[Obturator nerve block].

Obturator nerve block is commonly used for transurethral resections of the bladder in order to inhibit reflectory adductor muscle reaction during electrocoagulation and to reduce the risk of bladder wall perforation during transurethral surgery. Furthermore, obturator block is used to complete regional blocks for major knee surgery in addition to femoral and sciatic nerve blocks. Continuous techniques are sometimes used to treat chronic pain problems such as adductor spasm. During a so called "3 in 1" block (femoral nerve block) the obturator nerve will only be anaesthetized in 0-62% of the pa

Obturator nerve entrapment.

PURPOSE: To describe a case of obturator nerve entrapment, a previously unreported cause of chronic groin pain in athletes. CASE SUMMARY: A 23-year-old man, an elite Australian rules footballer, presented with a 2-year history of groin pain. Conservative treatment was unsuccessful. Examination postexercise revealed adductor weakness and medial thigh paraesthesia. Diagnosis of obturator nerve entrapment was confirmed by EMG and nerve block. Surgical neurolysis was performed. The patient returned to sport and has remained symptom free. DISCUSSION: Although intrapelvic obturator nerve entrapmen

A Morphometric Study of the Obturator Nerve around the Obturator Foramen.

ObjectiveObturator neuropathy is a rare condition. Many neurosurgeons are unfamiliar with the obturator nerve anatomy. The purpose of this study was to define obturator nerve landmarks around the obturator foramen.MethodsFourteen cadavers were studied bilaterally to measure the distances from the nerve root to relevant anatomical landmarks near the obturator nerve, including the anterior superior iliac spine (ASIS), the pubic tubercle, the inguinal ligament, the femoral artery, and the adductor longus.ResultsThe obturator nerve exits the obturator foramen and travels infero-medially between th

The location of the obturator nerve: a three-dimensional description of the obturator canal.

Satisfactory analgesia cannot be achieved in every obturator nerve block. To attempt to improve the success rate of obturator nerve block, this study describes the detailed anatomy of the obturator region and canal. Eleven (5 female and 6 male) cadavers, totally 22 sides were dissected. Anatomical positions of the structures entering and leaving the canal were defined. The position of the obturator nerve and its branches and their relation with the obturator artery, vein, and with the internal iliac and femoral veins were investigated. A mould of the canal and a model were created. Detailed me

Obturator Nerve Variations: A Narrative Review.

The aim is to understand the clinical significance of these variations in surgeries and diseases. To conduct this review, we used the PubMed database, considering factors such as the availability of full texts, the language and relevance to the topic, in order to acquire comprehensive and detailed findings. After applying our exclusion criteria, we narrowed the findings down to 11 useful results. Throughout our study, we observed significant variations concerning the obturator nerve. The nerve displays diverse paths and trajectories, leading to deviations from the commonly accepted anatomical

[Obturator nerve block in transurethral surgery].

The obturator nerve passes in close proximity to the bladder as it courses through the pelvis. During transurethral operations, resection may result in stimulation of the obturator nerve, causing violent adductor contraction. Bladder perforation and incomplete tumor resection are the most important complications. All techniques proposed since transurethral surgery began, until nowadays are reviewed: neuromuscular blockade, electric circuit modifications, transparietal endoscopic blockade, periprostatic and subvesical infiltration, obturator nerve blockade and the "3 in 1 block" described by Wi

[Inguinal Approach in Obturator Nerve Block].

The most commonly utilized approaches to obtura- tor nerve block are the pubic approach and the inter- adductor approach. However, the pubic approach is difficult and extremely invasive because a needle must be inserted into the pelvis and there have been some cases of vascular puncture using the pubic approach. Moreover, some cases accompanied clinical signs of local anesthetic toxicity using both approaches. Thus, we devised and performed the inguinal approach, where the femoral artery and vein are shifted outward in the level of the inguinal crease and the needle is inserted vertically from

Surgical management of obturator nerve lesions.

OBJECTIVE: Obturator neuropathy is a rare condition that may result from orthopedic, gynecological, or urological procedures or injuries. These pathologies are amenable to surgical intervention, and an accurate physical examination and electromyography are necessary before treatment. METHODS: Six patients with obturator nerve lesions underwent surgical exploration and decompression or primary repair. Their charts were retrospectively reviewed, and the electromyographic, surgical, and follow-up data were recorded. RESULTS: Postoperatively, all patients reported improved symptoms in the form o

The Role of the Obturator Nerve-Acute Nerve Grafting After Resection of a Plexiform Obturator Nerve Schwannoma: A Case Report.

