a predisposition to increased levels?

OBJECTIVE: The current study aims to assess the relative risk of isolated increased nuchal translucency (NT) in consecutive normal pregnancies. METHODS: A retrospective search of the fetal medicine unit database in which NT assessment was conducted. Only singleton fetuses with an unremarkable pregnancy outcome were included in this study. The repeated NT either at a threshold of >2.5 mm or > or =95th centile of the normal range for various CRLs (2 mm at a CRL < or =44 mm to 2.85 mm at CRLs 83-84 mm) was assessed. Pearson's correlation was applied to investigate any correlation between a log(1

Screening for trisomy 21 in monochorionic twins by measurement of fetal nuchal translucency thickness.

OBJECTIVE: To determine whether in screening for trisomy 21 by measurement of fetal nuchal translucency (NT) thickness in monochorionic twin pregnancies it is preferable to use the higher, smaller or average NT. METHODS: We retrospectively examined 769 monochorionic twin pregnancies that had undergone NT screening. The selection criteria were that first, in each pregnancy both fetuses were alive at the 11 to 13 + 6-week scan and second, the fetal karyotype had been determined by prenatal invasive testing or the pregnancy outcome was known. In each pregnancy the risk for trisomy 21 was calcula

The feasibility of nuchal translucency measurement in higher order multiple gestations achieved by assisted reproduction.

Nuchal translucency (NT) measurement for screening chromosomal abnormalities and detecting fetal anomalies is an effective ultrasonographic marker, originally developed for singleton pregnancies. This study sought to evaluate the feasibility of NT measurements in higher order multiple gestations. Pregnant patients who conceived following assisted reproduction and were carrying three or more fetuses were enrolled in the study. Each fetus was ultrasonographically assessed, a NT measurement was obtained, and the findings were used for counselling prior to any invasive procedure. In all, 24 pregna

Effect of deviation from the mid-sagittal plane on the measurement of fetal nuchal translucency.

OBJECTIVE: Screening for trisomy 21 by fetal nuchal translucency (NT) thickness at 11 to 13 + 6 weeks' gestation requires an appropriate examination of the NT. Errors in the assessment of NT may lead to an under- or overestimation of the NT and to incorrect patient-specific risks. In this study we aimed to examine the importance of the mid-sagittal section and whether the acquired plane of the head and face influences the measurement of the fetal NT thickness. METHODS: Sixty three-dimensional volumes of the fetal head and face in a mid-sagittal plane were acquired. NT thickness was firstly me

Is there still a role for nuchal translucency measurement in the changing paradigm of first trimester screening?

OBJECTIVES: To give an overview of the genetic and structural abnormalities occurring in fetuses with nuchal translucency (NT) measurement exceeding the 95th percentile at first-trimester screening and to investigate which of these abnormalities would be missed if cell-free fetal DNA (cfDNA) were used as a first-tier screening test for chromosomal abnormalities. METHODS: This is a national study including 1901 pregnancies with NT≥95th percentile referred to seven university hospitals in the Netherlands between 1 January 2010 and 1 January 2016. All cases with unknown pregnancy outcome were ex
