A 69-year-old man presented to bowel cancer screening after testing for faecal occult blood. Initial colonoscopy had been limited by a sigmoid stricture and a barium enema had revealed a suspicious mass in the corresponding area. He was referred for repeat colonoscopy, which showed a hard structure crossing the mid sigmoid colon, with both ends impacted into the bowel wall. A CT scan revealed what appeared to be a small bone impacted in a diverticulum with evidence of recent inflammation and a sealed perforation. The patient had recently been managed conservatively for an episode of diverticul

BACKGROUND: Concerns of gastrointestinal (GI) bleeding after cardiac surgery are increasing with increased use of antiplatelets and anticoagulants. We investigated the roles of preoperative screening for fecal occult blood by fecal immunochemical test (FIT) widely used to detect GI bleeding and cancer.
METHODS: A retrospective review was done in 1,663 consecutive patients undergoing FIT before cardiac surgery between years 2012 and 2020. One or two rounds of FIT were performed 2 to 3 weeks before surgery, when antiplatelets and anticoagulants were not suspended yet.
RESULTS: Positive FIT (>

Screening with Fecal Occult Blood Test (FOBT) fails to adequately select patients who could have colorectal tumors and should be sent to colonoscopy. The latter investigation should be considered as the first intention examination in every person aged more than 50 years old (or, 40 years if there is a family history of elevated cancer risk). Colonoscopy will allow to identify the numerous patients who, in spite of significant disease, would have a negative FOBT.

The haemoccult-test is recommended above all for the early recognition of the colon carcinoma. The stool of 183 relatively unselected patients was examined for occult blood by means of this test in order to control its importance for clinical practice. In 37 patients (20%) the test was positive at least once. Apart from origins of haemorrhage in the lower gastrointestinal tract in the upper one haemorrhages were found in epistaxis, hiatal hernia, gastric and duodenal ulcers [7] and carcinomas of the stomach, the latter in an already advanced stages. In one female patient with carcinoma of the

Ferrous fumarate (Fersamal) and ferrous carbonate (Ferrodic) may produce false-positive results with the orthotolidine tablet tests (Occultest and Hematest) if administered to patients being investigated for alimentary bleeding. Misleading results are also likely if the benzidine test is employed as a filter-paper procedure. Ferrous sulphate, gluconate, succinate, iron and ammonium citrate, and other preparations of iron have no influence on the modern tablet tests for occult alimentary bleeding.

BACKGROUND: Current screening protocols for colorectal cancer depend primarily on fecal occult blood testing (FOBT). However, positive test results do not always indicate the presence of a colonic neoplasm.
METHODS: We reviewed the results of 100 consecutive bidirectional (upper and lower) endoscopic procedures performed to evaluate positive FOBT results. Patients were excluded if they presented with gross bleeding, a history of bowel lesions, or previous intestinal operations. There were 31 women and 69 men whose mean age was 51 years.
RESULTS: Major abnormalities were found on esophagogast

UNLABELLED: It has recently been suggested that in adults with coeliac disease, faecal blood loss may play a role in the development of iron deficiency. A group of 45 children diagnosed with coeliac disease during 1996 and 1997 were therefore prospectively evaluated for the presence of gluten in their diet, iron deficiency anaemia, and faecal occult blood. Sixty children admitted for elective surgery or asthma served as controls. Faecal occult blood was found in four iron deficient children on normal diet, of whom three were newly diagnosed. Occult blood loss disappeared in three of the four c

The literature on faecal occult blood (FOB) testing reveals a vast amount of research determining the efficacy of this test for the early detection of colorectal cancer. However, it offers no details of the information that patients require before undertaking it. The test detects the presence of blood in a patient's faeces that may not be visible on examination.

Fecal occult blood testing (FOBT) is currently Food and Drug Administration (FDA) approved only for colorectal cancer (CRC) screening. There is now widespread off-label use of FOBT in the hospital setting as a diagnostic test. Here we present a brief case and a more detailed review of the literature arguing against inpatient FOBT. Inpatient use of FOBT is problematic for several reasons including failure to account for false positives or negatives, delays in appropriate consultations or endoscopy, increased costs, increase length of stays, unnecessary procedures, and test results that do not c