CASE: A 45 year-old woman presented with an obturator nerve schwannoma (ONS), which was originally suspected to be a gynecologic malignancy. After thorough workup, the mass and obturator nerve were resected en bloc, and sural nerve grafting was performed. The patient had minimal deficits with return to baseline activities by 7 weeks postoperatively. CONCLUSION: We guide the reader through the challenging diagnosis of ONS, which may mimic many other intrapelvic masses. The treatment decision of whether to acutely reconstruct the obturator nerve is also discussed, along with implications and fu

Laparoscopic obturator nerve neurolysis after pelvic lymphadenectomy.

Postoperative neuropathy is a known complication of major pelvic oncologic surgery. Obturator nerve injury complicating pelvic lymph node dissection has been reported in the literature. Severity and duration of symptoms are determined by the severity of the initial nerve lesion. Different management options have been described in the literature. We report here on a patient who developed right obturator neuropathy after pelvic lymphadenectomy. Initial management by physical therapy was unsuccessful, and the patient underwent a successful laparoscopic surgical neurolysis. Whereas preventive meas

[Effective radiofrequency lesioning for obturator nerve neuropathy].

A 73-year-old female complained of pain in the right lower-abdomen and medial aspect of the distal thigh after a gynecological operation. Computer tomography and MRI findings excluded obturator canal herniation or lumbar plexopathy. Both lumbar epidural block and obturator nerve block with local anesthetics were effective, except their limited duration of pain relief and muscle weakness for several hours after the blockade. Treatment with radio-frequency lesion of 50 degrees C for 60 seconds of the right obturator nerve relieved her pain for 9 months without muscle weakness. This case shows us

Obturator nerve injuries during intra-abdominal surgery.

Selective obturatory nerve lesions are described to occur relatively infrequent. In the present paper two patients developing signs and symptoms of an obturatory neuropathy as a result of abdominal surgery were reported. In a 48-year-old woman a bilateral obturator neuropathy occurred after bilateral ovarectomy with a weakness of the adductor muscles, typical electromyographic findings and sensory deficits in the medial aspect of the thigh. Similar unilateral signs and symptoms were seen in a 65-years-old man as a result of an aortofemoral bypass operation. Symptomatology and differential diag

Obturator nerve injury diagnosed by nerve conduction: a case report.

The obturator nerve lies deep within the pelvis, and it can be damaged by direct injury during surgery. In this study, nerve conduction was used to confirm an obturator nerve injury in a patient who presented with hip adductor weakness following gynecological surgery for endometrial cancer. A 56-year-old woman complained of weakness in the right adductor muscles after a laparoscopic hysterectomy due to endometrial cancer. Seven days after surgery, the degree of weakness of the right hip adductor was Medical Research Council (MRC) Scale 1; thus, a nerve conduction velocity test was conducted. T

Ectopic pregnancy presenting with obturator nerve pain.

A 27 year old woman had a three day history of pain in the cutaneous distribution of the left obturator nerve before she developed the classical picture of ectopic pregnancy with lower abdominal pain and vaginal bleeding. A left tubal pregnancy was subsequently confirmed by laparoscopy. Referred pain along the obturator nerve has been reported in other pelvic conditions, but has not previously been reported as a manifestation of ectopic pregnancy. Ectopic pregnancy may present with a very wide range of signs and symptoms and should be excluded in females of child bearing age with unexplained s

Contralateral obturator nerve transfer for femoral nerve restoration: a case report.

BACKGROUND: Femoral nerve injury may occur in severe traffic accident injuries with pelvic fracture. Sural nerve grafts or ipsilateral obturator nerve transfer may be used to restore femoral nerve function. We report a new procedure transferring the contralateral obturator nerve to restore femoral nerve function. CASE DESCRIPTION: A 30 year-old male suffering complete lumbar plexus rapture received a contralateral obturator nerve transfer in our hospital. At 2 years follow up he had gained Medical Research Council Grade 3 muscle strength in his 23th months follow-up, with normal gait, Lower E

Modification of obturator to femoral nerve transfer for femoral nerve palsy.

Postoperative femoral nerve palsy with nerve division is a potentially devastating complication with a poor prognosis, especially when there is a delay in diagnosis. We present a novel 'belt and braces' approach for managing this unusual injury, using a modification of a nerve transfer, which has only been previously reported in the literature three times to our knowledge.
