[Malleolar osteotomy--osteotomy as approach].

Both arthroscopic and open surgical techniques may be used for treatment of osteochondral lesions of the ankle joint. Osteotomy around the ankle joint is a well established technique to extend the approach in cases where the osteochondral lesions are located more posteriorly. Medial, oblique, monoplanar malleolar osteotomy should be used in patients with lesions of the medial talus shoulder. The posterolateral ostechondral lesions are less frequent and in such cases distal fibular osteotomy is recommended. In this study the indications for different forms of osteotomy are discussed and the sur

Evolution of the Weil Osteotomy: The Triple Osteotomy.

Weil osteotomy (WO) is the most common technique worldwide for the treatment of mechanical metatarsalgia. The main indication for WO is propulsive/third rocker metatarsalgia that is in relation with an abnormal length of a certain metatarsal with respect to the neighboring metatarsals in the frontal plane. Most clinical studies have showed good to excellent results after WO. However, complications such as floating toes led to evolution of WO and the development of the triple-cut WO that allows for shortening coaxial to the shaft without plantar translation of metatarsal head. Other variations

Zadek Osteotomy.

The Zadek osteotomy, also known as a dorsal closing wedge calcaneal osteotomy, is a surgical procedure used to treat insertional Achilles tendinopathy and Haglund's deformity when conservative management fails. This procedure aims to reduce the bony prominence at the heel and to relieve Achilles tendon impingement to improve function and decrease pain. Recent innovations in minimally invasive/percutaneous approaches have enhanced outcomes while minimizing complications. Clinical and radiographic evaluation aids in diagnosis and surgical planning. Literature consistently demonstrates improved p

Torsional Osteotomy.

This article provides an overview of symptomatic torsional deformities of the lower extremity, and operative treatment techniques are described in detail. A definition of torsion versus rotation as well as information to physical examination and the relevance of radiological evaluation is given. Based on current literature and the own personal experience of the authors in osteotomies, surgical techniques at the proximal and at the distal femur, as well as at the tibia are presented.

Lumbar Osteotomy.

When a patient presents with spine problems, the spine surgeon would do well to avoid use of, reliance on, and acceptance of radiographs as the sole or primary source of information. Measurement of pelvic incidence and lumbar lordosis, although crucial, does not take into account the effort the patient must make to move, the level of involvement of other parts of the body, and the history of previous procedures and outcomes. Radiographs may show pathology that is not consistent with the appearance of the patient. How should we assess this situation?

Double osteotomy in the surgical treatment of Perthes' disease: Dega's transiliac osteotomy and subtrochanteric osteotomy.

Background. The combination of Dega transililiac and femoral subtrochanteric is used in our department in the treatment of Legg-Calvé-Perthes' disease. Subtrochanteric osteotomy, initially performed as varus-derotation osteotomy, is now mainly extension osteotomy with shortening. Femoral shortening is crucial for joint decompression and makes room for pelvic redirection after pelvic osteotomy. Material and methods. 34 children (36 hips) were analyzed retrospectively. All children were operated using subtrochanteric osteotomy with shortening and Dega's transiliac osteotomy of the pelvis. There

Chiari osteotomy in association with intertrochanteric osteotomy in adults.

The authors report the results obtained in 34 adult patients suffering from early degenerative arthritis of the hip following mixed dysplasia in which the reduction in the central-edge angle and the increase in the acetabular index is associated with modification of the neck-shaft angle and/or femoral anteversion. These patients underwent a combined Chiari iliac osteotomy (C.I.O.) and intertrochanteric osteotomy (In.O.). The results indicate that, when the indications and technique are respected, C.I.O. associated with In.O. is a reliable surgical method.

Osteotomy Planner: An open-source tool for osteotomy simulation.

There has been a recent emphasis in surgical science on supplementing surgical training outside of the Operating Room (OR). Combining simulation training with the current surgical apprenticeship enhances surgical skills in the OR, without increasing the time spent in the OR practicing. Computer-assisted surgical (CAS) planning consists of performing operative techniques virtually using three-dimensional (3D) computer-based models reconstructed from 3D cross-sectional imaging. The purpose of this paper is to present a CAS system to rehearse, visualize and quantify osteotomies, and demonstrate i

Complications of minimally invasive calcaneal osteotomy versus open osteotomy.

