Intracranial pathology in young children with apparently isolated nystagmus.

PURPOSE: To assess the incidence of intracranial pathology in young children who had brain magnetic resonance imaging (MRI) to investigate apparently isolated nystagmus noted within the first few years of life. METHODS: Retrospective institutional review of such children (up to 5 years old). RESULTS: Twenty-six patients (17 boys, 9 girls) were identified. Nystagmus was clinically bilateral in 22 (85%) and unilateral in 4 (15%). Seventeen cases (65%) were described as horizontal pendular nystagmus, 5 (19%) as horizontal jerk nystagmus, 2 (8%) as vertical pendular nystagmus, and 1 each as down

[Importance of the general radiograph of the cervical spine in vertigo and pathologic proprioceptive cervical nystagmus].

The general roentgenogram of the cervical spine often shows manifold pathological alterations by patients without any clinical complaints. On the other hand, there are cervical vertigo and different symptoms caused by the cervical spine, its joints, muscles and ligaments without any X-ray manifestation. Nevertheless, some connections are assumed to exist between vertigo and alterations of the cervical spine an assumption supported especially by the frequently successful elimination of cervical troubles by chirotherapy. We compared the X-ray manifestations in patients complaining of vertigo wit

Clinical implications of head-shaking nystagmus in central and peripheral vestibular disorders: is perverted head-shaking nystagmus specific for central vestibular pathology?

BACKGROUND AND PURPOSE: The patterns of head-shaking nystagmus (HSN) aid in differentiation between central and peripheral vestibular disorders, and perverted HSN (pHSN) has been considered a central sign. The aim was to determine the characteristics of HSN in a large number of patients with either peripheral or central vestibular disorders in a dizziness clinic of a university hospital. METHODS: The medical records of 7544 dizzy patients were reviewed during a year and 822 patients with a clinical diagnosis of vestibular disorders were recruited. The findings of spontaneous nystagmus (SN) an

Prevalence and Characteristics of Physiological Gaze-Evoked and Rebound Nystagmus: Implications for Testing Their Pathological Counterparts.

Objective: Cerebellar diseases frequently affect the ocular motor neural velocity-to-position integrator by increasing its leakiness and thereby causing gaze-evoked nystagmus (GEN) and rebound nystagmus (RN). Minor leakiness is physiological and occasionally causes GEN in healthy humans. We aimed to evaluate the characteristics of GEN/RN in healthy subjects for better differentiation between physiological and pathological GEN/RN. Methods: Using video-oculography, eye position was measured in 14 healthy humans at straight ahead eye position before and after, and during 30 s of ocular fixation a

Comprehensive Review of Nystagmus and Vertigo Diagnostics: From Pathological Foundations to AI-Driven Telemedicine.

Nystagmus, the involuntary rhythmic oscillation of the eyes, is a critical diagnostic marker in vestibular medicine, distinguishing life-threatening central disorders such as stroke from benign peripheral conditions including Benign Paroxysmal Positional Vertigo (BPPV). Despite its clinical importance, accurate nystagmus assessment has long been constrained by expensive infrared video-oculography equipment such as videonystagmography, specialist dependency, and the episodic nature of vestibular symptoms that are often resolved before a clinical encounter. This review synthesizes approximately

Direction-fixed positional nystagmus following head-roll testing: how is it related with a vestibular pathology?

OBJECTIVE: The goal of this study is to analyze the clinical view of patients with direction-fixed positional nystagmus (DFPN) following head-roll maneuver. METHODS: Sixty patients with DFPN were reviewed retrospectively. Patients were categorized into 3 groups according to the direction of nystagmus based on rotation side. Associated problems were documented, and cumulative data were compared between groups. One-way analysis of variance (ANOVA test) was used for statistical analysis (P < 0.05). RESULTS: Thirty-three patients (55%) had stronger nystagmus beating towards the direction of head

Approach to the Examination and Classification of Nystagmus.

BACKGROUND AND PURPOSE: Physical therapists caring for patients with neurologic or vestibular disorders must routinely examine and characterize nystagmus and other oscillatory eye movements. Often, the diagnosis hinges on proper interpretation of the nystagmus pattern. This requires understanding the terminology surrounding the numerous attributes and influencing factors of nystagmus, a systematic approach to the examination, and a classification structure that guides practitioners to the specific nystagmus type and subsequent evaluation and management. SUMMARY OF KEY POINTS: Nystagmus is an

Pathologic correlates of electronystagmographic tracings.

