[Ophthalmic manifestations of facial nerve palsy].

Facial nerve palsy is a common pathology that develops due to various diseases, traumas and medical interventions. The most common peripheral type of facial paralysis leads to acute unilateral involvement of facial muscles along with the development of paralytic lagophthalmos. The absence of adequate and timely medical treatment in patients with paralytic lagophthalmos can lead to severe corneal complications such as cornea perforation causing loss of eye and consequently restricted professional ability, work capacity and significant decrease of patient's life quality. Patients with paralytic

Ophthalmic management of facial nerve palsy.

The ophthalmologist plays a pivotal role in the evaluation and rehabilitation of patients with facial nerve palsy. It is crucial to recognize and treat the potentially life-threatening underlying causes. The immediate ophthalmic priority is to ensure adequate corneal protection. The medium to long-term management consists of treatment of epiphora, hyperkinetic disorders secondary to aberrant regeneration and poor cosmesis. Patients should be appropriately referred for general facial re-animation. This review aims to provide a guide to the management of this complex condition.

Orbital neuromuscular choristoma of the ophthalmic nerve.

INTRODUCTION: Neuromuscular choristoma (NC) is a rare tumor composed of striated muscle fibers admixed with nerve fibers that often affects large peripheral nerve trunks in children. To the best of our knowledge, this is the first reported case in the literature of a NC of the ophthalmic nerve in an adult. CASE DESCRIPTION: A 27-year-old woman presented with a 20-day history of left periorbital headache radiating to the frontotemporal region, with associated tearing, nausea, and vomiting. The examination was normal, except for hypoesthesia in the distribution of the first trigeminal nerve bra

Hypertonic saline test for ophthalmic nerve impairment.

In order to compare the accuracy of methods for testing ocular surface sensitivity (e.g. function of the first branch of the trigeminal nerve) three different methods were compared in patients with unilateral acoustic neurinomas. The three methods 1) hypertonic (3%) saline in the conjunctival sac (apparently not previously described in the literature) 2) esthesiometer (Cochet & Bonnet) touching of the cornea, and 3) touching the cornea with a cotton wool wisp, was found to reveal reduced ocular surface sensitivity on the neurinoma side in 50% (hypertonic saline), 23% (esthesiometer) and 14% (c

Ophthalmic management of facial nerve palsy: a review.

Facial nerve palsy affects individuals of all ages, races, and sexes. Psychological and functional implications of the paralysis present a devastating management problem to those afflicted, as well as the carriers. Since Sir Charles Bell's original description of facial palsy in 1821, our understanding and treatment options have expanded. It is essential that a multidisciplinary approach, encompassing ophthalmologists; Ear, Nose, and Throat surgeons; plastic surgeons; and psychologists work closely to optimize patient management in a staged approach. Although the etiology remains unknown, stro

Ophthalmic and periocular management of facial nerve palsy.

Facial nerve palsy (FNP) can be a highly distressing condition with sight-threatening sequelae. However, major advances in ophthalmic and oculoplastic management have been achieved in recent decades, guided by a better understanding of clinical manifestations and an ever-expanding armamentarium of surgical and nonsurgical therapies. Ophthalmologists should be aware of the contemporary approach to FNP, its periorbital complications, and the broad range of treatment options now available.

The trigeminal nerve. Part II: the ophthalmic division.

The ophthalmic, or first division (V1) of the trigeminal nerve, is the smallest of the three divisions and is purely sensory or afferent in function. It supplies sensory branches to the ciliary body, the cornea, and the iris; to the lacrimal gland and conjunctiva; to portions of the mucous membrane of the nasal cavity, sphenoidal sinus, and frontal sinus; to the skin of the eyebrow, eyelids, forehead, and nose; and to the tentorium cerebelli, dura mater, and the posterior area of the falx cerebri. At first glance, one might not expect one interested in the diagnosis and treatment of orofacial

South American blastomycosis: ophthalmic and oculomotor nerve lesions.

A case of South American blastomycosis began with an oropharyngeal lesion which was followed by a granulomatous uveitis. The patient was treated with Amphotericin B and showed a clinical regression. Four months later, he developed a right 3rd cranial nerve palsy, aggravating the clinical aspect with a severe generalized involvement of the central nervous system and death. Necropsy showed blastomycotic meningoencephalitis.

Neuro-ophthalmic complications following eighth cranial nerve microvascularx decompression.

PURPOSE: To discuss a novel diagnosis of post-surgical iatrogenic AICA vasospasm following eighth cranial nerve microvascular decompression as the most likely etiology for a lateral pontine infarct. OBSERVATIONS: A 49-year-old male presented with tinnitus, vertigo, binocular and monocular diplopia following left occipital decompressive craniectomy and microvascular decompression of cranial nerve VIII. Magnetic resonance imaging demonstrated patent vasculature with evolution of a subacute infarct of the left lateral pons and middle cerebellar peduncle in the territory of the left AICA. CONCLU

Patients' perspectives of ophthalmic management of facial nerve paralysis.

OBJECTIVE: Facial nerve paralysis (FNP) affects eye muscle function causing lagophthalmos. This leaves the cornea vulnerable to irritation and injury which, if untreated, may cause permanent vision impairment. Decreased eye muscle control impacts facial expressions and quality of life. Although non-surgical interventions such as lubrication are routinely used in standard eyecare, patient perspectives regarding these interventions have not been extensively investigated. This study aims to investigate patients' perspectives of conservative ophthalmic management for FNP. METHODS AND ANALYSIS: Pa

Optic nerve penetration by a carotico-ophthalmic artery aneurysm.

Fenestration of a cranial nerve by an internal carotid artery aneurysm is rarely identified at surgical repair of intracranial aneurysms. Classically this condition is associated with visual disturbance in either acuity, fields or diplopia. We describe a patient with optic nerve fenestration by a carotico-ophthalmic artery aneurysm and discuss its likely pathophysiology. It is important to consider this anatomical variation during microsurgical approaches to the pericarotid and perioptic cisterns to prevent inadvertent damage to the optic apparatus.

Atypical Optic Nerve Sheath Meningioma With Postbiopsy Ophthalmic Artery Occlusion.

The authors present the case of a 44-year-old female with progressive optic neuropathy secondary to an atypical posterior intraconal tumor who underwent incisional biopsy complicated by severe postoperative vision loss secondary to ophthalmic artery occlusion. Histology showed a fibroblastic meningioma WHO grade 1. Here, the authors discuss the clinical and radiologic findings, in particular, the exophytic "globular" configuration of the tumor, and perform a review of the literature.

Ophthalmic profile and systemic features of pediatric facial nerve palsy.

BACKGROUND: Facial nerve palsy (FNP) occurs less frequently in children as compared to adults but most cases are secondary to an identifiable cause. These children may have a variety of ocular and systemic features associated with the palsy and need detailed ophthalmic and systemic evaluation. METHODS: This was a retrospective chart review of all the cases of FNP below the age of 16 years, presenting to a tertiary ophthalmic hospital over the period of 9 years, from January 2000 to December 2008. RESULTS: A total of 22 patients were included in the study. The average age at presentation was

Blockade of branches of the ophthalmic nerve in the management of acute attack of migraine.

The aim of this study was the evaluation of the blockade of branches of ophthalmic nerve in the management of the acute attack of migraine headache localized to the ocular region. The study included 70 female patients 23-60 years old who presented to the pain clinic at our hospital with an acute attack of migraine headache localized to the ocular and retro-ocular region. A targeted history and a neurologic examination were performed in all patients to confirm the diagnosis and at the same time to rule out life-threatening neurological dysfunction. The method applied was the blockade of the sup
