[New views on aberrant innervation of oculomotor muscles ].

Aberrant innervations of oculomotor muscles are a special group of clinical entities, the common sign of which is an abnormal innervation of the oculomotor muscles with nerve fibres which originally were not meant for this purpose. The majority of these anomalies develops during the embryonic period of lesions at the level of nuclei of cranial nerves or peripheral fibres. Congenital aberrations affect most frequently n. VI, less frequently n. III and rarely n. IV. Acquired affections are most frequent in n. III. Abnormal innervation connections concern not only oculomotor muscles but also musc

Abnormalities of the oculomotor nerve in congenital fibrosis of the extraocular muscles and congenital oculomotor palsy.

PURPOSE: High-resolution magnetic resonance imaging (MRI) can now directly demonstrate innervation to extraocular muscles and quantify optic nerve size. A quantitative MRI technique was developed to study the oculomotor nerve (CN3) and applied to congenital fibrosis of extraocular muscles (CFEOM) and congenital oculomotor palsy. METHODS: The subarachnoid portions of the CN3s were imaged with a 1.5-T MRI scanner and conventional head coils, acquiring heavily T(2)-weighted oblique axial planes 1-mm thick and parallel to the optic chiasm. Thirteen normal subjects, 14 with CFEOM, and 3 with conge

[Measurements of ultrasound reflectivity of oculomotor muscles in endocrine orbitopathy].

BACKGROUND: In the course of Graves' disease (GD) the extraocular muscles become fibrotic after an acute inflammatory stage. Differentiation of these two stages is important for therapy. MATERIALS AND METHODS: First, we examined the eye muscles of 30 healthy volunteers with the 10 MHz-B probe (Ultrascan) Digital B 2000) and then those of 20 patients with GD. The thickness was measured in the longitudinal displayed muscle always with the same gain. The amplitude of the highest inner muscle spikes expressed as a percentage of amplitude of retina-sclera spike provided the internal reflectivity.

[Oculomotor muscles involvement revealing dermatomyositis in a patient with rheumatoid arthritis].

INTRODUCTION: Oculomotor muscles (OMM) involvement in dermatomyositis (DM) and in rheumatoid arthritis (RA) is unusual. The DM always leads to OMM inflammation, whereas the RA particularly leads to tenosynovitis of the superior oblique muscle referred to as the Brown syndrome. OBSERVATION: The patient is a 43-year-old woman who gives a 17-year-history of severe seropositive RA with bilateral coxite. She was hospitalized for acute painful proptosis. The clinical examination revealed an orbital erythema and a muscular rhizomelic weakness. The muscular enzymes were increased. The orbital CT reve

The oculomotor nucleus and extraocular muscles in a mutant anophthalmic mouse.

The object of this study was to determine whether the oculomotor nucleus and the extraocular muscles are present in the adult of a mutant mouse strain in which eye formation fails early in development (Chase and Chase, '41). Neuronal counts for the oculomotor (IIIrd) nucleus were obtained from serial paraffin sections made through the midbrains of seven mice of the anophthalmic mutant group (ZRDCT-An) and seven mice of the same strain in which eyes were present (ZRDCT-N). The orbital structures of the anophthalmic and control groups were reconstructed from 1-micrometer plastic serial sections.

Oculomotor nerve guidance and terminal branching requires interactions with differentiating extraocular muscles.

Muscle function is dependent on innervation by the correct motor nerves. Motor nerves are composed of motor axons which extend through peripheral tissues as a compact bundle, then diverge to create terminal nerve branches to specific muscle targets. As motor nerves approach their targets, they undergo a transition where the fasciculated nerve halts further growth then after a pause, the nerve later initiates branching to muscles. This transition point is potentially an intermediate target or guidepost to present specific cellular and molecular signals for navigation. Here we describe the navig

Oculomotor nerve and muscle abnormalities in congenital fibrosis of the extraocular muscles.

Congenital fibrosis of the extraocular muscles is an autosomal dominant congenital disorder characterized by bilateral ptosis, restrictive external ophthalmoplegia with the eyes partially or completely fixed in an infraducted (downward) and strabismic position, and markedly limited and aberrant residual eye movements. It has been generally thought that these clinical abnormalities result from myopathic fibrosis of the extraocular muscles. We describe the intracranial and orbital pathology of 1 and the muscle pathology of 2 other affected members of a family with chromosome 12-linked congenital

[Injection of botulinum toxin into the oculomotor muscles in disorders of ocular motility].

