Ofloxacin is a newly licensed fluoroquinolone with an antimicrobial spectrum similar to ciprofloxacin. Compared with ciprofloxacin, the MIC90 values for ofloxacin are lower for S aureus, C trachomatis, and Ureaplasma urealyticum, but somewhat higher against gram-negative bacteria (especially P aeruginosa). Ofloxacin has favorable pharmacokinetics with almost 100% bioavailability; peak serum concentrations obtained one to two hours following oral dosing are higher than those achieved with ciprofloxacin. Ofloxacin is a new fluorinated quinolone antibiotic with a broad spectrum of activity against a variety of gram-positive and -negative bacteria including Enterobacteriaceae, Pseudomonas aeruginosa, and methicillin-resistant Staphylococcus aureus. Flouroquinolones are widely used antibiotics with side effects that involve the central nervous system. Here, we are presenting the case of a patient who developed hypomania after taking ofloxacin. The clinical features included an elevated mood, increased talkativeness, a decreased need for sleep, and drawing up big plans. The episode was circumscribed and resolved after the discontinuation of the antibiotic.

The safety profile of ofloxacin was evaluated on the basis of adverse reactions and abnormal laboratory values seen in United States clinical trials and phase I studies addressing specific issues. In the present study, 33 adult patients with salmonella infections consisting of 13 typhoid fever, 9 paratyphoid fever, 8 enterocolitis, 2 carriers, 1 acute gastroenteritis have been treated with ofloxacin. The concentration of ofloxacin in prostatic tissue and serum was determined in order to evaluate the permeability of ofloxacin into prostatic tissue in patients with benign prostatic hypertrophy. Ofloxacin was administered orally at a dose of 200 mg several hours before subcapsular prostatectomy. The determination was performed in the surgically removed adenoma and in the serum taken one hour before and at the removal of the adenoma. The peak level in the prostatic tissue was 1.46 microgram/g at 4.5 hour after the administration. The ratio of prostatic tissue level to the serum level was 1.00.

The prostatic tissue levels of ofloxacin were determined in 20 patients undergoing transurethral resection of the prostate.
