Knee osteoarthritis (OA) typically presents with joint pain that is exacerbated by use and alleviated with rest. There is relatively brief, self-limited morning stiffness and absence of constitutional symptoms. Overweight and obesity are the most important modifiable risk factors. Although pharmacologic and nonpharmacologic interventions are generally effective at alleviating pain and improving physical function, they do not fundamentally reverse the pathologic and radiographic process of knee OA. As the severity of disease increases, the magnitude of pain and functional impairment intensifies

Symptomatic knee osteoarthritis (OA) can be viewed as the end result of a molecular cascade which ensues after certain triggers occur and ultimately results in irreversible damage to the articular cartilage. The clinical phenotype that knee OA can produce is variable and often difficult to accurately predict. This is further complicated by the often poor relationship between radiographic OA and knee pain. As a consequence, it can be difficult to compare studies that use different definitions of OA. However, the literature suggests that while there are multiple causes of knee OA, two have attra

Concepts regarding osteoarthritis, the most common joint disease, have dramatically changed in the past decade thanks to the development of new imaging techniques and the widespread use of arthroscopy that permits direct visualisation of intra-articular tissues and structure. MRI and ultrasound allow the early detection of pre-radiographic structural changes not only in the peri-articular bone but also in the cartilage, menisci, synovial membrane, ligaments and fat pad. The significance of MRI findings such as cartilage defects, bone marrow lesions, synovial inflammation/effusions and meniscal

BACKGROUND: Knee osteoarthritis (OA) is a major public health issue causing chronic pain, impaired physical function, and reduced quality of life. As there is no cure, self-management of symptoms via exercise is recommended by all current international clinical guidelines. This review updates one published in 2015.
OBJECTIVES: We aimed to assess the effects of land-based exercise for people with knee osteoarthritis (OA) by comparing: 1) exercise versus attention control or placebo; 2) exercise versus no treatment, usual care, or limited education; 3) exercise added to another co-intervention

Osteoarthritis is widely believed to result from local mechanical factors acting within the context of systemic susceptibility. Previous studies have demonstrated that malalignment is a potent predictor of disease progression in patients with osteoarthritis of the knee. Malalignment also mediates the effects of other risk factors, including obesity, quadriceps strength, laxity, and stage of disease. Recent studies have had conflicting results regarding whether malalignment increases the risk of incident tibiofemoral osteoarthritis. While the evidence is conflicting, malalignment appears to pla

Medial tibial plateaux excised during 46 unicompartmental arthroplasties for osteoarthritis were collected and photographed. The anterior cruciate ligament was intact in all joints. In every case the cartilage and bone erosion was centred anteriorly on the plateau and the posterior cartilage was intact. The site of the lesion and the intact state of the cruciate ligaments taken together explain why varus deformity was observed only in the extended knee, and why the deformity was correctable and had not become fixed. Failure of the anterior cruciate ligament may allow the erosion to extend post

Eighty-one patients with 116 osteoarthritic knees were seen by the author at the University College Hospital (UCH), Ibadan in six years. Below the age of 50 the disease was significantly commoner among Nigerians than Caucasians; 39.5% of our patients were less than 50 years old, compared with 1.1% of Caucasians. In contrast with the latter group, trauma was apparently an insignificant predisposing factor of knee degeneration in our patients. In some cases osteoarthritis was associated with severe valgus deformity in rather young patients, suggesting a probable causal relationship. Multiple joi

Osteoarthritis (OA) is a chronic disease that imposes a significant economic burden and deteriorates quality of life. Nevertheless, current therapeutic options for OA are limited to symptomatic remedies. As such, there is a high interest in novel methods for treating or preventing OA. One of the most promising medication modalities is through the clearance of cells that are cell cycle arrested but resistant to apoptosis, termed senescent cells. Additionally, these cells are also resistant to alternative programmed cell death modes, such as ferroptosis and pyroptosis. Senescent cells tend to ac

New imaging modalities are broadening the possibilities in osteoarthritis (OA) research, and are offering new insights to help better understand the pathogenesis of this disease. Although knee radiographs are widely employed in epidemiological and clinical studies to assess structural pathology, joint radiographs provide limited outcome measures in knee OA, and other more valid, reliable and sensitive imaging modalities are now available. In particular, magnetic resonance imaging can directly visualize articular cartilage and other joint structures, such as bone and soft tissue, that are now r

OBJECTIVES: To assess the risk of knee osteoarthritis (OA) attributable to obesity, and the interactions between obesity and other established causes of the disorder.
METHODS: We performed a population-based case-control study in three health districts of England (Southampton, Portsmouth and North Staffordshire). A total of 525 men and women aged 45 y and over, consecutively listed for surgical treatment of primary knee OA, were compared with 525 controls matched by age, sex and family practitioner.
RESULTS: Relative to a body mass index (BMI) of 24.0-24.9 kg/m(2), the risk of knee OA increa