GOALS: To evaluate gastroenterologists' use of esophagogastroduodenoscopy (EGD) for positive fecal occult blood test (FOBT).
BACKGROUND: Colonoscopy is recommended when an FOBT performed for colorectal cancer screening is positive. Guidelines suggest no further evaluation if anemia and gastrointestinal (GI) symptoms are absent.
METHODS: Online surveys included 4 vignettes: positive FOBT in average-risk adults 50 years of age or older with/without iron-deficiency anemia and with/without upper GI symptoms. For each scenario, respondents were asked if they would perform colonoscopy only, EGD on

AIM: To identify blood donors with occult hepatitis B virus (HBV) infection (OBI) to promote safe blood donation.
METHODS: Descriptive cross sectional study was conducted on 3167 blood donors negative for hepatitis B surface antigen (HBsAg), hepatitis C antibody (HCV Ab) and human immunodeficiency virus Ab. They were subjected to the detection of alanine aminotransferase (ALT) and aspartate transaminase (AST) and screening for anti-HBV core antibodies (total) by two different techniques; [Monoliza antibodies to hepatitis B core (Anti-HBc) Plus-Bio-Rad] and (ARC-HBc total-ABBOT). A man aged 81 known to suffer from atrial fibrillation - the treatment for which included acetylsalicylic acid - and chronic obstructive pulmonary disease, was hospitalized because of a respiratory infection. Since he had iron deficiency anaemia and the case history mentioned "intestinal bleeding', supplementary examinations were carried out. Endoscopy revealed colonic varices; because of the absence of portal hypertension and other disorders related to colonic varices, the varices were classified as idiopathic. In view of the extensiveness of the lesions, it was decided to refrain from endosc

BACKGROUND: Recent screening studies with fecal occult blood testing (FOBT) report that one of three patients with colorectal cancer (CRC) can be cured of the disease; minimal attention has been given to the two of three patients who despite repeated screening go on to die of silent CRC. We report the known "miss rate" (known false negatives) of our 14-year ongoing program of FOBT that was organized in 1979 to detect early CRC.
METHODS: From October 1979 through December 1993, 36,034 FOBT kits were distributed to patients who were without gastrointestinal complaints at Hines Veterans Affairs

The diagnostic approach to the asymptomatic patient who has tested positive for fecal occult blood varies among clinicians. Colonoscopic evaluation is the procedure of choice in initiating the workup of patients 50 years of age and older. In younger patients, examination by sigmoidoscopy and barium enema using air for contrast (double contrast study) may be considered as an alternative to colonoscopy as the initial test. Evaluation of the upper gastrointestinal tract should be considered if a colonic source is not detected. The challenge in diagnostic test selection lies in choosing the most a

BACKGROUND: An evaluation by the National Blood Center, the Thai Red Cross Society, of two commercial multiplex nucleic acid tests (NATs; the Chiron PROCLEIX ULTRIO test and the Roche Cobas TaqScreen MPX test) for screening Thai blood donors for hepatitis B virus (HBV), hepatitis C virus, and human immunodeficiency virus Type 1 identified 175 HBV NAT-reactive/hepatitis B surface antigen (HBsAg)-negative donors. The classification of the HBV infection of these donors was confirmed by follow-up testing.

OBJECTIVE: The purpose of this study was to test the feasibility of digital rectal fecal occult blood screening during pelvic examination.
STUDY DESIGN: We reviewed the data for 232 consecutive women who underwent digital rectal fecal occult blood screening during routine pelvic examination and who had had at least 1-year of follow-up visits: 59% of the women were followed for gynecologic cancer, and 41% of the women were followed for benign gynecologic disease. The median age was 62 years. Patients with positive digital rectal fecal occult blood screening were sent for gastroenterologic exam

BACKGROUND: Phantom limb pain (PLP) is a common complication after amputation, affecting up to 80% of the amputee population. However, only 5% to 10% of amputees have severe PLP impacting daily function. The present report details the management of severe, treatment-resistant PLP in a 72-year-old man with a traumatic left transradial amputation and a comorbid complication of heterotopic ossification (HO).
OBJECTIVE: To describe a case of PLP with HO and the possible role of calcitonin in the treatment of both conditions.
METHODS: A systematic review of the literature regarding the management

BACKGROUND: The fecal immunochemistry test (FIT) has been proposed as a surrogate marker of intestinal inflammation. Psoriasis is a chronic inflammatory skin disease that is linked to underlying systemic inflammatory conditions, including inflammatory bowel disease.
METHODS: We investigated the association between occult blood in feces and the risk of psoriasis using data from the National Health Insurance System. This study was conducted involving 1,395,147 individuals who underwent health examinations from January 2009 to December 2012 and were followed up until the end of 2017.
