Diode Laser-Assisted Oral Frenectomy as a Modern Alternative: A Systematic Review and Meta-Analysis.

BACKGROUND: Clinicians frequently use diode laser-assisted oral frenectomy to correct anatomical restrictions in the oral cavity as a minimally invasive alternative. However, evidence for its effect on wound healing and on reducing common postoperative complications remains unclear. This systematic review and meta-analysis aimed to evaluate the clinical efficacy of diode laser-assisted frenectomy compared with the conventional scalpel technique. METHODS: This review adheres to the PRISMA statement, and the protocol is registered in PROSPERO (CRD420251050170). A comprehensive electronic and ma

Impact of frenectomy on the oral exercise in patients with ankyloglossia and obstructive sleep apnea: double-blind randomized controlled clinical trials.

OBJECTIVE: This study compares the efficacy of oral exercise alone to oral exercise with frenectomy in improving obstructive sleep apnea (OSA) symptoms and quality of life (QOL) in patients with ankyloglossia. MATERIALS AND METHODS: A prospective, controlled, double-blind clinical study enrolled fifteen adults (20-60 years) newly diagnosed with mild to moderate OSA and ankyloglossia. Participants were randomly assigned to either oral exercise alone (control group; n = 8) or oral exercise with frenectomy (intervention group; n = 7). Outcomes were assessed after a 3-month therapy period using p

Significant Bleeding Event Secondary To Maxillary Labial Frenectomy.

Maxillary frenectomies are invasive surgical procedures that are increasingly being performed in dental clinics to address functional and esthetic concerns. The purpose of this case report is to describe a 16-month-old boy who presented with oral bleeding lasting over 12 hours following a laser maxillary labial frenectomy, which eventually led to a diagnosis of severe hemophilia A. This case highlights the importance of thorough patient assessment, including the need to rule out systemic conditions, such as coagulopathies, prior to frenectomies. This report also raises concerns about the numbe

Does the maxillary midline diastema close after frenectomy?

OBJECTIVE: To analyze the closure, persistence or reopening of the maxillary midline diastema after frenectomy in patients with and without subsequent orthodontic treatment. METHOD AND MATERIALS: All patients undergoing frenectomy with a CO2 laser were included in this retrospective study during the period of September 2002 to June 2011. Age and sex, the dimension of the diastema, eruption status of the maxillary canines, and the presence of an orthodontic treatment were recorded at the day of frenectomy and during follow-up. RESULTS: Of the 59 patients fulfilling the inclusion criteria, 31

Lingual frenulum: changes after frenectomy.

PURPOSE: To describe the changes after frenectomy concerning mobility and functions of the tongue. METHODS: Participants were 53 subjects who had never undergone speech therapy or lingual frenulum surgery. A specific lingual frenulum protocol with scores was used by speech-language pathologists when there was evidence of frenulum alteration. Ten subjects had abnormal frenulum and were referred to an otolaryngologist for frenectomy. After surgery, the subjects were re-evaluated using the same protocol. Photos and videos were taken for comparison. RESULTS: Thirty days after surgery, the subjec

Refining Frenectomy by Miller's Technique: A Case-Based Overview.

An aberrant labial frenum can lead to midline diastema and gingival recession, affecting both the aesthetic appearance and functional integrity of the oral cavity. Several treatment approaches are available for managing such frenum abnormalities. The traditional frenectomy method is a more invasive procedure that often results in visible scarring and the loss of the interdental papilla, compromising aesthetics. To address these limitations, Miller proposed a modified technique that combines frenectomy with a laterally displaced flap. This approach offers the advantage of healing by primary int

Twelve-Year Follow-Up of Laser Frenectomy during Early Mixed Dentition.

Median maxillary labial frenum (MMLF) is one of the oral freni, found on the inner side of the centre of the upper lip. Maxillary midline diastema (MMD) is commonly associated with highly attached frenum, and frenectomy during early mixed dentition is controversial. A 6-year-old boy came with a chief complaint of unpleasant spacing between the two upper front teeth. A median maxillary high attached labial frenum with midline spacing of 5 mm was diagnosed. A consensus was made with the child's father to remove the frenum by laser. Diode laser frenectomy was done, and subsequent follow-up for 12

Etiology and clinical recommendations to manage the complications following lingual frenectomy: A critical review.