Injections of botulinum toxin into the oculomotor muscles was used in incorrectible diplopia, ocular torticollis, Duane's syndrome and congenital nystagmus. Favourable results were obtained and the sole complications which could be observed were a transitory ptosis and subconjunctival haemorrhages. Frequently 2 to 3 injections were sufficient for a permanent effect.

[Does proprioception of the extrinsic eye muscles participate in equilibrium, vision and oculomotor action?].

Extrinsic eye muscles respond to both proprioceptive and visual impulses. Histology of ocular proprioceptive receptors shows them to be qualitatively and original, neuromuscular bundles are found in primates, and so called "palisade sensory nerve endings" in felines. Golgi's osteotendinous receptors are apparently absent. The number of muscle receptors is high and their proportion in relation to muscle weight even more so. The afferent pathway for this sensory system initially accompanies the motor fibers of oculomotor III, IV, ans VI nerves. Some fibers continue with these nerves to the brain

Reinnervation of extraocular muscles by facial-to-oculomotor nerve anastomosis in rats: anatomic nuclear changes.

OBJECTIVE: Oculomotor nerve palsy greatly impairs the patient's daily life. After oculomotor nerve injury, when the central nerve stump is not available, neurotization of the distal nerve stump with a donor nerve may be performed. Here, we present an experimental anatomic study in rats related to the motor nuclear organization after facial-to-oculomotor nerve anastomosis. METHODS: In adult rats, the right oculomotor nerve was transected at the skull base. Then, the ipsilateral facial nerve was exposed at the stylomastoid foramen and connected side-to-end to one extremity of a peroneal nerve a

[Effects of external orbital radiotherapy on the oculomotor muscle in Graves disease].

PURPOSE: Orbital radiotherapy is recognised to be effective in the treatment of acute Graves' disease. The effect on the oculomotor muscles is still controversial. MATERIAL AND METHODS: To assess this effect, we conducted a prospective study on 15 patients with acute Graves' ophthalmopathy. Patients were investigated before radiotherapy, 3 months later and at long term (mean = 2 years). To study the extent of the ophthalmopathy we chose the following criteria: class IV in the NOSPECS classification system, amplitude of gaze, measurement of the thickness of oculomotor muscles (CT-scan). Muscle coenzyme Q10 deficiencies in ataxia with oculomotor apraxia 1.

APTX gene mutations responsible for ataxia-oculomotor apraxia 1 (AOA1) were identified in a family previously reported with ataxia and coenzyme Q10 (CoQ10) deficiency. We measured muscle CoQ10 levels in six patients with AOA1 and found decreased levels in five. Patients homozygous for the W279X mutation had lower values (p = 0.003). A therapeutic trial of CoQ10 may be warranted in patients with AOA1.

Neuroanatomical Structures in Human Extraocular Muscles and Their Potential Implication in the Development of Oculomotor Disorders.

The potential role of sensory feedback from human extraocular muscles has been subjected to considerable speculation in the ophthalmic literature. Extraocular muscles pull against a fairly even load and do not initiate a stretch reflex, even when the eyes are directed toward the boundaries of their respective field of action. These unique working conditions and physiological properties have led to the notion that the sensory signal arising from receptors in extraocular muscles differs from the conventional proprioceptive signal arising from their somatic counterparts. The interest in the recep

Magnetic Resonance Imaging of Extra Ocular Muscles and Oculomotor Nerves in Patients with Congenital Monocular Elevation Deficit.

BACKGROUND: There is sparse literature on radiological findings in cases of monocular elevation deficit (MED). We conducted this study to report magnetic resonance imaging (MRI) characteristics of extra-ocular muscles and oculomotor nerves in patients with congenital MED and to investigate its patho-mechanisms. METHODS: We included patients with congenital MED without prior strabismus surgery. The cross-sectional area (CSA) of the extra-ocular muscles in quasi-sagittal and quasi-coronal scans was measured manually on 2-mm-thick T2-weighted MRI images using Osirix MD software. [A 54-year-old man with generalized muscle atrophy and oculomotor paresis].

We report a 54-year-old man with progressive generalized muscle atrophy and ophthalmoparesis in the terminal stage. He was well until 44 years of age (1982) when he noted weakness in his right hand and muscle atrophy; in May of 1985, he noted weakness in his left hand and in both legs. His weakness had become progressively worse, and he became unable to walk in November of 1985. He noted dysarthria one month later, and dysphagia in March of 1986. His difficulty in swallowing had also become worse; he regurgitated foods into the trachea in September of that year, and he developed a low grade fe
