Prevalence and Characteristics of Physiological Gaze-Evoked and Rebound Nystagmus: Implications for Testing Their Pathological Counterparts.

Objective: Cerebellar diseases frequently affect the ocular motor neural velocity-to-position integrator by increasing its leakiness and thereby causing gaze-evoked nystagmus (GEN) and rebound nystagmus (RN). Minor leakiness is physiological and occasionally causes GEN in healthy humans. We aimed to evaluate the characteristics of GEN/RN in healthy subjects for better differentiation between physiological and pathological GEN/RN. Methods: Using video-oculography, eye position was measured in 14 healthy humans at straight ahead eye position before and after, and during 30 s of ocular fixation a

Visual Function, Brain Imaging, and Physiological Factors in Children With Asymmetric Nystagmus due to Chiasmal Gliomas.

PURPOSE: Asymmetric nystagmus can be an important presenting sign of optic pathway gliomas in young children. We investigated the causes of asymmetric nystagmus in children with chiasmal or suprasellar optic pathway gliomas compared with children with similar optic pathway gliomas and stable gaze. METHODS: Longitudinal magnetic resonance imaging before and after treatment, age-corrected visual acuity, ocular examinations, video-oculography, visual evoked potentials, and retinal nerve fiber layer thickness were retrospectively reviewed. RESULTS: Twenty-two children were included (eight with a

Stochastic Physiological Gaze-Evoked Nystagmus With Slow Centripetal Drift During Fixational Eye Movements at Small Gaze Eccentricities.

Involuntary eye movement during gaze (GZ) fixation, referred to as fixational eye movement (FEM), consists of two types of components: a Brownian motion like component called drifts-tremor (DRT) and a ballistic component called microsaccade (MS) with a mean saccadic amplitude of about 0.3° and a mean inter-MS interval of about 0.5 s. During GZ fixation in healthy people in an eccentric position, typically with an eccentricity more than 30°, eyes exhibit oscillatory movements alternating between centripetal drift and centrifugal saccade with a mean saccadic amplitude of about 1° and a period in

Skull Vibration-Induced Nystagmus Reveals Hidden Vestibular Asymmetry Dissociated from vHIT, VEMPs and Velocity-Storage-Related Responses: A Clinical-Physiological Case Series and Temporal-Domain Hypothesis.

BACKGROUND/OBJECTIVES: Skull vibration-induced nystagmus (SVIN) is a robust sign of vestibular asymmetry, yet its relationship with tests of impulsive canal function, short-latency otolithic reflexes, and sustained visuo-vestibular processing remains incompletely defined. To describe four clinical-physiological cases in which SVIN was dominant or disproportionate relative to the canal, otolithic-reflex, and sustained-domain profiles, and to propose a temporal-domain interpretation of this physiological non-congruence. METHODS: Four patients with persistent disequilibrium, episodic vertigo, or

Approach to the Examination and Classification of Nystagmus.

BACKGROUND AND PURPOSE: Physical therapists caring for patients with neurologic or vestibular disorders must routinely examine and characterize nystagmus and other oscillatory eye movements. Often, the diagnosis hinges on proper interpretation of the nystagmus pattern. This requires understanding the terminology surrounding the numerous attributes and influencing factors of nystagmus, a systematic approach to the examination, and a classification structure that guides practitioners to the specific nystagmus type and subsequent evaluation and management. SUMMARY OF KEY POINTS: Nystagmus is an

Infantile and acquired nystagmus in childhood.

Nystagmus is an involuntary, periodic eye movement caused by a slow drift of fixation which is followed by a fast refixation saccade (jerk nystagmus) or a slow movement back to fixation (pendular nystagmus). In childhood most cases are benign forms of nystagmus: idiopathic infantile, ocular or latent nystagmus. They arise at the age of 3 months, without oscillopsia and show the absence of the physiologic opto-kinetic nystagmus. A full ophthalmologic evaluation is all that is needed in most cases: albinism, macular or optic nerve hypoplasia and congenital retinal dystrophies are the most common

Nystagmus and Saccadic Intrusions.

PURPOSE OF REVIEW: This article provides an overview of nystagmus and saccadic intrusions with the goal of facilitating recognition and differentiation of abnormal eye movements to assist with accurate diagnosis of neurologic disease and evidence-based specific treatment of oscillopsia. Myriad advances have been made in the understanding of several types of nystagmus and saccadic intrusions, even in the past 5 to 10 years, especially regarding underlying pathophysiology, leading to pharmacologic advances rooted in physiologic principles. RECENT FINDINGS: Specific recent advances in the study

Epidemiology of Adult-Onset Nystagmus and Saccadic Intrusions of Central Etiologies: A Population-Based Study in Olmsted County.

