OBJECTIVES: To review contemporary knowledge and recent research in the nursing assessment and management of patients receiving radiation therapy.
DATA SOURCES: Published scientific papers, review articles, book chapters, and clinical practice.
CONCLUSIONS: Despite the enormous advances in treatment technology, dose-limiting acute and late effects of radiation therapy have not been eliminated. Astute assessment and early interventions can have a positive impact on the quality of life of the patient receiving radiation therapy.
IMPLICATIONS FOR NURSING PRACTICE: Oncology nurses need to be we

Perceptual dysfunction is a complex problem and can be a serious barrier to the patient's recovery of independence. Nurses play a significant role in the identification of a possible perceptual deficit and in the use of nursing interventions that complement and are coordinated with the work of the occupational therapist. Four common categories of perceptual dysfunction are identified. One-sided neglect and apraxia are emphasized and specific assessment data and nursing interventions for these two problems are discussed. Agnosia and visual-spatial relationship problems are described briefly. Th

The importance of the respiratory system to total body functioning cannot be overemphasized. It is imperative that the nurse realize that the first priority in trauma care is to maintain and support the respiratory system. The nurse must be able to rapidly and effectively perform a primary survey, recognize the clinical manifestations of life-threatening thoracic injuries, and intervene in the care of the patient to help stabilize and maintain the patient's respiratory function. The nurse must not only be able to asses, but must also be able to technically intervene rapidly and effectively. Ot

IMV provides the ICU nurse responsible for total multifunctional patient care with many advantages. First of all, it allows more time to perform other needed tasks. Time is made available for counseling the patient and family; consulting with the social worker, occupational therapist, and other staff members; and participating actively on team patient care conferences. IMV permits the patient to perform at an optimal level throughout the ICU stay and to participate in the treatment program. This is perhaps the greatest advantage of IMV for both the nurse and the patient. The patient's comprehe

Physical assessment for nursing purposes is necessarily different from that of medicine because the focus of nursing is to diagnose and treat human responses to disease rather than the disease itself. This article will consider nursing assessment, pathophysiology, implications for daily living, and nursing implications for selected functional aspects of movement, namely seeing, eating, expressing (facially), speaking, moving, and walking.

AIMS AND OBJECTIVES: To describe the nursing student-patient relationship in terms of three types of relationships-mechanistic, authoritative and facilitative-and analyse the factors related to the type of relationship.
BACKGROUND: As future professionals, nursing students have a central role in facilitating patient autonomy while working in partnership with patients. Supporting student-patient relationship throughout the nursing education may result in positive outcomes for both students and patients.
DESIGN: A cross-sectional study.
METHODS: The data were collected from a convenience samp

In recent times the need for physical care of residents of Dutch old people's homes has clearly increased. This is strongly related to the ageing of the population of these homes as well as to the introduction of legal measures concerning the admission of new residents. Staff members have also noted an increase in the mental and social problems of residents. In this article we describe a rating scale devised for the assessment of psychosocial problems in a home for the aged. The staff completed this scale and an ADL scale four times at three-month intervals. The prevailing problems appear to b

In this replication study pain assessment and patient perception by nurses was studied in relation to patient characteristics and nurse characteristics. Nurses were randomly assigned to one of 24 descriptions of a hypothetical patient of constant age and unspecified sex. The 24 descriptions varied by duration of pain, presence of a physical pathology, diagnosis category, and depression symptoms. Results showed that third- and fourth-year student nurses of a hospital based program, and registered nurses attributed more pain when test results of physical pathology were positive. Third- and fourt

Constipation results in infrequent or hard stools and pain during bowel movements and is a widespread issue across all age groups, but particularly among hospitalised older adults. Undertaking a holistic assessment of constipation in adults is essential to enable the nurse to identify any underlying causes, prevent complications and ensure effective management. Early recognition of constipation reduces the risk of faecal impaction, discomfort and associated conditions, which can affect patients' well-being and quality of life. This article describes the pathophysiology of constipation, the dia

BACKGROUND: Given the potential adverse effects of antipsychotics, high use in nursing homes creates concern. Our study goal was to explore the use of antipsychotics in relation to resident characteristics, and to assess the appropriateness of antipsychotic prescribing in Belgian nursing homes.
METHODS: Data were used from a cross-sectional study (Prescribing in Homes for the Elderly; PHEBE) conducted in 76 nursing homes in Belgium. Antipsychotics were classified into typical and atypical, using the anatomical therapeutic and chemical classification. Ten inappropriate antipsychotic prescribin

Many children's nurses have significant contact with children who have breathing difficulties and should be using systematic criteria to assess their nursing needs. Children's nurses do not appear to follow systematic criteria but are strongly influenced by the medical model and this may be detrimental to holistic assessment and the development of nursing diagnoses based on nursing needs. As nursing, along with other disciplines, develops evidence-based practice, children's nurses should develop evidence for best practice mindful of children's views, parental needs and the education of future

Nursing students require communication and spiritual assessment skills in the curriculum to provide whole-person palliative care to patients. This study used the communication style of motivational interviewing with the Faith, Importance, Community, Address care spiritual history tool to improve student confidence (N = 30) in connecting with others. Three qualitative themes demonstrated the potential for student growth: (1) active listening and nonjudgment allows people to open up, (2) the unknown territory of spirituality using Faith, Importance, Community, Address care as a guide, and (3) bu

OBJECTIVES: Twenty-five to thirty percent of hip fractures occur in men, and nursing home residents have a 5-10-fold greater fracture risk than community-dwellers. Osteoporosis prevalence in men in long-term care, however, is poorly defined. Our objectives were to determine the prevalence of osteoporosis, as assessed by peripheral bone mineral density (BMD), in a group of institutionalized veterans, and to determine how many men with low BMD had received a prior diagnosis of osteoporosis.
METHODS: Subjects were residents in a 740-bed skilled nursing facility (78% men). Male residents (n = 103

The aims of this study were to investigate the frequency of dysphagia among patients with acute stroke and compare non-dysphagic and dysphagic stroke patients regarding demographic aspects, performance in activities of daily living and type of neurological deficiency and to test a bedside screening tool to identify patients with dysphagia. The frequency of dysphagia was found to be 27%, or 40% if those patients who were unconscious, terminally ill or had a previous history of dysphagia were included. Overall stroke severity seemed to be an indicator for dysphagia, i.e. being significantly more

The promotion of continence is a multidisciplinary responsibility, with the nurse specialist's focus being on management, education, and ensuring a team approach. The patient's cognitive and functional abilities, general medical condition, and informed choice will ultimately determine the management plan. Behavioral techniques, supportive devices and intermittent catheterization are among the low risk interventions available to treat patients.

An accurate assessment of the skin and dermatologic lesions depends on the nurse, who is directly involved with patient care. However, demands of new technology, which supports an increasingly debilitated and aged patient population, makes it more difficult for the nurse to meet high standards of skin maintenance at a time when it should be a priority. Nursing assessment skills need not be extensive but confined to descriptively brief and familiar terms. Basic knowledge of the most common skin lesions allows the nurse to document in accurate and concise terms instead of nondescript generalizat
