The sagittal ramus osteotomy is a versatile procedure, routinely performed for the correction of various dentofacial deformities. Despite the popularity of this operative approach some problems may occur related to several technical and anatomic aspects of this technique. Complications are in a large part avoidable with some tricks and little modifications of the original procedure.

Sagittal split ramus osteotomy (SSRO) is one of the most common maxillofacial operations, and the technique relies on a directed fracture involving different biomechanical variables. The aim of this study was to find out the biomechanical characteristics involved during each step of sagittal split osteotomy. We sampled eight fully dentate human mandibles and used the right side for hardness tests and the left side for a traction-to-fracture test within an unfinished SSRO. Right sides were sampled in five parts underlying the corticotomy course and tested with a hardness testing automatic machi

Protecting the adjacent soft tissues using a channel retractor prevents complications, such as excessive bleeding, during sagittal split ramus osteotomy. However, the saucer-shaped blade of the conventional channel retractor fits poorly into the posterior border of the mandibular ramus during medial osteotomy, which is typically performed parallel to the occlusal plane. Therefore, a novel channel retractor was developed with an adjusted blade bending angle, based on cephalometric data. The lateral cephalograms of 339 Japanese patients with jaw deformities were collected. Cephalometric landmark

The aim of this study was to elucidate the anatomical structures that are vulnerable to injury during sagittal split ramus osteotomy (SSRO). Twenty-nine hemifaces of 19 Korean adult cadavers (11 men and 8 women; age range, 50-91 years) were dissected, and the locations of the facial nerve, retromandibular vein (RMV), and external carotid artery (ECA) were measured on the base of the mandibular posterior border and occlusal plane. Sagittal split ramus osteotomy was performed on the cadaver heads at intervals of 10 mm, and the proximity of the facial nerve was observed. The buccal branch and man

The sagittal split ramus osteotomy is probably the most frequently used procedure for correction of mandibular skeletal dentofacial deformities. Despite numerous improvements in the technique in the 30 years since the procedure was introduced, a number of troublesome complications still occur. These include unfavorable fracture during surgery, paresthesia, and relapse. The purpose of this study was to determine where fusion of the buccal and lingual cortical plates occurs in the upper mandibular ramus, as it is thought that placement of the horizontal medial osteotomy above the point of fusion

Prediction of neurosensory deficit in the lower lip and chin after sagittal split ramus osteotomy (SSRO) is challenging. This study aimed to elucidate factors related to the development and improvement of neurosensory disturbance (NSD) after SSRO with respect to surgical procedure and the anatomical and structural characteristics of the craniomaxillofacial skeleton. Subjects comprised 50 patients treated by a single experienced surgeon. Anatomical data and landmarks were obtained by computed tomography (CT) imaging. There was a significant difference between patients with or without NSD for th

The sagittal split mandibular ramus osteotomy is a versatile surgical option for the correction of mandibular prognathism, retrognathism, and asymmetry. At the same time, it enables the use of internal rigid fixation. However, surgical complications including injury to the inferior dental neurovascular bundle and unfavorable fracture can occur. A technical modification and a concept in instrumentation is suggested with the view of minimizing such complications.

Mandibular osteotomy carries with it a risk of damaging blood vessels and nerves when using traditional surgical techniques. Piezosurgery®, is a new technique that uses ultrasonic vibration to enable bone-selective sectioning without damage to the surrounding soft tissues. However, paralysis may not be completely eliminated using Piezosurgery® for osteotomy. We investigated how piezoelectric surgery in bilateral sagittal splitting ramus osteotomy (BSSRO) affected the surrounding soft tissue. Forty-four patients with skeletal mandibular prognathism underwent mandibular setback with BSSRO. Patie

In surgery for facial asymmetry, mandibles can be classified into two types, rotational and translational, according to the required mandibular movements for surgery. During surgery for rotational mandibular asymmetry, a bilateral sagittal split ramus osteotomy (BSSRO) may cause a large bone gap between the proximal and distal segments as well as condylar displacement, resulting in a relapse of the temporomandibular joint disorder, especially in severe cases. The intraoral vertical ramus osteotomy has an advantage, in this respect, because it causes less rotational displacement of the proximal

Orthognathic surgery is highly effective in improving overall facial esthetics, in addition to achieving an ideal occlusion. Sagittal split ramus osteotomy (SSRO) of the mandible is the mainstay of modern orthognathic surgery, but intraoperative injury to the inferior alveolar nerve (IAN) remains one of the common complications. The authors report a case of 19-year-old male patient with left cleft lip and palate who received orthognathic surgery involving SSRO. The patient's right IAN ran close to the outer cortex of the mandible, but SSRO was successfully performed without injuring the nerve.

Sagittal split ramus osteotomy (SSRO) has wide-ranging indications and results in simultaneous improvements in occlusion and facial appearance. Thus, it is the most frequently used osteotomy for jaw deformities. Its main intraoperative and postoperative complications are massive bleeding, atypical fractures, inferior alveolar nerve paralysis, and relapse. This report describes a case of temporomandibular joint dislocation, a rare complication of SSRO. The patient was a 23-year-old man with mandibular prognathism which was treated by bilateral SSRO. Postoperative x-ray showed that the left temp

In sagittal split ramus osteotomy (SSRO) surgery, variable methods are used to maintain the proximal segment in the anatomic position during fixation to minimize early built-in relapse. However, there is still a chance of condyle malposition. In this study, a new titanium plate, which has a sliding oval hole in its distal part, was used to enable postoperative mobility of the proximal segment. This plate makes it possible to reposition the condyle during the postoperative intermaxillary fixation period. These plates were used successfully in 23 patients who received SSRO setback surgery.
