The metabolic matrix hypothesis of recurrent BPPV: Otoconia-otolithic membrane-endolymph fragility.

Benign paroxysmal positional vertigo (BPPV) is treated as a purely mechanical disorder of displaced otoconia, yet this model alone does not readily account for spontaneous otoconia detachment, bilateral or multicanal disease, high recurrence rates, or the clustering of BPPV with systemic metabolic conditions in a subset of patients. This article proposes and structures a unifying mechanistic model-the Metabolic Matrix Hypothesis-linking systemic metabolic states to otoconial fragility, and derives testable predictions from it. It is a hypothesis-generating narrative and theoretical synthesis-n

Vestibular Drop Attacks and Meniere's Disease as Results of Otolithic Membrane Damage-A Numerical Model.

Meniere's disease (MD) is a condition of the inner ear with symptoms affecting both vestibular and hearing functions. Some patients with MD experience vestibular drop attacks (VDAs), which are violent falls caused by spurious vestibular signals from the utricle and/or saccule. Recent surgical work has shown that patients who experience VDAs also show disrupted utricular otolithic membranes. The objective of this study is to determine if otolithic membrane damage alone is sufficient to induce spurious vestibular signals, thus potentially eliciting VDAs and the vestibular dysfunction seen in pat

Otolith Membrane Herniation, not Semicircular Canal Duct Dilation, Is Associated with Decreased Caloric Responses in Ménière's Disease.

Ménière's disease (MD) is a debilitating disorder with unclear pathophysiology whose diagnosis often relies on clinical judgment rather than objective testing. To complicate matters further, a dissociation has emerged between two vestibular function tests commonly used in patients with MD to examine the same end-organ (the semicircular canals): the caloric test and video head impulse testing (vHIT). Caloric responses are often abnormal, while vHIT results remain normal. Explaining this dissociation could reveal novel insights into MD pathophysiology. Here, we conduct a histopathological study

The new vestibular stimuli: sound and vibration-anatomical, physiological and clinical evidence.

The classical view of the otoliths-as flat plates of fairly uniform receptors activated by linear acceleration dragging on otoconia and so deflecting the receptor hair bundles-has been replaced by new anatomical and physiological evidence which shows that the maculae are much more complex. There is anatomical spatial differentiation across the macula in terms of receptor types, hair bundle heights, stiffness and attachment to the overlying otolithic membrane. This anatomical spatial differentiation corresponds to the neural spatial differentiation of response dynamics from the receptors and af

Types of Inheritance and Genes Associated with Familial Meniere Disease.

Meniere disease (MD) is a rare disorder of the inner ear defined by sensorineural hearing loss (SNHL) associated with episodes of vertigo and tinnitus. The phenotype is variable, and it may be associated with other comorbidities such as migraine, respiratory allergies, and several autoimmune disorders. The condition has a significant heritability according to epidemiological and familial segregation studies. Familial MD is found in 10% of cases, the most frequently found genes being OTOG, MYO7A, and TECTA, previously associated with autosomal dominant and recessive non-syndromic SNHL. These fi

Gentamicin ototoxicity in otoconia: quantitative electron probe X-ray microanalysis.

Chronic gentamicin ototoxicity was evaluated in the otolithic membrane of adult OF1 mice at the otoconial layer of the saccule and utricle by quantitative electron probe X-ray microanalysis of Ca and K. The otolithic membranes were plunge-frozen and freeze-dried. The analysis was carried out with an energy dispersive detector using the peak-to-back-ground ratio method and different inorganic salts of Ca and K as standards to calibrate the microprobe. Ca and K in the otoconia are related via a linear function in both the saccule and the utricle. This association is not maintained after exposure

Expression of cochlin in the vestibular organ of rats.

The COCH gene mutated in autosomal dominant sensorineural deafness (DFNA9) encodes cochlin, a major constituent of the inner ear extracellular matrix. Cochlin constitutes 70% of the inner ear protein and cochlin isoforms can be classified into three subgroups, p63s, p44s and p40s. Symptoms of some DFNA9 patients are consistent with those of Ménière's disease. Here, we report the expression of cochlin in the vestibular organ of rats using isoform-specific antibodies that recognize all three isoforms. Cochlin is highly expressed in the stromata of the maculae of otolithic organs and cristae of s

Do patients with Ménière's disease have attacks of syncope?

The aim of the present study was to evaluate the prevalence and associated factors for syncope among patients with Ménière's disease (MD). An attack of syncope was defined as a sudden and transient loss of consciousness, which subsides spontaneously and without a localizing neurological deficit. The study used an across-sectional survey design. Information from a database consisting of 961 individuals was collected from the Finnish Ménière Association. The data contained case histories, general health-related quality of life (HRQoL), and impact measurements of the complaints. In the current st

Regional fidelity vs dispersal of capelin (Mallotus villosus) in the Gulf of St. Lawrence, inferred from otolith chemistry.

