Active middle ear implants-Enhanced efficiency of oval window stimulation with larger piston size.

INTRODUCTION: Direct piston-stimulation of the oval window with a middle ear implant, following a stapedotomy or stapedectomy, is one treatment option for patients with otosclerosis. Here, we experimentally investigated whether increasing the surface area of the actuated piston enhances transmission efficiency in oval window (OW) stimulation. METHODS: Fresh-frozen human cadaveric temporal bones (N = 14) were used to evaluate the output of different oval window couplers (OWCs) of A = 0.38 mm², 1.26 mm2, 3.06 mm2 by intracochlear pressure difference measurements between scala vestibuli (PSV) an

Congenital middle ear abnormalities with absence of the oval window: diagnosis, surgery, and audiometric outcomes.

OBJECTIVE: Congenital absence of the oval window (CAOW) is a rare condition in which the stapes footplate fails to develop, resulting in a significant conductive hearing loss in the affected ear. The purpose of this study was to describe the surgical management and outcomes of patients with CAOW undergoing the oval window drill-out (OWD) procedure. MATERIALS AND METHODS: A retrospective chart review of patients with CAOW between 1996 and 2011 was performed. Clinical data of patients who underwent OWD were collected. Seventy-nine patients (103 ears) were confirmed using exploratory tympanotomy

Enhanced oval window and blocked round window passages for middle-inner ear transportation of gadolinium in guinea pigs with a perforated round window membrane.

To elucidate the communication between the middle and inner ear, and the fluid dynamics of the inner ear with the perilymphatic fistula (PLF) of the round window membrane (RWM). The PLF of the RWM was created in nine guinea pigs. Gadolinium diethylenetriamine pentaacetic acid bismethylamide (Gd-DTPA-BMA) was delivered into the middle ear and followed in the inner ear using a 4.7 Tesla MRI. Pressure was delivered to the external ear canal of PLF ear in an attempt to enhance the inner ear uptake of Gd-DTPA-BMA. The immediate loading of Gd-DTPA-BMA in the scala tympani of the basal turn was ablat

Is oval window transport a royal gate for nanoparticle delivery to vestibule in the inner ear?

Drug delivery to the inner ear by nanomedicine strategies has emerged as an effective therapeutic approach for the management of inner ear diseases including hearing and balance disorders. It is well accepted that substance enters the perilymph from the middle ear through the round window membrane (RWM), but the passage through the oval window (OW) has long been neglected. Up to now, researchers still know little about the pathway via which nanoparticles (NPs) enter the inner ear or how they reach the inner ear following local applications. Herein, we engineered fluorescence traceable chitosan

Unusual middle-ear mischief: trans-tympanic trauma from a hair grip resulting in ossicular, facial nerve and oval window disruption.

A case of piercing of the tympanic membrane, resulting in unusual consequences, is described. This is the first reported case of the long process of a dislocated incus resulting in trauma to the horizontal portion of a dehiscent facial nerve. Simultaneous depression of the stapes footplate resulted in a perilymph leak, but with delayed presentation.

Efficient penetration of ceric ammonium nitrate oxidant-stabilized gamma-maghemite nanoparticles through the oval and round windows into the rat inner ear as demonstrated by MRI.

We aimed to evaluate the magnetic resonance imaging (MRI) contrast effect and delivery efficiency through the middle ear into the inner ear using novel super-paramagnetic maghemite (γ-Fe2 O3 ) nanoparticles (NPs) generated using ceric ammonium nitrate (CAN)-mediated oxidation of Fe3 O4 NPs (CAN-γ-Fe2 O3 NPs). The CAN-γ-Fe2O3 NPs, having hydrodynamic diameters of 50-60 nm and potentials of +55.2 mV, displayed super-paramagnetic behavior characterized by a saturation magnetization Ms of 75.2 emu/g NPs. The r1 and r2* relaxivity (curve slopes) values were 0.0015 and 189 mmol-1  s-1 , respectively

Recurrence of an Oval Window Rupture 25 Years After Traumatic Stapediovestibular Luxation Repair: A Case Report of a Novel Cartilage Sealing Technique.

