[Ototoxicity].

Ototoxic drugs, the best known of which are aminoglucoside antibiotics, can cause irreversible lesions in the ciliate cells of the organ of Corti and the maculae and crests of the vestibular apparatus. The cellular lesion is linked to the formation of complex substances between the ototoxic drug and the polyphospho-inositides of the cellular membrane. Auditory and vestibular monitoring must be ensured permanently throughout the use of such drugs, especially in the presence of risk factors. The lesions induced experimentally in animals having studied by electrophysiological methods, with record

Developmental ototoxicity.

The aminoglycoside antibiotics are active bactericidal agents used for the treatment of tuberculosis and gram-negative bacterial infections. Well over 2 million patients per year receive aminoglycosides; however, a major limitation to their use is ototoxicity (cochleotoxicity and vestibulotoxicity), nephrotoxicity, and neuromuscular blockade. Ototoxicity can result from aminoglycoside administration to any patient population, including adults, fetuses, and preterm and full-term neonates. Preterm infants and experimental animals are hypersensitive to aminoglycoside ototoxicity during the period

Aminoglycoside ototoxicity.

Aminoglycoside antibiotics are frequently employed in the treatment of serious infections caused by aerobic gram negative bacilli. The use of these potent antibacterial agents is limited by the risks of ototoxicity and nephrotoxicity. Aminoglycosides are excreted by glomerular filtration at a rate proportional to the serum concentration. Impaired renal excretion reduces the rate of clearance from the serum. Utilizing information about aminoglycoside pharmacokinetics, the susceptibility of infecting pathogens to aminoglycosides, and risk factors for ototoxicity, physicians can attempt to optimi

Are some ear drops ototoxic or potentially ototoxic?

In this article we review the literature concerning the capacity of certain substances to cause ototoxicity when administered topically. The ototoxicity experimentally observed in animals only occurs in humans under certain circumstances. It is concluded that many of these products. which are commonly used for treatment in humans, are ototoxic in animals and may also be ototoxic in humans. Therefore, the use of other substances which have the same action but are not ototoxic

Aminoglycoside ototoxicity.

The author participated in two prospective studies of patients receiving aminoglycoside antibiotics. In the first study, 54 patients received amikacin, and 54 received gentamicin. In the second study, 61 patients received gentamicin, 50 received netilmicin, and 52 received tobramycin. The studies were randomized and the investigator was blinded when evaluating auditory and vestibular toxicity and nephrotoxicity. All patients had pure tone audiometric evaluations, and two thirds of the patients had vestibular function tests consisting of an electronystagmogram performed with 30 degrees C and 44

Erythromycin ototoxicity.

Erythromycin is considered one of the safest antibiotics in common use today. In its otolaryngologic use, the authors have found it effective in treating acute suppurative sinusitis and occasionally otitis media, when combined with sulfonamides. There are few complications of erythromycin administration. Probably the least generally acknowledged of these is ototoxicity. There have been three reports of six cases with ototoxic complications from erythromycin, primarily from administration of its intravenous form. The authors present a case study of an 18 year old girl in severe renal failure, w

[Current aspects of ototoxicity. Ototoxic substances and their effects].

Ototoxicity describes reversible or irreversible disorders of inner ear functions due to the influence of chemical, biological, or physical substances. Ototoxicity should be kept in mind during differential diagnosis of hearing loss, tinnitus, dizziness, and vertigo. In clinical practice, drug-induced ototoxic effects play a major role. The otorhinolaryngologist should also be involved in interdisciplinary cooperation, e.g., during treatment with antineoplastic chemotherapeutic agents with potential ototoxic side effects. In clinical practice, multimedication and interactions between different

Aminoglycoside-induced ototoxicity.

It has long been known that the major irreversible toxicity of aminoglycosides is ototoxicity. Among them, streptomycin and gentamicin are primarily vestibulotoxic, whereas amikacin, neomycin, dihydrosterptomycin, and kanamicin are primarily cochleotoxic. Cochlear damage can produce permanent hearing loss, and damage to the vestibular apparatus results in dizziness, ataxia, and/or nystagmus. Aminoglycosides appear to generate free radicals within the inner ear, with subsequent permanent damage to sensory cells and neurons, resulting in permanent hearing loss. Two mutations in the mitochondrial

[Ototoxicity-related dysequilibrium].

Many substances may be the source of dizziness or transient equilibrium disturbances due to dysfunction of the CNS or to an impairment of visual or proprioceptive informations. Other agents are responsible for drop of arterial pressure by changing position, including antihypertensive drugs, alpha-blocking agents used in urology, antipsychotics, cyclic antidepressants, vasodilators and nitrates, dopaminergic antiparkinson drugs, sedatives, etc. Only drug with true ototoxic properties will be discussed here, namely substances that are able to damage the inner ear (cochlear or vestibular damage)

Ototoxicity of netilmicin.

Ototoxicity and efficacy of netilmicin was evaluated in 76 consecutive patients with moderate to severe infections with Gram-negative rods and Staphylococcus aureus. Netilmicin alone was efficacious in 47 patients and in another 23 patients cure or improvement was seen with netilmicin combined with another antibiotic, usually penicillin. Therapeutic failure was seen in 4 cases, 2 of which were regarded as netilmicin failures. Vestibular function and hearing acuity was thoroughly examined by electronystagmography and audiometry. In spite of the extensive examination of the audiovestibular funct

Cisplatin ototoxicity histopathology.

This study investigates the histopathological changes of the cochlea and vestibular end organs of a patient who received cisplatin for Hodgkin's lymphoma. He experienced acute high-frequency sensorineural hearing loss and tinnitus after receiving treatment. Using histopathological analysis of his temporal bones after he unfortunately succumbed to his disease, we found that the ototoxic effect of cisplatin is primarily within the cochlea, with significant damage located at the organ of Corti at the base-hook region, consistent with findings in animal models. The effects of cisplatin were minima

Ototoxicity and Teprotumumab.

OBJECTIVES: Teprotumumab, a novel monoclonal antibody, targets the insulin-like growth factor 1 (IGF-1) receptor. IGF-1 receptors, found in muscle and fat adjacent to the eye and implicated in Graves Ophthalmopathy, are also in the cochlea. In clinical trials, 5 participants reported self-limited audiologic symptoms but there are no objective data in the literature. The aim of this report is to describe one of the first known cases of teprotumumab-induced irreversible sensorineural hearing loss. METHODS: Case report at a tertiary referral center. RESULTS: A 61 year old female with Graves oph

Bromate-induced ototoxicity.

For decades, it has been known that ingested potassium bromate and sodium bromate can induce hearing loss. Hearing loss onset, following high-dose ingestion, is generally rapid occurring within 4-16 h and of a severe to profound degree. Unlike the sensorineural hearing loss which is generally irreversible, bromate-induced tinnitus, which is less well-studied, may reportedly be permanent or temporary. It is not clear whether actual bromate-induced vestibulotoxicity occurs in clinical populations. The primary sites of lesion for bromate-induced ototoxicity appear to be in the cochlea. However, p

[Topically induced ototoxicity].

Topically induced ototoxicity caused by applying substances into the middle ear is discussed in detail. The numerous ototoxic cases described in the recent literature and the high aminoglycoside antibiotic concentrations contained in ear drops are stressed. Methods for reducing the aminoglycoside antibiotic ototoxicity of these ear drops are given. These include safe and effective concentrations of 30 or 60 mg/l Gentamicin, Sisomicin or Tobramycin and their application by ointment. Different chapters deal with the ototoxicity of other antibiotics and solutions, particularly with the risk in st
