Orthodontic-periodontal interactions: Orthodontic extrusion in interdisciplinary regenerative treatments.

Orthodontics is a periodontal treatment. "Guided orthodontic regeneration" (GOR) procedures use orthodontic movements in perio-restorative patients. The GOR technique includes a guided orthodontic "soft tissue" regeneration (GOTR) and a guided orthodontic "bone" regeneration (GOBR) with a plastic soft tissue approach and a regenerating reality. The increased amount of soft tissue gained with orthodontic movement can be used for subsequent periodontal regenerative techniques. The increased amount of bone can as well improve primary implant stability and, eventually, simplify a GTR technique to

Surgical extrusion of a maxillary premolar after orthodontic extrusion: a retrospective study.

OBJECTIVES: Crown-root fracture and cervical caries in maxillary premolars constitute a challenge in cases of subgingival placement of restoration margins. Surgical extrusion has been practiced successfully in permanent anterior teeth. The aim of the present retrospective study was to assess the clinical outcome of surgical extrusion after orthodontic extrusion in maxillary premolars. MATERIALS AND METHODS: Twenty-one single, tapered root maxillary premolars with subgingival crown-root fracture or caries were included. Presurgical orthodontic extrusion was performed on all teeth to prevent ro

The Impact of Orthodontic Extrusion on Keratinized Gingiva.

Background and Objectives: The key factor that enables osteoblastic activity and the formation of new bone, as well as gingiva, during orthodontic tooth extrusion (OE) is the periodontal ligament. The reaction of periodontal tissues associated with changes in the gingiva is a part of orthodontic tooth displacement. The aim of this study was to examine the effect of OE on the width of the zone of the keratinized and attached gingiva, the position of the mucogingival junction, and the height of the interdental papillae in the region where the OE was performed as well as in the adjacent region. Orthodontic extrusion: its use in restorative dentistry.

The technique described in this study demonstrates that the use of orthodontic extrusion can provide an alternative to extraction and extensive periodontal surgery. Selection of patients is an important aspect when considering this type of treatment. The crown-root ratio, as well as the root anatomy, if inadequate, may render this type of treatment inappropriate. The concept of a biologic width is discussed, and its application to forced eruption is described.

Raising the root. A look at orthodontic extrusion.

A simple technique using bonded wires to establish anchorage and an attachment to a tooth requires a minimum of special materials or advanced skills in orthodontics. The bonded wire stabilizes the anchor unit and avoids any adverse tooth movement.

Orthodontic extrusion of the lower third molar with an orthodontic mini implant.

Neurologic changes owing to damage to the inferior alveolar nerve (IAN) are the most serious complication of lower third molar (M3) extraction because of their close spatial relationship. We adopted the concept of regional orthodontic treatment and extrusion, using skeletal anchorage with an orthodontic mini implant. Two malformed M3s that were closely apposed to the IAN were extruded with the aid of 3 or 4 orthodontic brackets and a mini implant. Both of the M3s were extruded successfully. The patients experienced little discomfort with the orthodontic appliances and there was neither permane

Orthodontic Extrusion: Guidelines for Contemporary Clinical Practice.

Orthodontic extrusion (OE) is an orthodontic tooth movement in a coronal direction to modify the tooth position and/or induce changes on the surrounding bone and soft tissue with a therapeutic purpose. Evidence emanating from clinical reports and case series studies indicates that OE is a predictable treatment option to manage a variety of clinical situations. Common indications include traction of impacted teeth, exposure of teeth presenting structural damage to facilitate restorative therapy, treatment of periodontal bony and papillary defects, and implant site development. Unfortunately, th

Orthodontic Extrusion vs. Surgical Extrusion to Rehabilitate Severely Damaged Teeth: A Literature Review.

The need to rehabilitate severely compromised teeth is frequent in daily clinical practice. Tooth extraction and replacement with dental implant represents a common treatment choice. However, the survival rate for implants is inferior to teeth, even if severely damaged but properly treated. In order to reestablish a physiological supracrestal tissue attachment of damaged teeth and to arrange an efficient ferrule effect, three options can be considered: crown lengthening, orthodontic extrusion and surgical extrusion. Crown lengthening is considered an invasive technique that causes the removal

[Crown lengthening by orthodontic extrusion. Principles and technics].

