Organs at risk radiation dose constraints: 2025 update.

We present the updated recommendations from the Société française de radiothérapie oncologique (SFRO, the French society for radiation oncology) regarding radiation dose constraints for organs at risk in adult patients, with the aim of supporting clinical decision-making in contemporary radiotherapy practice. These guidelines address a wide range of clinical scenarios, including normofractionation, hypofractionation, stereotactic radiotherapy, and reirradiation. Dose constraints are essential to ensure both the safety and efficacy of radiotherapy, especially with the growing use of advanced te

Male reproductive organs are at risk from environmental hazards.

Male reproductive disorders that are of interest from an environmental point of view include sexual dysfunction, infertility, cryptorchidism, hypospadias and testicular cancer. Several reports suggest declining sperm counts and increase of these reproductive disorders in some areas during some time periods past 50 years. Except for testicular cancer this evidence is circumstantial and needs cautious interpretation. However, the male germ line is one of the most sensitive tissues to the damaging effects of ionizing radiation, radiant heat and a number of known toxicants. So far occupational haz

Radiation exposure of extracranial organs at risk during stereotactic linac radiosurgery.

BACKGROUND AND PURPOSE: Stereotactic radiosurgery of intracranial tumors and vascular malformations can be performed by either a linear accelerator (Linac) or gamma knife. The aim of this first study on patients was to determine the radiation exposure of organs at risk and assess the risk for late effects including secondary tumors and/or hereditary disorders after stereotactic Linac radiosurgery. MATERIAL AND METHODS: Thermoluminescent dosimetry (TLD) measurements were done on 21 consecutively admitted patients with various intracranial lesions scheduled for Linac radiosurgery. The TLD chips

4D CT analysis of organs at risk (OARs) in stereotactic radiotherapy.

Internal organs at risk volumes (IRV) represent the propagation of organs at risk (OARs) in 4DCT. Sixty consecutive patients that underwent 4DCT for thoracic stereotactic radiotherapy were analyzed and IRVs for heart, trachea, esophagus, bronchial tree, great vessels, and spinal cord were calculated. IRVs were then tested for the respect of dose constraints. IRVs were significantly bigger than standard OARs (p-value <0.001 for all the IRVs). IRVs that did not respect the dose constraints were, respectively, 7/60 (11.7%) for Heart IRV, 6/60 (10%) for Esophagus IRV, 11/60 (18.3%) for Trachea IRV

[Doses to organs at risk for conformational and stereotactic radiotherapy: Bladder].

Bladder dose constraints in case of conformational radiotherapy/intensity-modulated radiotherapy and stereotactic radiotherapy are reported from the literature, in particular from the French radiotherapy society RECORAD recommendations, according to the treated pelvic tumor sites. The dose-volume effect on urinary toxicity is not clearly demonstrated, making difficult to establish absolute dose constraints for the bladder. In case of high-dose prostate cancer radiotherapy, the bladder dose constraints are: V60Gy<50% and maximum dose<80Gy for standard fractionation and V60Gy<5%, V48Gy<25% and V

Toxicity risk of non-target organs at risk receiving low-dose radiation: case report.

The spine is the most common site for bone metastases. Radiation therapy is a common treatment for palliation of pain and for prevention or treatment of spinal cord compression. Helical tomotherapy (HT), a new image-guided intensity modulated radiotherapy (IMRT), delivers highly conformal dose distributions and provides an impressive ability to spare adjacent organs at risk, thus increasing the local control of spinal column metastases and decreasing the potential risk of critical organs under treatment. However, there are a lot of non-target organs at risk (OARs) occupied by low dose with und

Impact of cervicothoracic region stereotactic spine radiosurgery on adjacent organs at risk.

OBJECTIVE Stereotactic radiosurgery (SRS) of the spine is a conformal method of delivering a high radiation dose to a target in a single or few (usually ≤ 5) fractions with a sharp fall-off outside the target volume. Although efforts have been focused on evaluating spinal cord tolerance when treating spinal column metastases, no study has formally evaluated toxicity to the surrounding organs at risk (OAR), such as the brachial plexus or the oropharynx, when performing SRS in the cervicothoracic region. The aim of this study was to evaluate the radiation dosimetry and the acute and delayed toxi

Anatomically consistent CNN-based segmentation of organs-at-risk in cranial radiotherapy.

Planning of radiotherapy involves accurate segmentation of a large number of organs at risk (OAR), i.e., organs for which irradiation doses should be minimized to avoid important side effects of the therapy. We propose a deep learning method for segmentation of OAR inside the head, from magnetic resonance images (MRIs). Our system performs segmentation of eight structures: eye, lens, optic nerve, optic chiasm, pituitary gland, hippocampus, brainstem, and brain. We propose an efficient algorithm to train neural networks for an end-to-end segmentation of multiple and nonexclusive classes, addres

Volumetric change of selected organs at risk during IMRT for oropharyngeal cancer.