Stapediovestibular luxations are often caused by trauma to the external ear canal. Sealing the oval window while leaving the stapes in the vestibule is a safer procedure than stapedectomy, considering the risk of additional inner ear damage, but it risks recurrence. We report a rare case of oval window rupture recurrence 25 years after initial surgery and introduce a new method using cartilage to seal the oval window. A 37-year-old woman experienced a recurrence of oval window rupture after nasal blowing. She had a history of left ear ossicular chain dislocations and oval window rupture caused

Recurrent Meningitis Following Previous Repair of Congenital Oval Window Defect.

A 19-year-old male patient with a history of congenital left oval window defect has had recurrent admissions to a children's hospital for bacterial meningitis since the age of 7. Investigations revealed a left-sided type 1 incomplete partition deformity of the left ear with a congenital oval window defect, leading to communication between the cranium and the middle ear. The patient underwent a left canal wall down mastoidectomy and oval window defect repair, remaining well for 3 years. He experienced two more episodes of bacterial meningitis as he got older, prompting repeated exploratory tymp

The leaking labyrinthine lesion resulting from direct force through the auditory canal: report of five cases.

The leaking labyrinthine lesion is treated by conservative methods or surgical procedures. With respect to the stapes, the surgical treatment is controversial. Five cases of middle ear injuries accompanying oval window rupture are reported herein. In each case, direct force through the auditory canal damaged not only the ossicular chain but also the oval window. Initial symptoms were sudden hearing loss with significant conductive disturbance and various degrees of unsteadiness. Spontaneous horizontal nystagmus directed toward the uninvolved ear was observed in each case. Tympanic cavities wer

The design of a lumped parameter model considering the stimulus path of round window.

BACKGROUND: Sound normally enters the ear canal, passes through the middle ear, and stimulates the cochlea through the oval window. Alternatively, the cochlea can be stimulated in a reverse manner, namely round window stimulation. The reverse stimulation is not well understood, partly because in classic lumped-parameter models the path of reverse drive during the round window stimulation is usually not considered. OBJECTIVE: The study goal is to gain a better understanding of the hearing mechanism during round window stimulation. METHODS: A piezo actuator was coupled to the oval and round wi

Congenital absence of the oval window: radiologic diagnosis and associated anomalies.

BACKGROUND AND PURPOSE: In most children with conductive hearing loss, acquired otitis media and/or middle ear effusion are ultimately diagnosed. Congenital conductive hearing loss is a rare condition; absence of the oval window is an unusual pathogenesis for this type of hearing impairment and can be associated with an anomalous horizontal facial nerve canal. Our goal was to describe the imaging features of congenital absence of the oval window, to determine the frequency with which anomalous development of the horizontal facial nerve canal occurs, and to review the developmental error respon

Location of the Tympanic Nerve Relative to the Round and Oval Windows.

BACKGROUND: The aim of the study was to measure the distance of the tympanic nerve to the oval window and round window niche in adult cadavers for evaluating its usability as an anatomical landmark during middle ear-related surgeries, including stapedotomy and cochleostomy, and for preventing its iatrogenic damage during surgical practices such as otosclerosis surgery and cochlear implantation. METHODS: The middle ears of 10 adult cadavers aged 74.70 ± 14.56 years were bilaterally dissected with the help of an endoscope and microscope to measure the distance of tympanic nerve to round window

[Bifenestration--a hearing-improving operation in severe ear malformations].

Bifenestration as an operation for hearing improvement is described in a case of congenital absence of the oval and round window. Besides the classical fenestration of the horizontal semicircular duct, an artificial round window was performed in the middle ear. Due to the acoustic resonance of the mastoid cavity, the resulting conductive loss in the higher frequencies was only 20-30 dB.
