A 15-year-old boy suffering from osteoid osteoma of the right lamina of the axis presented with torticollis and local pain without neurological deficits. The tumour was treated by en-bloc resection and laminoplasty with autogenous iliac crest. The follow-up at 14 months showed complete pain relief and a perfect incorporation of the transplanted bone.

Osteoid osteomas located within the capsule of a joint are uncommon and present challenging diagnostic difficulties. A follow-up study, based on clinical, radiological and pathological information was performed of 20 patients who had suffered from an osteoid osteoma and of three patients who had been treated for an osteoblastoma in the period 1978-1988. 50% of the patients mentioned both pain at night and a good reaction to salicylates. Osteoid osteoma is a very painful benign bone tumor that affects young adults. It is exceptional before 5 and after 30 years of age. It can be seen in any part of the skeleton. A case of intracortical haemangioma in the tibial diaphysis is reported. The radiological and macroscopic features were identical with osteoid osteoma. In view of this similarity, haemangiomata, despite their rarity at this site, must be considered in the differential diagnosis of osteoid osteoma.

Osteoid osteoma is a common benign bone tumor of unknown etiology affecting the young adult with characteristic clinical and radiographic presentation in its common locations. However, when it arises in unusual intra-articular locations diagnosis may be misleading, often mimicking other entities leading to delayed management. We present a case series of three cases with intra-articular osteoid osteoma that were diagnosed with the help of correlative imaging.

BACKGROUND: Osteoid osteoma is a benign bone tumor. Its etiology is not fully understood and the role of trauma is still elusive.
OBJECTIVE: Osteoid osteoma mostly presents with a poorly localized pain that is worst at night and characteristically relieved by salicylates. It usually occurs on the weight-bearing bones of the lower extremities, but toe location is quite rare. Thirteen cases of osteoid osteoma of the phalanxes are reported: 10 fingers and 3 toes. Any phalanx may be affected, with no preference for a particular site, in the young adult. Segmentary tumefaction of the soft parts near the lesion is a more consistent sign than pain, which is often quite minimal. Radiographies demonstrate the increase in the size of the phalanx, whose matrix is condensed. All radiographic techniques must be used to determine the exact location of the nidus, that is always very small (1 to 2 mm), and whose total removal is the only guarantee of cure.
