Traveling with an Ostomy.

People undergoing ostomy surgery face an extended physiologic and psychosocial recovery. It is not uncommon to experience interruptions in work, pursuing life goals, relationships, physical activity, and social life. Even those with previous travel experience may feel inhibited by their new ostomy. Nurses, including those experienced in wound and ostomy continence, provide essential care to patients recovering from surgery and adjusting to their stoma. With counseling, education, and experience, patients can develop increasing competence and autonomy. This, in turn, helps an ostomate look forw

Psychosocial adjustment following ostomy.

Ostomy patients have been identified as a chronic illness population frequently experiencing adjustment difficulties. The present study, based on the biopsychosocial model (Engel, 1977) of chronic illness, examined a range of post-surgical adjustment difficulties in a sample of 131 ostomy patients. The patient population reported experiencing a significant number of technical, emotional, social, marital/family, and sexual difficulties post-surgically. Technical difficulties were associated with impaired emotional, social, and marital/family functioning. Emotional difficulties were also associa

The impact of an ostomy on sexuality.

Creation of an ostomy results in physiological and psychological changes that affect sexuality. Major physiological complications for men include erectile dysfunction and ejaculatory difficulties. For women, dyspareunia is the most common physiological complication. The presence of an ostomy can alter a person's body image, which, in turn, influences the desire for sexual activity. Sexuality concerns should be addressed with all patients undergoing ostomy placement. The PLISSIT model, which outlines four stages of interventions used in sexual counseling, can be used to guide nursing care.

Ostomy Complications in Crohn's Disease.

Prostatic abscess (PA) is a rare clinical entity due to a variety of causative organisms including gram-negative bacilli, anaerobic and fungal agents. We report on a 55-year-old, HIV+ patient presenting with a 2-week history of urethral discharge and a large PA. He was successfully treated with a combination of radiological-guided transperineal drainage plus antibiotics. Treatment decisions in patients with PA are multifactorial and should be made with all diagnostic information available from the most current modes of medical imaging. In the case of PA several factors should be assessed, incl

Morbidity and mortality of ostomy closure.

Ostomy closure following the Hartmann procedure is perceived to be associated with higher morbidity and mortality rates than is ostomy closure following divided colostomy, loop colostomy, and divided ileostomy-colostomy so that ostomy closure after Hartmann procedure may be denied to certain patients. To test that perception, the charts of 59 patients undergoing a Hartmann procedure and 43 patients having ostomy closure after divided colostomy, loop colostomy, or divided ileostomy-colostomy were reviewed. Ostomy closure after Hartmann procedure was accomplished in 46 patients. These 46 patient

Factors associated with neonatal ostomy complications.

BACKGROUND: Neonatal ostomies, either temporary or permanent, are created for numerous reasons. Limited attention has been given to understanding what factors might place infants at risk for surgical wound complications. PURPOSE: The purpose of the study is to identify factors associated with risk of significant abdominal wound complications (wound dehiscence and wound infection) following neonatal ostomy creation. METHODS: This is a retrospective chart review of infants undergoing ostomy between January 2009 and December 2013 at the University of Alberta Hospital. MAIN FINDINGS: 66 infants

Providing sexual information to ostomy patients.

Forty patients with a permanent colostomy, ileostomy, or ileal conduit were interviewed. Besides changes in sexual performance postoperatively, the authors specifically attempted to determine answers to other sexual variables such as attractiveness, appliance problems, and partner reactions. The extent of information provided to patients on sexuality prior to the permanent ostomy was also examined. There was a significant but predictable incidence of male impotence and female dyspareunia after surgery. Despite innumerable sexual variables, other than performance, which these patients faced pos

Ostomy or intestinal anastomosis in cases of peritonitis.

Twenty-six patients showing peritonitis due to nontraumatic acute abdomen were submitted to ostomy. Mean age was 51 years (range 25-83), being 13 males and 13 females. Bowel obstruction (BO) was the most frequent cause of peritonitis (11 cases), followed by intestinal perforation (IP) (8 cases), acute mesenteric infarction (AMI) (5 cases), and acute abdomen of inflammatory/infectious origin (AAIO) (2 cases). Brook's ileostomy was performed on 65% of the patients. Jejunostomy was performed only in 4 patients, leading to a bad evolution. Overall mortality was 54%. Primary ostomy or anastomosis i

Ostomy Management for Pediatric Perianal Crohn's Disease.

INTRODUCTION: Pediatric patients with perianal Crohn's Disease (CD) suffer recalcitrant fistulas, abscesses, and strictures. Fecal diversion is a palliative last resort, but the expected clinical course and long-term management of the ostomy for this population is unclear. We sought to identify factors predictive of ostomy takedown and establish management recommendations for fistulizing and stenosing disease. METHODS: We reviewed our institutional registry for patients aged 1-18 years with CD who received perianal surgery from 2011 to 2021. We analyzed medical therapy, examinations under ane
