A technique for obturating palatal fistulas.

Use of nontoxic self-adhesive material is described to obturate residual hard palate fistulas. This material may be used during perceptual and aerodynamic speech assessments to evaluate speech distortion and the relative contributions of velopharyngeal port and fistula to nasal air leakage during speech. It may also be used as an interim obturator prior to surgical repair of a fistula.

The nance obturator, a new fixed obturator for patients with cleft palate and fistula.

In patients with fistulas that impair function (e.g., feeding, resonance, intelligibility), obturators are used to improve feeding and reduce nasal air emission by occluding the abnormal opening between the oral and nasal cavities. This report describes a novel method for occluding an anterior palatal fistula in patients with cleft palates. The new design for a fixed obturator is based on the Nance appliance, which was originally used as a space maintainer, but has been redesigned for closing an anterior palatal fistula in a patient with cleft lip and palate. The Nance obturator may be used wh

Obturation of the partial soft palate defect.

A partially resected soft palate represents a more significant challenge for effective prosthodontic obturation than for complete soft palate resections. In fact, for some patients it may not be possible to prosthetically rehabilitate with partial soft palate resections, resulting in velopharyngeal dysfunction, which could include hypernasal voice quality and nasal regurgitation of food and liquids. This article presents a technique for recording the contours of a partial soft-palate defect for prosthetic obturation.

Cheiloplasty associated to a palatal obturator in a preterm infant with a cleft lip and palate.

INTRODUCTION: Preterm infants are commonly treated by non-invasive ventilation (NIV) and nasal continuous positive airway pressure (CPAP), which are associated with fewer complications than nasotracheal intubation. In preterm infants with a cleft lip and palate, this method of respiratory management is difficult or impossible to perform because of air leakage through the cleft defect. CASE: We report a male infant who was born at 29 weeks of gestation with a left complete unilateral cleft lip and palate. Birth weight was 900 grammes. Because of the cleft, we have any problems to ventile this

Indirect retainers in soft palate obturator design.

Definitive framework design for the maxillofacial patient with complete maxillary dentition who has had a soft palate resection requires the use of direct and indirect retainers. It is possible to retain these prostheses, however, with only two posterior clasps and indirect retainers without clasps. This design offers an esthetic, retentive, and functional restoration while preserving existing structures. Four clinical applications are described.

Polydimethylsiloxane button obturator for hard palate clefts.

A polydimethylsiloxane obturator for hard palate clefts is described. Because it is soft, the obturator uses anatomic undercuts of the nasal cavity for maximum retention and stability. The obturator covers only a small part of the palate. The material is nontoxic and noncarcinogenic.

Upper lip prosthesis retained to a palatal obturator.

In cases of total or partial maxillectomies, the prosthetic rehabilitation is an effective alternative to minimize the sequelae left by surgical resection. The present study reports a clinical case of a 52-year-old patient who underwent partial maxillectomy, with upper lip involvement. The oronasal communication, resultant from surgical resection, did not allow the patient to return to her normal social life. Besides, the upper lip partial resection damaged her face's aesthetics. The proposed treatment was the confection of an upper lip prosthesis retained by a palatal obturator. The prosthesi

Cleft palate obturation with Brånemark protocol implant-supported fixed denture and removable obturator.

A 41-year-old man with cleft palate presented with a wide dehiscence and missing teeth. Six implants had been placed for fabrication of an overdenture, which was unsatisfactory. A bar was waxed and cast for connection to the implants; precision attachments were placed laterally for retention. A fixed partial denture was fabricated, and milled crowns were fabricated at the molar region to provide a guiding plane for insertion of a removable palatal obturator. Good swallowing and speech outcomes were achieved. This technique provided functional and esthetic benefits, enhanced oral hygiene, and i

Management of an Acquired Palatal Defect With a 3-Dimensional Printed Palatal Obturator in a Dog.

