An exploration of the factors that affect nurses' record keeping.

It is generally accepted that written documentation is significant not only as a communication medium in nursing, but also in the fulfilment of a number of other professional and legal obligations for nurses. The literature suggests, however, that the records nurses keep often fail either to represent the care delivered or the patient's health status accurately. This article explores a number of possible reasons for this, including: nurses' apathy to documentation; the time-consuming nature of documentation; discrepancies between nurse/patient and nurse/nurse interpretation of the significance

Trial of Converting Nursing Records into Simplified and Structured Information Utilizing ChatGPT-3.5.

In Japan, the excessive length of time required for nursing records has become a social problem. A shift to concise "bulleted" records is needed to apply speech recognition and to work with foreign caregivers. Therefore, using 96,000 descriptively described anonymized nursing records, we identified typical situations for each information source and attempted to convert them to "bulleted" records using ChatGPT-3.5(For return from the operating room, Status on return, Temperature control, Blood drainage, Stoma care, Monitoring, Respiration and Oxygen, Sensation and pain, etc.). The results showe

Manifestations and Potential Consequences of Information Distortion in Electronic Nursing Records: Qualitative Study.

BACKGROUND: Nursing records are a critical reflection of nursing practices and play a key role in ensuring the quality of patient care. However, these records sometimes fail to accurately represent the real-world health truth, resulting in information distortion. The manifestations and potential consequences of such distortion remain understudied, which may hinder efforts to improve nursing practices. OBJECTIVE: This study aims to explore the primary manifestations and potential consequences of information distortion in electronic nursing records (ENRs). METHODS: We conducted semistructured

Traces of spiritual care in nursing records: A qualitative study of cancer care.

BACKGROUND: Serious illnesses, such as cancer, bring the threat of loss of health and life closer. This may compromise spiritual well-being. Addressing patients' spirituality is essential in nursing care. Therefore, nursing records should reflect, clarify, and enable spiritual-care follow-up. AIM: The aim of this study is to explore how spiritual care is expressed in nursing records in cancer care. METHOD: This study adopted a hermeneutic approach, and a qualitative content analysis was used to explore the nursing records of 43 inpatients with cancer from Norway. Ethical approval was obtaine

Care of elderly lower limb amputees, as described in medical and nursing records.

The aim of this study was to characterize elderly lower limb amputees and explore problems/requirements inherent in their care. A retrospective study of medical and nursing records of patients who had undergone lower limb amputation (LLA) at Uddevalla General Hospital in 1997 was conducted. Demographic data were compared with those from a comparable regional health care district. Hospitalization, rehabilitation and nursing-related data related to subjects alive after 6 months were compared with data concerning those deceased during hospital stay and within 6 months after amputation. During the

An automated medical record system for a skilled nursing facility.

This paper reports on the development of an on-line automated medical record system suitable for nursing homes. The software was written in standard MUMPS (Massachusetts General Hospital Utilities Multi-Programming System) and is easy for the first-time user to operate. The data managed by the system may be divided into four major components, including text tables, the hierarchical problem dictionary, the patient admission data, and the care plan data. The system met all of the project's objectives and has been enthusiastically received by the nursing home staff.

[The record book of a general nursing station--a reflection of daily interventions?].

There were analyzed report books of three nursing stations to win starting points for a milieu therapeutical reform. The methodical inventory included the analysis of the contents as well as rules of functional-communicating linguistic description. The qualitative analysis showed that most texts should be classified as texts of the class 'inform'. The members of the staff fall back upon the emotional report in problematical daily situations. The quantitative analysis of the dates showed the tendency for describing live at the nursing stations in the report books with routine notes. In cases th

Staff utilisation of an electronic medical record during nursing handover.

BACKGROUND: Electronic medical records (EMRs) are widely implemented to improve information access, patient safety and care coordination. However, less is known about how EMRs are used in routine clinical processes such as nursing handover. Understanding how EMRs influence handover practices is important, given the central role of handover in continuity of care and patient safety. METHOD: An exploratory mixed-methods comparative case study using a cross-sectional survey design was conducted within a large multi-site public healthcare organisation in Victoria, Australia. An organisation-wide s

Nursing Students' Experiences of Electronic Medical Records During Placements: A Mixed-Method Sequential Explanatory Design.

As the adoption of electronic medical records has become a global trend, the need to integrate electronic medical records education into undergraduate nursing curricula has been internationally recognized. There are very few nursing colleges in China put an emphasis on the skills required to properly use nursing informatics. This study explored Chinese nursing students' experiences with Electronic Medical Records during clinical placements. This research used a sequential explanatory mixed-methods design. First, a survey of 355 participants assessed electronic medical records exposure, attitud

A Nursing Homecare Data Science Investigation Using "Persons with ALS" (PALS) Electronic Health Records (EHRs).

BackgroundData science, rooted in computer science, statistics, and information science is advancing healthcare research by unlocking "insights" from big data to discover knowledge about patient experiences and answer previously unanswerable questions. The comparatively limited engagement of Canadian nurse researchers in this field, relative to counterparts in other jurisdictions (e.g., United States) served as a key impetus for initiating this data science study.PurposeTo investigate homecare electronic health records (EHRs) of "persons with ALS" (PALS) disease and identify care-related facto

Silent Pain or Silent Records? Pain Visibility, Documentation Ethics, and Nursing Management in Neurocritical Care.

Pain assessment in critical care is organised primarily around patient self-report and numerical scoring, an arrangement that protects the patient's first-person authority. In neurocritical care, however, critically ill patients with stroke may be unable to communicate because of aphasia, impaired consciousness, sedation, mechanical ventilation, or delirium. Under these conditions, the absence of a numerical score may reflect limitations in the systems through which pain is made visible rather than the absence of pain. This paper offers an empirically informed philosophical analysis of what a

The graduate record examination: help or hindrance in nursing graduate school admissions?

Do graduate record examination (GRE) scores serve as strong predictors of student success in graduate school in nursing, and if so, is the extent to which they may indicate success outweighed by their perceived role as a barrier to application to graduate school in nursing? Academic ability, defined as cumulative grade point average (GPA), was used as the outcome indicator for success in graduate school and was compared with admission GRE scores for 217 students admitted to graduate programs at the University of Washington School of Nursing over a 1-year period. The GRE presented a large barri