Capelin (Mallotus villosus), a key forage fish in the Northwest Atlantic, links zooplankton to predators including commercial fishes, seabirds, and marine mammals, yet its life-cycle connectivity in the Gulf of St. Lawrence (GSL) remain poorly understood. Between 2022 and 2024, otoliths from 927 individuals collected during and after spawning were analysed by Laser Ablation Inductively Coupled Plasma Mass Spectrometry (LA-ICP-MS), and seven trace elements (Li, B, Mg, K, Zn, Sr, Ba) were used to discriminate three chemically distinct regions within the GSL. Early-life regional origin was inferr

Vestibular compensation of otolith graviceptive dysfunction in stroke patients.

BACKGROUND AND PURPOSE: A sensitive and frequent clinical sign of a vestibular tone imbalance is the tilt of the perceived subjective visual vertical (SVV). There are no data yet focusing on lesion location at the cortical level as a factor for predicting compensation from the tilt of the SVV. METHODS: With modern voxelwise lesion behavior mapping analysis, the present study determines whether lesion location in 23 right-hemispheric cortical stroke patients with an otolith dysfunction could predict the compensation of a vestibular tone imbalance in the chronic stage. RESULTS: Our statistical

Fish otolith microchemistry as a biomarker of the world's largest mining disaster.

The Fundão dam collapse occurred in late 2015, resulting in the largest disaster in the world involving mine tailings, releasing at least 50 million m3 of iron ore tailings into the Doce river basin (Brazil). Studies realized along the Doce river after the disaster showed high concentrations of some elements above recommended Brazilian standards in different environmental matrices such as Al, Ba, Co, Fe, Hg, Mn, and Ni. This study aims to test the hypothesis that otolith microchemistry can be used as a pollution biomarker over time, that is, the otoliths sampled in the directly impacted area (

Two-dimensional otolith size trends in myctophid fishes.

Lanternfishes are a key food source for marine and terrestrial predators, and play an important role in energy transfer between surface waters and deeper layers. This study investigates otolith size variation in 105 myctophid species included in the AFORO database. The study covers the five Myctophidae subfamilies: Diaphinae, Gymnoscopelinae, Lampanyctinae, Myctophinae and Notolychninae. Several measurements were taken from the otoliths, and relative shape and sizes indices were calculated for the different fishes. Morphometric analyses revealed that Diaphinae and Gymnoscopelinae possess the l

Remarkable dominance of myctophid otoliths in Upper Miocene Chagres Formation, Caribbean Panama.

Marine fossils from the Upper Miocene Chagres Formation in northern Panama offer critical insights into the paleoenvironmental conditions and ecological responses prior to the separation of the Atlantic from the Pacific by the formation of the Isthmus of Panama. Here we present a systematic study based on more than 6,200 otoliths collected from a coastal exposure near the town of Piña, Colón. This assemblage is remarkable for the extraordinary dominance of the family Myctophidae, constituting over 96% of specimens. The otolith density in the sediments is among the richest known globally (278.8

Anatomical and theoretical observations on otolith repositioning for benign paroxysmal positional vertigo.

OBJECTIVE: To determine if there is an anatomic basis for the assumption that loose, "rogue" otoliths presumed to arise from the utricular macula and theorized to cause benign paroxysmal positional vertigo (BPPV) by impinging on semicircular canal ampullae could be returned to their original site by a series of changes in the position of the head called particle repositioning maneuvers (PRMs). Further, if such otolith movement were possible, once they were replaced into the utricle, would they adhere to the utricular macula? STUDY DESIGN: Kodachrome photographs of 2-mm-thick macrosections of

Benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo should be considered when a patient complains of attacks of dizziness of brief duration, that seem to occur under particular circumstances related to the position of the head. The true nature of the disorder may be missed unless examination is carried out in a certain way: the head must be positioned below the rest of the body and turned to one or other side. Diagnosis then depends on the occurrence of vertigo and on noting the transient nystagmus. There are a number of causes of benign paroxysmal positional vertigo-head injury is a relatively frequent one.

Viral labyrinthitis: early pathology in the human.

The histologic findings in the temporal bones of three patients who died from viral encephalopathy are presented. Pathology was restricted to the scala media, vestibular labyrinth, and internal auditory canal and was considered to be expressions of viral labyrinthitis. The changes were different degrees of degeneration of the organ of Corti, early encapsulation of the tectorial membrane, degeneration of the stria vascularis, and round cell infiltration of the modiolus and contents of the internal auditory canal. A new finding in the organ of Corti and early stages of cystic degeneration of the

A model of otolith stimulation.

A new model of otolithic stimulation by linear acceleration is presented and compared to previous models, based upon anatomical evidence and on the ability of normal subjects to sense the direction of a linear acceleration vector acting in the coronal plane (roll-tilt perception). There are two basic methods of generating roll-tilt stimuli: 1) tilt-chairs either inside or outside a centrifuge and 2) fixed-chair centrifuges. The present model is based on consideration of the probable otoconial displacement produced by these two different methods of stimulation and the model incorporates a major