Large defects in the hard palate can be difficult to treat surgically, as dehiscence is common. These defects may instead be managed with a palatal obturator, which can serve to separate the oral and nasal cavities. In this report, a 7-year-old, mixed breed dog was treated with a palatal obturator, after presenting with an acquired palatal defect following treatment of a giant cell tumor of bone in the hard palate. The palatal obturator was designed and manufactured using computer-aided design and manufacturing (CAD/CAM), and 3-dimensional printing. The obturator, anchored to 4 maxillary teeth

Obturator prostheses following palatal resection: clinical cases.

Malignant tumours of the upper gum and hard palate account for 1-5% of malignant neoplasms of the oral cavity; two thirds of the lesions which involve these areas are squamous cell carcinomas. Most of these carcinomas are diagnosed late, when they invade the underlying bone. The procedures of choice for removal are: alveolectomy, palatectomy, maxillectomy, which may be total or partial. Surgical reconstruction of the defect may be carried out using a wide range of microvascularized flaps: osteomuscolocutaneous of the internal iliac crest, an osteocutaneous flap of the fibula or scapula, fascia

Successful use of a feeding obturator for an infant with a cleft palate.

The report of case presents the postnatal and early infancy period for a child born with a cleft of the soft palate. The infant was admitted to the hospital several times for failure to thrive as a result of feeding difficulties. The feeding problems resulted from both the cleft and a lack of consistent and concerted effort by the family to nourish the infant. When the child was 3 months old, a feeding obturator was constructed that resulted in improvement in the child's weight gain. The feeding problems of an infant with a cleft palate and the pressures that these problems present to the pare

Impact of obturation of palatal fistulae on speech quality and aerodynamic parameters in patients with cleft palates.

OBJECTIVES: The aim of this prospective study was to measure nasal and oral airflow during speech, before and after obturation. PATIENTS AND METHODS: Included were children aged 3-18 years with nonsyndromic clefts and palatal fistulae. The corpus used was: syllable /pi/; a sentence containing stop consonants and a nasal phoneme; and the description of a picture of a scene. Analysis criteria were: percentage of nasality; value of average flow for the explosion; perceived nasality and intelligibility; and tolerance of the proposed device. RESULTS: Only 5 children were included due to the obser

Oral Rehabilitation With Palatal Obturator After Treatment of Complete Bilateral Cleft Lip and Palate.

Cleft lip and palate require reparative surgeries, and even after these procedures, many patients continue to experience speech difficulties, especially due to velopharyngeal dysfunction, which may necessitate the use of prosthetic devices to complement rehabilitation. The aim of this case report is to describe the resolution of a case of velopharyngeal dysfunction following multiple corrective surgeries for a complete bilateral cleft lip and palate. A 36-year-old female patient sought care from the extension project "Prosthetic rehabilitation of patients with tissue loss in the head and neck

Early Intervention With Obturators: A Case Series of Infants With Cleft Palate.

A cleft palate is a congenital anomaly that can significantly impact an infant's ability to feed and grow normally. This article presents a case series of three infants diagnosed with a congenital cleft palate, who were fitted with a custom obturator for feeding purposes. The infants exhibited difficulty with suction and oral feeding due to the cleft, which led to inadequate nutrition and increased risk of aspiration. To address this issue, a removable obturator was fabricated to temporarily occlude the palatal gap and improve the efficiency of sucking after taking consent for the same from th

A New Membrane Obturator Prosthesis Concept for Soft Palate Defects.

When soft palate defects lead to palatal insufficiency, the patient's quality of life is affected by difficulties swallowing, hypernasality, and poor intelligibility of speech. If immediate surgical reconstruction is not an option, the patient may benefit from the placement of a rigid obturator prosthesis. Unfortunately, the residual muscle stumps are often unable to adequately move this stiff and inert obturator to properly restore the velopharyngeal valve function. In the present case history report, a new membrane obturator concept is described: Using a dental dam to compensate for the soft
