Planned and progressive exercise, that is, exercise training, is essential for promoting skeletal muscle adaptation and improvements in athletic performance. However when the training demands exceed the ability to recover, it may lead to a state of persistent maladaptation. A prolonged imbalance between training or competition and recovery can cause extended periods of performance impairment, known as functional or non-functional overreaching, which may progress to overtraining syndrome (OTS). This condition can significantly impair athletic development and, in severe cases, cause athletes to

The phenomenon of reduced athletic performance following sustained, intense training (Overtraining Syndrome, and OTS) was first recognized more than 90 years ago. Although hundreds of scientific publications have focused on OTS, a definitive diagnosis, reliable biomarkers, and effective treatments remain unknown. The present review considers existing models of OTS, acknowledges the individualized and sport-specific nature of signs/symptoms, describes potential interacting predisposing factors, and proposes that OTS will be most effectively characterized and evaluated via the underlying complex

Overtraining syndrome (OTS) is characterised by persistent fatigue and performance decline, but it currently lacks specific diagnostic criteria and medical coding. Diagnosis is primarily based on a decline in maximal performance and a subjective feeling of 'staleness', after ruling out other medical conditions. This study aimed to identify physiological and self-reported predictors of OTS by comparing individuals diagnosed with OTS (n=110; 2008-2022) with a healthy control group (n=447; 2005-2023). Both groups consisted of athletes and recreational exercisers assessed at the Helsinki Sports an

Chronic obstructive pulmonary disease (COPD) is a condition that inevitably leads to airflow limitation. COPD is among the leading causes of increased mortality and morbidity worldwide. A non-invasive and non-pharmacological approach is rehabilitation training, where the patient follows an active program to stimulate the limb and respiratory muscles. Training involves a constant increase in workloads throughout the rehabilitation process. A fundamental concept absent from the literature is that of including training sessions with reduced loads and periods of "unloading" intensity within the re

Overtraining syndrome (OTS) is a significant medical condition observed predominantly in endurance athletes and characterised by a decline in performance and disruptions in multiple physiological functions. Given the increasing popularity of endurance training and the complex nature of OTS diagnosis, there is a compelling need for comprehensive case studies. The case series summarises one professional runner, one competitive runner and one recreational runner who were diagnosed with OTS. Key parameters observed include symptoms, diagnostic journey, recovery initiatives and return-to-sport outc

Overtraining syndrome is a condition resulting from excessive training load associated with inadequate recovery and poor sleep quality, leading to performance decrements and fatigue. Here we hypothesized that vitamin D (VitD) deficiency is a lead factor in the development of the overtraining syndrome. To test this hypothesis, two groups of 60-week-old C57BL/6 mice followed a 16-week excessive eccentric-based overtraining by excessive downhill running with or without dietary VitD depletion (EX and EX-D- groups). Two control groups were trained by uphill running at the same load with or without

Successful training not only must involve overload but also must avoid the combination of excessive overload plus inadequate recovery. Athletes can experience short-term performance decrement without severe psychological or lasting other negative symptoms. This functional overreaching will eventually lead to an improvement in performance after recovery. When athletes do not sufficiently respect the balance between training and recovery, nonfunctional overreaching (NFOR) can occur. The distinction between NFOR and overtraining syndrome (OTS) is very difficult and will depend on the clinical out

Excessive exercise combined with inadequate recovery time may trigger fatigue, performance impairment, and ultimately the overtraining syndrome. The intramyocellular mechanisms involved in the overtraining syndrome remain only partially known. Here, we combined multi-omics analyses from isogenic BXD mouse strains with a mouse model of overtraining and excessive exercise protocol in mice and humans to evaluate the molecular mechanism involved in the performance impairment induced by excessive exercise. We identified that BXD mouse strains with elevated levels of Parp1 gene expression in the ske

The modern-day athlete participating in elite sports is exposed to high training loads and increasingly saturated competition calendar. Emerging evidence indicates that inappropriate load management is a significant risk factor for acute illness and the overtraining syndrome. The IOC convened an expert group to review the scientific evidence for the relationship of load-including rapid changes in training and competition load, competition calendar congestion, psychological load and travel-and health outcomes in sport. This paper summarises the results linking load to risk of illness and overtr

