Common Otologic Surgical Procedures: Clinical Decision-Making Pearls and the Role of Imaging.

Neuro-otologists rely on the expertise and judgment of a skilled neuroradiologist to identify radiographic abnormalities in the complicated regional anatomy of the temporal bone and middle and posterior fossa, and more importantly, to alert the surgeon to potential operative pitfalls. This article highlights some of the common otologic surgical procedures that stress this important dynamic. The surgical perspective on quick and effective clinical decision-making pearls to keep in mind during a thorough radiographic analysis of the ear and lateral skull base is presented.

Facial nerve monitoring in otologic surgery: clinical indications and intraoperative technique.

While identification of the intratemporal portion of the facial nerve is mandatory in most otologic surgical procedures, inadvertent instrumentation, traction, or thermal injury may still result from inaccurate delineation, purposeful avoidance, or false protection of this critical structure. Improved functional preservation of the facial nerve has been achieved in acoustic neuroma surgery through the monitoring of evoked facial electromyographic activity. This technique may also be used during otologic procedures in which facial nerve manipulation is anticipated in the management of recurrent

Otologic surgery in HIV-infected patients.

As the lifespan of patients with HIV continues to increase secondary to improvements in antiretroviral medications, patients can keep the virus in a latent state for a longer period of time than ever before. As a result, patients live longer with infection and demand a higher quality of life during the course of the disease. The most important decision in the management of otologic manifestations of AIDS is whether the patient should be operated. The surgery of HIV-infected patients includes two important issues: Risk of transmission of HIV to health care workers and delay of wound healing. In

COVID-19 and ear endoscopy in otologic practices.

PURPOSE: Otolaryngologists have had to postpone the majority of surgical procedures in the current COVID-19 pandemic. Airborne transmission, beyond the projection of droplets from upper airways, expose healthcare workers to a risk of viral infection. Aerosol generating procedures (AGP) increase the risk of viral transmission to staff within the operating room. METHODS: Surgery of middle ear and mastoid is also considered an AGP, particularly mastoidectomy performed using a high-speed drill. The authors report their experience in endoscopic ear surgery as an alternative technique to reduce AGP

How standardised are antibiotic regimens in otologic surgery?

BACKGROUND: Within otologic surgery, a paucity of well-controlled studies assessing the use of systemic antibiotic to reduce surgical site infections exists. Moreover, discrepancies in wound classification of procedures challenge consensus in antimicrobial prescribing patterns. We sought to compare surgeons from two different health systems to examine how surgeons' prescribing habits compared to practice guidelines for numerous otologic procedures. METHODS: An online questionnaire was distributed to 33 Canadian and 32 Austrian surgeons who regularly perform otologic surgery. Current systemic

Drill-induced noise level during cochleostomy.

CONCLUSIONS: Noise induced by drilling during cochleostomy is of high level and can cause acoustic trauma to the inner ear, particularly after the membranous labyrinth has been exposed. Our method utilizes non-invasive noise measurement and the equipment that is connected to the operative area can be sterilized, thus it can be applied to intraoperative recordings. OBJECTIVES: To investigate the noise level generated by otological electrical drills during the cochleostomy procedure and to further explore the feasibility of noise monitoring in vivo. METHODS: Acoustic measurements during drilli