Ankyloglossia, also known as tongue-tie, is an embryological anatomical malformation of the tongue, characterized by an abnormally short and a thick lingual frenum. Tongue-tie restricts the physiologic movements of the tongue and results in various functional, behavioral and speech abnormalities along with the development of frontal and lateral lisps. Ankyloglossia in infants is also linked with the difficulty in breastfeeding difficulty, gagging, choking or vomiting food, delayed development or deterioration of speech and behavioral issues. A lingual frenectomy is a common oral surgical proce

Labial Frenectomy Using Conventional Scalpel Technique: A Case Report.

Frenum morphology is of pivotal concern while treating patients who desire improved aesthetics and for treating patients with midline diastemas, as it may lead to failure or relapse of orthodontic treatment. Frenum, a thick band of muscle fiber, may present with abnormalities in the shape, size, form, number, and thickness, which may cause gingival recession along with poor oral health. Frenectomy is adopted as the plan of action while treating abnormal frenum cases by resecting the frenum attachment to provide closure of the spacing between the upper anterior teeth, as in the case of midline

Labial Frenectomy using Laser: A Scoping Review.

Labial frenectomy is a surgical technique, that aims to remove the frenulum with its attachment to the underlying bone. Frenectomy, is indicated if the frenulum attachment causes midline diastema, gingival recession, hindrance in maintaining oral hygiene, or if it interferes with lip movements and for prosthetic needs. A labial frenectomy can be performed either by the routine scalpel technique, electrocautery, and most recently medical lasers. The aim of this study was to evaluate, whether the laser technique is more effective than the conventional surgical technique, and whether there are di

Modified frenectomy: a review of 3 cases with concerns for esthetics.

The maxillary labial frenum is a normal anatomical structure in the oral cavity. An abnormal labial frenum causes localized gingival recession and midline diastema, both of which can interfere with oral hygiene procedures, and eventually affect esthetics. When the frenum maintains its high papillary attachment, frenectomy is the treatment of choice. Though this technique has undergone many modifications, the zone of attachment and esthetics in the anterior maxillary region have been neglected. This article highlights a new frenectomy technique that results in good esthetics, excellent color ma

Association between superior labial frenum and maxillary midline diastema - a systematic review.

BACKGROUND: Pediatric otolaryngologists have seen an increased focus on upper lip frenum as a possible culprit for feeding difficulties and the development of maxillary midline diastema (MMD). This increase may be encouraged by parents' exposure to medical advice over the internet about breastfeeding and potential long-term aesthetic concerns for their children. Subsequently, there has been increased pressure on pediatric otolaryngologists to perform superior labial frenectomies. There has been a reported 10-fold increase in frenectomies since the year 2000. However, there is no consensus with

Laser-Assisted Surgical Management of Ankyloglossia in Mixed Age Groups: A Case Series With Eight-Month Follow-Up.

Ankyloglossia restricts tongue mobility and may impair speech, oral hygiene, and quality of life. Lasers offer advantages over scalpel techniques, including hemostasis and improved patient comfort. This report presents three cases of Kotlow Class II and III ankyloglossia treated with diode laser frenectomy across different age groups, with an eight-month follow-up. A 12-year-old boy (Class III), a 20-year-old female (Class III), and a 24-year-old male (Class II) underwent diode laser frenectomy. Postoperative care included analgesics and tongue exercises, and functional assessment was carried

Postoperative Tongue Exercises for Ankyloglossia Following Lingual Frenectomy: A Case Report.

Ankyloglossia, also named tongue-tie, is an innate developmental anomaly where the lingual frenum appears anchored to the sublingual space. Even though it is not considered a major congenital condition, it can create a variety of problems, such as difficulty in feeding newborns, difficulty in mastication, speech problems, poor oral hygiene, malocclusion, and hindrance in social interaction because of restricted tongue movements. Lingual frenectomy is the treatment of choice. Postoperative tongue exercises should be considered a valuable addition after lingual frenectomy, providing the patient

Clipping the (tongue) tie.

Ankyloglossia is an uncommon congenital oral anomaly that can cause difficulty with breast-feeding and speech articulation. For many years, the subject of ankyloglossia has been controversial with practitioners of many specialties having widely different views regarding its significance and management. This study is about a series of five cases of ankyloglossia subjected to surgical correction by frenectomy procedure. Three cases were treated with electrocautery, one case with diode laser and one case with conventional scalpel technique. 1 week and 1 month post-operative follow-ups were done a
