Ossification of posterior longitudinal ligament (OPLL) is a hyperostotic disease of the spine associated with myelopathy which is occurred by an anterior compression to the spinal cord. OPLL was first reported by Key GA in 1838, and was previously considered specific to east Asian people, especially Japanese. However, now OPLL is recognized as a subtype of diffuse idiopathic skeletal hyperosteosis, which is detected in Europe and the United States. We discuss the etiology and natural history of OPLL in this review.

Symptom prevalence of OPLL is estimated between 20% and 50%. Patients with OPLL have neurological disorders, pain/numbness, and trunk stiffness. These symptoms of stenotic myelopathy and of ossification. spondylosis deteriorate patients'quality of life. The ossification extends and elongates very gradually and tiny ossification at first visit hardly grows up to enough to cause myelopathy. A majority of patients experience a chronic course, but some suffer spinal cord injury. Preemptive surgery should be carefully contemplated because definite criteria have not been established yet.

The authors described 21 patients aged 37 to 83 with ossification of the posterior longitudinal ligament (10 with ischemic myelopathy, 6 with aftermaths of a trauma of the cervical spine, and 5 with the radicular syndrome on the basis of vertebral osteochondrosis. X-ray signs of three types of ossification of the posterior longitudinal ligament were defined and specified: linear, curved and mixed. The linear type was more frequently observed in the cervical spine whereas the curved type was more frequent in the lumbar spine.

OBJECTIVE: Ossification of posterior longitudinal ligament (OPLL) is a progressive disease that causes spinal canal compromise and serious neurological sequelae in advanced cases. The incidence of OPLL in the Asiatic population is 2%-3%, but the incidence is more in the background of fluorosis. Our aim was to study the association of OPLL with fluorosis by comparing urine fluoride levels and to study the types of OPLL.
MATERIALS AND METHODS: Thirty consecutive patients with OPLL, observed on cervical skiagram, and confirmed by the computed tomography (CT) of the cervical spine, underwent a 24

Forty patients with cervical myelopathy due to OPLL (Ossification of the Posterior Longitudinal Ligament) of the cervical spine were studied. According to Abe's or Yamamoto's classification, 12 of them had a 50% decrease in the cross-sectional area of the spinal canal. Subtotal vertebrectomy was carried out in 8 of these patients and the remaining 4 patients received posterior decompression. We concluded that anterior decompression, if possible, is the treatment of choice and posterior decompression is recommended only for the longitudinal type involving more than 3 segments.

Ossification of the posterior longitudinal ligament (OPLL) is a multi-factorial disease involving an ectopic bone formation of spinal ligaments. It affects 0.8-3.0% aging Asian and 0.1-1.7% aging European Caucasian. The ossified ligament compresses nerve roots in the spinal cord and causes serious neurological problems such as myelopathy and radiculopathy. Research in understanding pathogenesis of OPLL over the past several decades have revealed many genetic and non-genetic factors contributing to the development and progress of OPLL. The characterizations of aberrant signaling of bone morphog

OBJECTIVE: To discuss surgical approaches of ossification of the posterior longitudinal ligament (OPLL) of cervical spine.
METHODS: Between June 2005 to July 2010, 36 patients with OPLL of cervical spine were reoperated. There were 23 male, 13 female, age from 39 to 72 years (mean 57 years). The time of the first operation to the reoperation were 4 months to 24 years, an average of 3.9 years. Among 20 patients underwent anterior corpectomy and fusion (ACD) at first operation, 14 cases combined stenosis of cervical spinal canal, 10 cases were insufficient decompression of OPLL, 5 cases injured

Ossification of the posterior longitudinal ligament (OPLL) is most commonly found in men, the elderly, and Asian patients. There are many diseases associated with OPLL, such as diffuse idiopathic skeletal hyperostosis, ankylosing spondylitis, and other spondyloarthropathies. Several factors have been reported to be associated with OPLL formation and progression, including genetic, hormonal, environmental, and lifestyle factors. However, the pathogenesis of OPLL is still unclear. Most symptomatic patients with OPLL present with neurological deficits such as myelopathy, radiculopathy, and/or bow

The purpose of this study was to investigate the relationship of ossification of posterior longitudinal ligament (OPLL) with enthesis, the site where the posterior longitudinal ligament (PLL) attaches to the vertebral body, by multi-detector CT reconstruction images. Twenty-nine patients with OPLL were studied. According to the plaques' continuity to the vertebral body, OPLL plaques were classified into two categories: "free" and "contiguous". A "broken sign" was defined as a crack between two plaques. The sites where each "contiguous" plaque attached to the vertebral body were then analysed.