PURPOSE: To assess volumetric changes of selected organs at risk (OAR) during intensity-modulated radiotherapy (IMRT) for oropharyngeal carcinoma. MATERIALS AND METHODS: Twenty-six consecutive patients that were treated with definitive IMRT ± chemotherapy between November 2007 and November 2008 were selected for the present study. As part of an internal quality assurances program, a repeat kilovolt (KV) computed tomography was planned weekly during the 7-week treatment course. On each available scan, a single observer contoured the parotid submandibular, and thyroid glands (PG/SMG/TG), larynx

Geometrically focused training and evaluation of organs-at-risk segmentation via deep learning.

BACKGROUND: Deep learning methods are promising in automating segmentation of organs at risk (OARs) in radiotherapy. However, the lack of a geometric indicator for dosimetry accuracy remains to be a problem. This issue is particularly pronounced in specific radiotherapy treatments where only the proximity of structures to the radiotherapy target affects the dose planning. In cervical cancer high dose-rate (HDR) brachytherapy, treatment planning is motivated by limiting dose to the hottest 2 cubic centimeters (D2cm3) of the OARs. Similarly, Ethos online adaptive radiotherapy system prioritizes

Deep learning-based delineation of whole-body organs at risk empowering adaptive radiotherapy.

BACKGROUND: Accurate delineation of organs at risk (OARs) is crucial for precision radiotherapy. Most previous autosegmentation models were only constructed for single anatomical region without evaluation of dosimetric impact. We aimed to validate the clinical practicability of deep-learning (DL) models for autosegmentation of whole-body OARs with respect to delineation accuracy, clinical acceptance and dosimetric impact. METHODS: OARs in various anatomical regions, including the head and neck, thorax, abdomen, and pelvis, were automatedly delineated by DL models (DLD) and compared to manual

[Limits of dose constraint definition for organs at risk specific to stereotactic radiotherapy].

Stereotactic radiotherapy is a very hypofractionated radiotherapy (>7.5Gy per fraction), and therefore is more likely to induce late toxicities than conventional normofractionated irradiations. The present study examines four frequent and potentially serious late toxicities: brain radionecrosis, radiation pneumonitis, radiation myelitis, and radiation-induced pelvic toxicities. The critical review focuses on the toxicity scales, the definition of the dose constrained volume, the dosimetric parameters, and the non-dosimetric risk factors. The most commonly used toxicity scales remain: RTOG/EORT

Neurocognition and mean radiotherapy dose to vulnerable brain structures: new organs at risk?

BACKGROUND: Children with brain tumors are at high risk of neurocognitive decline after radiotherapy (RT). However, there is a lack of studies on how RT doses to organs at risk (OARs) impacts neurocognition. The aim of this study was to examine dose-risk relationships for mean RT dose to different brain structures important for neurocognitive networks. We explored previously established OARs and potentially new OARs. METHODS: A sample of 44 pediatric brain tumor survivors who had received proton and/or photon RT were included. Correlations between mean RT doses to OARs and IQ were analyzed. P

Assessment of megavoltage imaging dose to organs at risk in breast cancer radiotherapy.

INTRODUCTION: Advanced radiotherapy require frequent imaging to ensure accurate target coverage. However, imaging verification doses are often unaccounted for, raising questions regarding normal tissue exposure. Daily orthogonal (anterior = 1MU and lateral = 1MU) megavoltage (MV) portal images over 30 fractions may provide 40-400 mSv of dose, according to limited data. Additionally, between 17 % and 19 % of cancer survivors who have undergone radiotherapy, 5 % may acquire secondary malignancies. Daily MV imaging is common during breast cancer radiotherapy at the study site as a departmental pr

Defining and sparing sexual function-related organs at risk for rectal cancer radiotherapy.

BACKGROUND AND PURPOSE: Sexual dysfunction is a common consequence of pelvic radiotherapy, influenced by psychological, physical, social, and relational factors. Research has focused on vaginal dose and stenosis in females and penile bulb dose and erectile dysfunction in males, with limited attention to domains, such as arousal, desire, and satisfaction. In the Danish Colorectal Cancer Radiotherapy Group, we aimed to: (1) Develop an atlas of sexual function-related organs at risk and (2) Evaluate if these organs at risk could be spared without compromising target coverage in rectal cancer radi

Target coverage and organs at risk dose in hypofractionated salvage radiotherapy after prostatectomy.

BACKGROUND AND PURPOSE: Introducing moderately hypofractionated salvage radiotherapy (SRT) following prostatectomy obligates investigation of its effects on clinical target volume (CTV) coverage and organ-at-risk (OAR) doses. This study assessed interfractional volume and dose changes in OARs and CTV in moderately hypofractionated SRT and evaluated the 8-mm planning target volume (PTV) margin. MATERIALS AND METHODS: Twenty patients from the PERYTON-trial were included; 10 received conventional SRT (35 × 2 Gy) and 10 hypofractionated SRT (20 × 3 Gy).