BACKGROUND: The incidence and clinical characteristics of adult-onset abnormal spontaneous eye movements of central origin remain poorly defined. METHODS: We conducted a retrospective, population-based study using the Rochester Epidemiology Project to identify all adults (age ≥18 years) in Olmsted County, MN, diagnosed with nystagmus or saccadic intrusions between 1970 and 2019. Cases involving childhood-onset nystagmus, physiologic, or peripheral vestibular nystagmus, as well as those with insufficient diagnostic information, were excluded. Fixational Eye Movement Waveforms in Amblyopia: Characteristics of Fast and Slow Eye Movements.

Fixational eye movements comprise of fast microsaccades alternating with slow intersaccadic drifts. These physiologic eye movements play an important role in visual perception. Amblyopic patients are known to have fixation instability, particularly of the amblyopic eye. We examined eye movement abnormalities that contribute to this instability. We found that fixation stability is affected by the presence of fusion maldevelopment nystagmus (FMN). However, some amblyopes can have nystagmus without nasally directed slow phases and reversal in direction of the quick phase on ocular occlusion, feat

Fixation instability in amblyopia: Oculomotor disease biomarkers predictive of treatment effectiveness.

Amblyopic patients are known to have fixation instability, particularly of the amblyopic eye. The stability of the fixation is affected by the presence of nystagmus, the frequency and amplitude of fixational saccades and inter-saccadic drifts. Amblyopic patients without nystagmus have increased amplitude of the fixational saccades with reduced frequency of the physiologic microsaccades and have increased inter-saccadic drifts. Amblyopia patients who have experienced a disruption in binocularity in early infancy develop fusion maldevelopment nystagmus (FMN) previously called latent nystagmus as

Acquired nystagmus.

Nystagmus is the repetitive to and fro movement of the eyes, which may be physiological or pathological. The movements can be horizontal, vertical, torsional or a combination of these movements. It starts by a slow movement of the eye away from the visual target. The second movement brings the eye back to the visual target. If this second movement is quick, the nystagmus is called jerk nystagmus. If the second movement is slow, the nystagmus is said to be pendular. Maintaining steady gaze is dependent upon visual fixation, the vestibulo-ocular reflex and the gaze-holding neural integrator syst

Exploring Generative Pre-Trained Transformer-4-Vision for Nystagmus Classification: Development and Validation of a Pupil-Tracking Process.

BACKGROUND: Conventional nystagmus classification methods often rely on subjective observation by specialists, which is time-consuming and variable among clinicians. Recently, deep learning techniques have been used to automate nystagmus classification using convolutional and recurrent neural networks. These networks can accurately classify nystagmus patterns using video data. However, associated challenges including the need for large datasets when creating models, limited applicability to address specific image conditions, and the complexity associated with using these models. Central positional nystagmus in inferior cerebellar peduncle lesions: a case series.

We aimed to characterize the central positional nystagmus (CPN) observed in lesions involving the inferior cerebellar peduncle (ICP). We analyzed the clinical and neurotologic findings in six patients with an isolated unilateral ICP infarction that had been diagnosed at a university hospital in South Korea from 2003 to 2019. Patients usually presented with acute vestibular syndrome in isolation (4/6, 67%). Ipsilesional spontaneous nystagmus was observed in five while supine (5/6, 83%). Four (4/6, 67%) patients showed CPN which included apogeotropic (n = 3) or geotropic (n = 1) during head turn

Nystagmus.

Ophthalmologic nystagmus can be congenital and manifest/latent both of whose waveforms, compensatory mechanisms, and treatment are different. Physiologically, latent nystagmus may be reversed at will, whereas nystagmus and head nodding may be simultaneously triggered at will. Occasional oscillopsia is more frequent than is usually believed; extraretinal signals may be a reason for their absence in congenital nystagmus. Astigmatism could be the consequence of nystagmus, myopia is present in most of the cases of congenital nystagmus with blocking convergence, whereas depth-of-focus anomalies are

[Differential diagnosis of dizziness (author's transl)].

The common denominator of the different meanings of dizziness is a disturbance of spatio-postural orientation, which indicates an imminent danger of fall. Höhenschwindel (fear of hights) with grasping and holding in the primate is instinctive behavior caused by a visual cliff or sudden dizziness. Because of the many possible meanings of dizziness, the case history is important for differential diagnosis. Vertiginous and non-vertiginous dizziness, black out and unsteady gait without vertigo should be distinguished. In addition, the duration and time course of the attack, releasing mechanisms an