BACKGROUND: Calcaneal osteotomy is an established technique for correcting hindfoot deformity. Patients traditionally receive an osteotomy through the open lateral approach to the calcaneus. To reduce the rate of wound complications associated with a direct open lateral approach, a minimally invasive surgical (MIS) technique has been adopted. This uses a low-speed, high-torque burr to perform the same osteotomy under radiographic guidance. We hypothesized that the new MIS calcaneal osteotomy would be a safe alternative to open calcaneal osteotomy while obtaining the same displacement. METHODS

[SCARF osteotomy].

Hallux valgus is one of the most common deformities of the lower extremity. Multiple etiologies have been discussed, and various techniques have been described. The SCARF osteotomy is indicated for moderate to severe hallux valgus deformities. The technique historically used by carpenters allows immediate postoperative weight bearing and also entails the possibility to correct the distal metatarsal articular angle by rotation of the osteotomy. The SCARF osteotomy has also been successfully used in revision surgeries.

[Olecranon osteotomy].

In recent years the increase of interest for surgery of the elbow has fostered the development of new and modified triceps sparing approaches. Olecranon osteotomy is a widely accepted method especially for open reduction and internal fixation of displaced intra-articular fractures because it provides excellent exposure of the joint surfaces. Due to several reports describing numerous complications, the enthusiasm for this procedure is currently limited. Several intra-articular and extra-articular techniques for refixation of the olecranon have been developed to limit these complications and go

Supramalleolar osteotomy.

Supramalleolar osteotomy for posttraumatic malunion, developmental or physeal deformities, congenital malalignment, or focal articular degenerative problems in the ankle is a useful surgical technique. Accurate evaluation of the distal tibial deformity, weight-bearing radiographs, and soft tissue examination for contractures and instability ensure proper decision making. Osteotomy planning must involve all components of the deformity, including the foot, and respect osteotomy principles.

Chiari osteotomy.

Chiari osteotomy as the therapeutic approach in congenital hip dislocation has a special value in the adolescent patient with acetabular dysplasia and femoral head subluxation. The advantage is in the preservation of as much as possible of the modeling relationship of femoral head and acetabulum involved in hip joint motion while creating an effective lateral femoral head cover. Experience with Chiari osteotomy is described in a small group of adolescents (average age, 14 years) whose clinical problem is associated with limp, increasing fatigability and recurrent pain, with discussion of the c

Calcaneal osteotomy.

The calcaneal osteotomy for the correction of valgus and varus deformities of the foot is a relatively simple and effective procedure. Since the main theme of the seminar was "Practical Reconstructive Foot Surgery," Dr. Sullivan's comments were limited to the principles of osteotomy with a brief discussion of the procedural techniques. The principles which he discusses relate to the goals to be achieved by osteotomy based on biomechanics. He also covers instrumentation, postoperative management, complications, tissue handling and choice of osteotomy sites.

High tibial osteotomy: osteotomy in minus or hemicallotasis with EAF?

Between January 1988 and November 1994 a total of 29 patient were submitted to high tibial osteotomy to treat varus knee. Osteotomy in minus stabilized by staples and plaster was used in 13 cases, a monoaxial external fixator (AEF) associated with hemicallotasis was used in 16. After a period of time ranging from 11 to 58 months satisfactory results were obtained in 90% of the cases in the first group, and in 95% of those in the second. As compared to the first surgical procedure the second was characterized by: a) a simpler surgical technique; b) reduction in time required for surgery and sur

Ganz periacetabular osteotomy: How to make the osteotomy cuts easier?

UNLABELLED: In 1982, Dr. Reinhold Ganz [1] introduced a new periacetabular osteotomy (PAO) as a therapeutic measure for hip dysplasia. The primary objective of this surgical procedure was the strategic reorientation of the acetabulum to optimize coverage of the femoral head and increase the surface area of the acetabular cartilage over the femoral head, all while preserving the structural integrity of the posterior column and maintaining the natural pelvic shape. The complexity of the Ganz periacetabular osteotomy presents a formidable learning curve, owing to the proximity of numerous neurova