Electronystagmographic tracings were correlated with the histopathologic findings in the peripheral vestibular system of 66 temporal bones of 34 patients who complained of dizziness. Although some findings (e.g., spontaneous nystagmus, unilaterally reduced caloric response, and directional preponderance) seemed to have doubtful value in localizing vestibular system disease, other findings indicated a peripheral site of the vestibular lesions. Bilaterally reduced or bilaterally absent caloric responses were consistently associated with pathologic changes in the peripheral vestibular system. Sim

The Developmental Eye Movement Test Does Not Detect Oculomotor Problems: Evidence from Children with Nystagmus.

SIGNIFICANCE: The Developmental Eye Movement (DEM) test, a test purported to assess oculomotor skills, does not detect eye movement disorder in nystagmus syndromes. The test should not be used for the clinical evaluation of oculomotor disorders. PURPOSE: The DEM test ratio compares a horizontal number naming subtest with a vertical one to identify oculomotor problems independent of a child's visual-verbal naming skills. Here, we tested the construct validity of this method by comparing scores of children with and without pathologic nystagmus. Such a nystagmus disturbs normal fixation and sacc

Vestibulo-oculomotor disturbances caused by industrial solvents.

Animal experiments show that intoxication with hydrocarbon solvents influences the vestibulo-oculomotor reflex arc and indicate that the solvents block the inhibition of the vestibulo-oculomotor reflex presumably exerted by the cerebellum. The blood concentrations necessary to elicit oculomotor disturbances in animals were smaller than those that disturbed the vestibular function. Accordingly, in human experiments styrene and toluene exposure did not elicit any positional nystagmus but caused an increased saccade speed and a diminished visual suppression of vestibular nystagmus. Some cerebella

Nystagmus.

Advances in understanding the organization of the ocular motor system, including its anatomy and pharmacology, have provided new insights into the pathogenesis of various forms of nystagmus. The discoveries of fibromuscular pulleys that govern the pulling directions of the extraocular muscles has provided a new conceptual framework to account for the different axes of rotation of vestibular and other types of movements that may contribute to nystagmus. Theoretical and experimental evidence has suggested that acquired pendular nystagmus, which is commonly due to multiple sclerosis, arises from

Nystagmus.

This article reviews the recent literature on nystagmus and various aspects of the pathophysiology of congenital idiopathic nystagmus and nystagmus treatment. One paper shows a new classification of latent/manifest latent nystagmus based on eye movement recordings. Nystagmus associated with complex syndromes and with onset in childhood represents the subject of several important recent articles, as does acquired nystagmus. Nystagmus as a manifestation of the toxicity of pharmacological agents is becoming increasingly recognized. Important contributions have been made to the genetics of various

Lessons from follow-up examinations in patients with vestibular neuritis: how to interpret findings from vestibular function tests at a compensated stage.

OBJECTIVES: Most patients complaining of dizziness seek medical services in the interictal period, which is thought to be a compensated stage. Thus, we wanted to investigate the results of vestibular function tests (VFTs) at a compensated stage in patients with vestibular neuritis to determine the presence and the sides of vestibular hypofunction. STUDY DESIGN: Retrospective case series review. METHODS: We analyze the results of VFT including spontaneous nystagmus (SN), caloric, vibration-induced nystagmus (VIN), head-shaking nystagmus (HSN), and subjective visual vertical (SVV) tests in 38

Vestibular and oculomotor disturbances in pathology of the fourth ventricle.

Positional nystagmus of the central type and gaze paretic (gaze evoked) nystagmus have been reported in pathology of the posterior fossa. It has also been reported that positional nystagmus of the central type is only infrequently accompanied by vertigo and nausea. In this paper five cases are discussed which showed positional nystagmus of the central type and/or gaze paretic nystagmus. All cases showed severe vertigo, nausea and vomiting in the provocative head position. In four out of five cases the pathology was found to be in the area of the IVth ventricle. On the basis of recent neurophys

Acquired nystagmus.

Nystagmus is the repetitive to and fro movement of the eyes, which may be physiological or pathological. The movements can be horizontal, vertical, torsional or a combination of these movements. It starts by a slow movement of the eye away from the visual target. The second movement brings the eye back to the visual target. If this second movement is quick, the nystagmus is called jerk nystagmus. If the second movement is slow, the nystagmus is said to be pendular. Maintaining steady gaze is dependent upon visual fixation, the vestibulo-ocular reflex and the gaze-holding neural integrator syst
