Hospital infections in Port Moresby General Hospital: studies in special care nursery.

Special Care Nursery of Port Moresby General Hospital has been experiencing too many infections particularly deaths from Septicaemia due to gram negative organisms. Bacteriological Survey in the Nursery reveals presence of too many pathogenic bacteria in the Nursery--in air, in beds, in incubators, in sinks--almost all over the Nursery. It also reveals that Nursing mothers and a good number of hospital workers are, in fact, carriers of dangerous pathogens. Proper cleaning and restructuring of the ward is essential. Based on the results of the study, measures have been recommended to deal with

The challenge of antibiotic-resistant Staphylococcus: lessons from hospital nurseries in the mid-20th century.

In the late 1940s, epidemics of antibiotic-resistant strains of Staphylococcus aureus began to plague postpartum nurseries in hospitals across the United States. Exacerbated by overcrowding and nursing shortages, resistant S. aureus outbreaks posed a novel challenge to physicians and nurses heavily reliant on antibiotics as both prophylaxis and treatment. This paper explores the investigation of the reservoir, mode of transmission, and virulence of S. aureus during major hospital outbreaks and the subsequent implementation of novel infection control measures from the late 1940s through the ear

[Nutritional status of vitamin A in mother/newborn pairs from 2 hospital nurseries in Rio de Janeiro, Brazil].

The aim of the present study was to assess maternal and newborn (umbilical blood) vitamin status in 220 mothers/newborns at birth from two public health centers in Rio de Janeiro, Brazil. The proportion of low retinol levels (cut-off point 1.05 mumol/L) in the umbilical cord of newborns (55.4%) was greater than found in their mothers (23.6%). A highly significant correlation (X2 = 14.2; p < 0.0001) was found between the levels of retinol of mothers and newborns. The overall prevalence of low levels of retinol in the mother was 23.6% whereas that of newborns was 55.4%. Umbilical cord mean conce

Infection and colonization by Staphylococcus aureus in a high risk nursery of a Brazilian teaching hospital.

Neonates are susceptible to nosocomial infections due to immunological immaturity, prolonged hospital stay and the use of invasive procedures. We evaluated the incidence of infections and the prevalence of colonization by MRSA (Methicillin-resistant Staphylococcus aureus) and MSSA (Methilin-susceptible Staphylococcus aureus), as well as colonization risk factors. Staphylococcal infections were observed by analyzing medical records in the HICS (Hospital Infection Control Service) and the HRN (High Risk Nursery). Additionally, four inquiries concerning colonization prevalence were made for S. au

Impact of prematurity on admissions to the neonatal nursery of a rural South African district hospital.

The objectives of this study were to determine causes of admission to a district hospital neonatal nursery; to describe outcomes; and to determine risk factors for these outcomes. The study was based at the neonatal nursery of Hlabisa hospital, KwaZulu/Natal; 149 consecutive admissions to the nursery between May and November 1995 were audited. The main outcome measures were diagnosis, gestational age, birthweight, critical event during admission (sepsis, severe vomiting, diarrhoea, jaundice, fits, apnoea), and outcome (discharged alive, death, discharged with deficit). Most admitted neonates (

Quality Improvement Reduces Excessive Serum Bilirubin Laboratory Draws in a Community Hospital Nursery.

BACKGROUND: Newborn jaundice is common and requires appropriate monitoring and treatment to prevent acute bilirubin encephalopathy and kernicterus. Minimizing total serum bilirubin (TsB) draws has many benefits, including decreasing blood loss and painful procedures, as well as saving time and costs. OBJECTIVE: The objective of this study was to test an iterative quality improvement process to reduce the percentage of newborns receiving TsB draws by half, ie, from 30.6% to 15.3%, at a community hospital nursery over 18 months. METHODS: Baseline data were collected retrospectively, and interv

Detection of cardiovascular abnormalities in the nursery of a general hospital in the Amazon region: correlation with potential risk factors.

Congenital heart diseases have been studied much more extensively in children than in neonates. In this study, we report on the findings from 57 neonates seen from June of 1995 through June 1996 in the nursery of a large public hospital in Belém, Pará, Brazil. All were routinely examined by a paediatrician just after birth, and, when indicated, these babies were referred to the cardiology unit of our Hospital for assessment by a paediatric cardiologist. Most of the diagnoses were made by means of Doppler and cross-sectional echocardiography with color flow mapping. Several abnormalities of the

The Six-Hour Window: How the Community Hospital Nursery Can Optimize Outcomes of the Infant with Suspected Hypoxic-Ischemic Encephalopathy.

Perinatal hypoxia is a devastating event before, during, or immediately after birth that deprives an infant's vital organs of oxygen. This injury at birth often requires a complex resuscitation and increases the newborn's risk of hypoxic-ischemic encephalopathy (HIE). The resuscitation team in a community hospital nursery may have less experience with complex resuscitation and post-resuscitation care of this infant than a NICU. This article provides the neonatal nurse in a Level I or Level II nursery with information about resuscitation and post-resuscitation care of an infant at risk of HIE w