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) is a pathological process of ectopic calcification with a preponderance for the cervical spine. Epidemiological and familial studies have both indicated predisposition; however, the genetic inheritance pattern and responsible genes for OPLL are still uncertain. The aim of this study was to evaluate and summarize the current understanding of the genetics underlying OPLL.
METHODS: The authors reviewed epidemiological and genetic studies surrounding OPLL, with a particular focus on inheritance patterns and potential genes respons

We report a case of quadriplegia complicating ossification of posterior longitudinal ligament (OPLL) in a patient who was also found to have diffuse idiopathic skeletal hyperostosis (DISH). She also had osteomalacia (Vit. D deficiency) with secondary hyperparathyroidism. There could be a cause and effect relationship between the abnormal biochemistry and OPLL.

Ossification of posterior longitudinal ligament (OPLL) is a pathological process in which lamellar bone is deposited at the posterior longitudinal ligament and can lead to a limited range of cervical motion and spinal cord compression. A 64-year-old man presented with a 10-month history of worsening clumsiness in the hands and impaired gait, and he occasionally had a feeling of an electric shock in the limbs when the neck was flexed. Physical examination revealed atrophy of the intrinsic hand muscles, rapid reflexes in the lower extremities, and positive Hoffman sign and Babinski sign results.

Vitamin A and its synthetic congeners are known to produce a variety of skeletal abnormalities in patients on prolonged treatment with these medications. Two patients are described who developed posterior longitudinal ligament ossification following treatment with the synthetic retinoid 13-cis-retinoic acid. In both cases, this finding became apparent after other retinoid-induced skeletal abnormalities were observed and was less marked than the ossification of the anterior longitudinal ligament. Although spinal cord compression did not occur in our patients, patients on long-term retinoid ther

BACKGROUND: Both ossification of the yellow ligament (OYL) and the ossification of the posterior longitudinal ligament are relatively rare clinical entities. We report a extremely rare case of the spinal canal stenosis because of OYL, OPLL and listhesis at the cervicothoracic junction.
CASE DESCRIPTION: A 69-year-old man had progressive pain over his bilateral axillar portion and right lower extremity for 1 year. Radiology showed cervical canal stenosis with ossification of OYL, OPLL, and listhesis at the cervicothoracic junction. Posterior decompression therapy was performed, and he recovere

OPLL is considered as a multi-factorial disease influenced by genetic and environmental factors. To identify genetic factors for OPLL, Genetic Study Group of Investigation Committee on Ossification of the Spinal Ligament performed a genome-wide association study(GWAS)using 1,660 OPLL patients. The study group identified six susceptibility loci for OPLL:20p12.3(rs2423294:P= 1.10 × 10(-13)), 8q23.1(rs374810:P= 1.88 × 10(-13)),12p11.22(rs1979679:P= 4.34 × 10(-12)), 12p12.2(rs11045000:P= 2.95 × 10(-11)),8q23.3(rs13279799:P= 1.28 × 10(-10))and 6p21.1(rs927485:P= 9.40 × 10(-9)).A further functional

Ossification of the ligamentum longitudinale posterius is a rare disease found in particular in Japan and southeastern Asia. In the literature it is described as the so-called Japanese disease or OPLL syndrome. The disease takes a slow, frequently asymptomatic, course but may cause severe stenosis of the spinal canal with neurological disorders, OPLL occurs frequently concurrently with ankylosing spondylitis, hypertrophic spondylosis, diffuse idiopathic hyperostosis of the skeleton. Own observation: a 68-year-old man with a long history of cervicalgia, subsequently paraesthesias of the upper e

Ossification of the posterior longitudinal ligament (OPLL), though commonly reported among the Japanese population, has however been infrequently reported among other racial groups. It has not been reported among black Africans. A case of OPLL of the cervical spine seen in a 62-year-old Nigerian woman is hereby reported.

Ossification of the posterior longitudinal ligament (OPLL) is most commonly found in men, in the elderly, and in Asian patients. The disease can start with mild or no symptoms, but some patients progress slowly to develop symptoms of myelopathy. An accurate diagnosis through the use plain radiograph, computed tomography, and magnetic resonance imaging findings is very important to monitor the development of symptoms and to make decisions regarding a treatment plan. When symptoms are mild and non-progressive, conservative treatments and periodic observations are good enough, but once symptoms o