Forced Eruption by orthodontic means is an effective way of managing subgingival caries and root fractures at the level of the osseous crest or below. It allows the dentist to construct restorations on otherwise hopeless teeth and maintains the biologic distance between the apical portion of the junctional epithelium and the alveolar crest.

Experimental surgical and orthodontic extrusion of teeth in dogs.

The purpose of this study was to evaluate histologically the root surfaces of teeth submitted to orthodontic and surgical extrusion procedures in a dog model. Eighteen adult male dogs, divided into six groups of three dogs each, were used in the study. Each animal underwent two procedures: rapid orthodontic extrusion and surgical extrusion of the maxillary lateral incisors. The animals were sacrificed to produce samples at 7, 14, 45, 90, 120, and 180 days after surgery for assessment of cross sections of the coronal, medial, and apical thirds of the treated teeth. At early time points, some ac

The effect of orthodontic extrusion on traumatically intruded teeth.

The management of traumatically intruded permanent incisors is controversial. Some authors suggest a decreased incidence of ankylosis in cases treated with orthodontic extrusion. The purpose of this study was to examine two common management techniques for traumatic intrusion, orthodontic extrusion, and observation for re-eruption. The four first premolars of three shepherd dogs were traumatically intruded with a mallet while a holding device was used to prevent tooth fracture. Five to 7 days following the injury, orthodontic force was applied unilaterally while the contralateral tooth served

Orthodontic extrusion in the transitional dentition: a simple technique.

Extrusion of teeth may be necessary in cases of delayed eruption, primary retention, traumatically intruded teeth, or subgingivally fractured teeth. Removable appliances are advantageous, as anchorage is not as tooth-dependant as in the case of fixed appliances. They are cost-effective, operator friendly, and a valuable treatment option to consider in cases where extrusion of anterior teeth in the transitional dentition is necessary. The purpose of this paper was to describe a simple, cost-effective technique using a removable appliance for extrusion of incisors in the transitional dentition.

Mandibular molar rehabilitation using orthodontic extrusion associated with odontoplasty.

The aim of this clinical report is to describe the successful treatment of a mandibular first molar presenting an extensive fracture at the buccal aspect in a young patient. The extension of the fracture was a negative prognostic factor for tooth maintenance. An alternative clinical treatment was proposed since the patient was young and presented with good oral hygiene and periodontal health. The treatment was based on orthodontic forced eruption associated with odontoplasty. A 3-year follow-up after the surgical procedure demonstrated the maintenance of periodontal health and good plaque cont

Immediate vs late orthodontic extrusion of traumatically intruded teeth.

Although the published papers about this matter is limited to clinical case reports, the aim of this review was to quantify the success rate of immediate vs late orthodontic extrusion of traumatically intruded teeth. From 55 reports in a PubMed and Medline computerized search, 13 reported patients involving 22 traumatically intruded anterior upper teeth with orthodontic extrusion were selected. In the sample of 13 patients, six were males and seven females and the average age was 16.4 years old. The selection criteria were patients presenting traumatized anterior upper teeth resulting in intru

Adhesive fragment reattachment after orthodontic extrusion: a case report.

In the treatment of crown fractures, adhesive fragment reattachment provides a good alternative to other restorative techniques, offering several advantages. The present paper reports a case in which the treatment of a cervical crown fracture was accomplished by reattaching the tooth fragment with a flowable resin composite. Orthodontic root extrusion was performed with a modified Hawley appliance prior to fragment reattachment. The clinical and radiographic results after 2.5 years were successful.

Combined apexification and orthodontic extrusion of a hypoplastic permanent canine.

Localized infection in the primary teeth was reported as an etiological factor in the formation of a hypoplastic defect in the underlying permanent teeth. The purpose of this report is to present the case of a 10-year-old girl with an unusual local hypoplasia in the mandibular left permanent canine and emphasize the importance of multidisciplinary management, including combined apexification and orthodontic extrusion. The 12-month recall examination revealed no pathology associated with the rehabilitation, and the patient's esthetic and functional expectations were satisfied.