Osteoarthritis is the most common joint disorder in the world and its incidence is rising. In Western populations it is one of the most frequent causes of pain, loss of function and disability in adults. The estimated lifetime risk for knee osteoarthritis is approximately 40% in men and 47% in women. The diagnosis of osteoarthritis is complex due to a lack of specific physical and/or laboratorial findings. The American College of Rheumatology (ACR) has recommended using the following criteria for the diagnosis of knee osteoarthritis: chronic knee pain (lasting for more than 6 weeks) and at lea

OBJECTIVE: Osteoarthritis (OA) is a multifactorial condition. The progression of knee OA is determined in part by mechanical effects on local structures. One of the mechanical influences on cartilage loss is limb alignment. We explored the structural factors associated with malalignment in subjects with symptomatic OA.
METHODS: We conducted a cross-sectional assessment using The Boston Osteoarthritis of the Knee Study, a natural history study of symptomatic knee OA. Baseline assessments included knee magnetic resonance imaging (MRI) and information on weight and height. Long-limb radiographs

Hip and knee osteoarthritis (OA) are leading causes of global disability. Most research to date has focused on the knee, with results often extrapolated to the hip, and this extends to treatment recommendations in clinical guidelines. Extrapolating results from research on knee OA may limit our understanding of disease characteristics specific to hip OA, thereby constraining development and implementation of effective treatments. This review highlights differences between hip and knee OA with respect to prevalence, prognosis, epigenetics, pathophysiology, anatomical and biomechanical factors,

OBJECTIVE: To determine the prevalence of generalized osteoarthritis in patients with knee osteoarthritis.
METHODS: Patients with knee osteoarthritis were recruited for a therapeutic trial via press announcement and selected based on American College of Rheumatology radioclinical criteria for femorotibial arthritis. Patients were asked to bring all their radiographs to the study visits, which included a physical examination. Each patient had an inclusion visit and a second visit 1 month later. The study physicians used a standardized chart to indicate clinical and radiographic evidence of ost

OBJECTIVE: We aimed to evaluate whether a recent knee injury was associated with accelerated knee osteoarthritis (OA) progression.
METHODS: In the Osteoarthritis Initiative, we studied participants free of knee OA on their baseline radiographs (Kellgren/Lawrence [K/L] <2). We compared 3 groups as follows: 1) individuals with accelerated progression of knee OA: defined as having at least 1 knee that progressed to end-stage knee OA (K/L grade 3 or 4) within 48 months, 2) common knee OA progression: at least 1 knee increased in radiographic scoring within 48 months (excluding those defined as ac

PURPOSE: In the last couple of years, a significant amount of studies comparing the UKA and TKA for unicompartmental knee osteoarthritis have been published. However, there is a lack of recent meta-analysis comparing the two implants. Since the number of performed UKAs is currently increasing and the indications are obsolete, it becomes important to update current evidences and outcomes. With these premises, a meta-analysis of clinical trials comparing UKA versus TKA was conducted.
METHODS: In October 2018, the following databases were accessed: Cochrane Systematic Reviews, Scopus, PubMed and

This study was undertaken to investigate whether high tibial osteotomy (HTO) still had a role in the treatment of osteoarthritis of the knee joint. The author has performed photoelasticity studies which confirmed abnormal stress distribution over the joint, as soon as its mechanical axis was deviated and the joint line had an obliquity over 10 degrees. High tibial osteotomy to correct varus or valgus deformity restores a symmetrical stress distribution and represents the only etiological treatment of secondary osteoarthritis of the knee. Two hundred and fifty HTO's were performed between 1971

BACKGROUND: Biomechanical markers including reductions in sagittal plane kinematics and moments, increases in knee adduction moments (KAM), and altered muscle activations have been identified as hallmark indicators of knee osteoarthritis (OA). However, it remains unknown whether these features of knee OA gait are exclusive to the diseased joint.
RESEARCH QUESTION: To determine whether specific gait outcomes previously linked to symptomatic medial compartment knee OA are unique to knee OA by concurrently investigating a group of asymptomatic individuals and those with hip OA.
METHODS: 16 indi

INTRODUCTION: Osteoarthritis is considered as a progressive disease that can result in disability. Radiological examination provides an effective non-invasive tool to visualize subtle changes. Therefore, more radiographic assessment methods are required to diagnose knee osteoarthritis at early stages. This study combined and evaluated six different radiographic measurements such as femorotibial angle between osteoarthritic group and non-osteoarthritic group, and assessed their inter-observer measurement reliability.
METHODS: A cross-sectional retrospective imaging study was conducted and incl

Knee osteoarthritis (KOA) is the most common form of joint degeneration with increasing prevalence and incidence in recent decades. KOA is a molecular disorder characterized by the interplay of numerous molecules, a considerable number of which can be detected in body fluids, including synovial fluid, urine, and blood. However, the current diagnosis and treatment of KOA mainly rely on clinical and imaging manifestations, neglecting its molecular pathophysiology. The mismatch between participants' molecular characteristics and drug therapeutic mechanisms might explain the failure of some diseas