Horses in racing and race training can develop high gamma-glutamyl transferase (GGT) syndrome (HGS), or increased GGT without overt evidence of primary liver disease. Current evidence points toward overtraining and a response to oxidative stress, but the etiology is incompletely understood. Evidence for an association of HGS with poor performance is mixed. Without a clear understanding of the etiology, the only evidence-based treatment option is rest or reduced training intensity. More research is needed to determine whether HGS is a physiologic adaptation, perhaps indicative of overtraining b

Background/Objectives: This study examined the efficacy of essential amino acid (EAA) supplementation on changes in behavior and hippocampal neurotrophin, dopaminergic and serotonergic markers to a volume overload stress resembling an overtraining syndrome. Methods: Thirty-two 3-month-old male C57Bl/6J mice were randomized into four groups: Resistance training (RT), resistance training with overtraining (RTO), resistance training with overtraining and EAA (RTOEAA), or control. Mice in RTOEAA received EAA supplementation (1.5 g·kg·day-1), while the other groups received a sham treatment. A 5-we

Children and adolescents who participate in intense sports training may face physical and psychologic stresses. The pediatric health care provider can play an important role in monitoring an athlete's preparation by obtaining a proper sports history, assessing sleep hygiene, discussing nutrition and hydration guidelines, and evaluating physiologic causes of fatigue. Educating parents and athletes on the potential risks of high-intensity training, inadequate rest and sleep, and a poor diet may improve the athlete's performance and prevent symptoms of overtraining syndrome. Overtraining syndrome (OTS) poses a critical challenge in youth soccer, particularly during periods of rapid physiological maturation combined with high training demands. This study aimed to develop and validate a multidimensional prediction model for overtraining risk in youth soccer players by integrating physiological, psychological, and performance parameters through advanced machine learning. A longitudinal study tracked 120 male youth players (aged 12-18) from six elite South Korean academies over one competitive season (August 2023-May 2024). Data included bi-weekly blood sampling (test

Chronic musculoskeletal pain is becoming increasingly common in young athletes. When these athletes do not respond well to standard treatments, for example physical theraphy and anti-inflammatories, other diagnoses must be considered, such as reflex sympathetic dystrophy, fibromyalgia, and/or overtraining syndrome.

Ischiofemoral impingement syndrome (IFIS) is an unusual cause of hip pain. Certain strenuous sports can cause this impingement, such as dancing. We report the case of an 11-year-old ballet dancer with bilateral IFIS secondary to overtraining and hypertrophy of the quadratus femoris. This syndrome is frequently evoked in the field of orthopedics following hip replacement. Bilateral IFIS is rare and should be sought in the context of an intense sport such as dance when any other cause has been eliminated.

Iron is an essential trace element that participates in multiple physiological processes, including oxygen transport, electron transfer, DNA synthesis, and red blood cell production. Iron loss is particularly severe among athletes, so maintaining iron homeostasis is crucial for sports nutrition and health. Excess iron, iron deficiency, and ferroptosis can lead to muscle disorders and health issues, including sarcopenia, muscular atrophy, myocardial fibrosis, skeletal muscle injury, cardiovascular disease, and metabolic disorders. Maintaining iron homeostasis within physiological limits is esse

Physical activity provides numerous health benefits but may also be associated with biological alterations related to training intensity, nutritional deficiencies, overtraining, or doping practices. This article reviews the main biological parameters used in athlete monitoring and their relevance for assessing health status, adaptation to exercise, and performance. These parameters encompass hematological, biochemical, and endocrine markers. The complete blood count enables the evaluation of blood cell adaptations to exercise and the detection of anemia or abnormalities in erythropoiesis. Bioc

INTRODUCTION: Moderate exercise induces beneficial adaptive responses in the body, whereas sustained high-intensity exercise without adequate recovery leads to overtraining syndrome (OTS), a pathological condition associated with multiple organ damage, especially liver injury. Currently, safe and effective nutritional interventions for OTS-induced liver injury remain limited. This study aimed to investigate the protective effect of ginsenoside Rb1 (Gs-Rb1), the primary bioactive component of ginseng, against OTS-induced liver injury and to elucidate its underlying molecular mechanism.
