Olanzapine: palliative medicine update.

Olanzapine is an atypical antipsychotic agent of the thienobenzodiazepine class. Olanzapine blocks multiple neurotransmitter receptors, including dopaminergic (D(1), D(2), D(3), and D(4)), serotonergic (5-hydroxytryptamine 2A [5-HT(2A)], 5-HT(2C), 5-HT(3), and 5-HT(6)), adrenergic (α(1)), histaminic (H(1)), and muscarinic (M(1), M(2), M(3), and M(4)) receptors. Olanzapine has a high affinity for the 5HT(2A) receptor, which is up to 5 times greater than the dopamine receptor, resulting in less propensity to the development of extrapyramidal side effects. The affinity of olanzapine for multiple

Palliative care in cardiovascular medicine.

Cardiovascular disease remains the leading global cause of morbidity and mortality. Although advances in prevention, diagnostics, and disease-modifying therapies have prolonged survival, many individuals now live longer with high symptom burden, functional decline, and complex decisional needs. Palliative care (PC) for adults with advanced cardiovascular disease can improve quality of life, support caregivers, and align treatments with patient values and goals. Core elements include symptom management, effective communication, shared decision-making, advance care planning, and integration of p

[Cancer pain in palliative medicine].

During the course of cancer progression up to 90% of the patients suffer from pain of nociceptive, neuropathic or mixed nociceptive/neuropathic origin. Psychological, social or existential factors may additionally affect the intensity of pain (concept of "total pain"). The WHO "analgesic ladder" provides a large variety of effective drugs that can be used according to the specific pain type. Parenteral or peridural opioid therapy as well as neurodestructive methods can effectively support the analgesic treatment in selected cases.

Pulmonary medicine and palliative care.

Gynaecological malignancies affect the respiratory system both directly and indirectly. Malignant pleural effusion is a poor prognostic factor: management options include repeated thoracentesis, chemical pleurodesis, symptomatic relief of dyspnoea with oxygen and morphine, and external drainage. Parenchymal metastases are typically multifocal and respond to chemotherapy, with a limited role for pulmonary metastatectomy. Pulmonary tumour embolism is frequently associated with lymphangitic carcinomatosis, and is most common in choriocarcinoma. Thromboembolic disease, associated with the hypercoa

Anticholinergics in palliative medicine: an update.

Anticholinergics, or antimuscarinic drugs, are drugs that competitively inhibit the action of acetylcholine at muscarinic receptors, leading to a blockade of the actions of the parasympathetic nervous system at sites where overactivity can lead to increased symptom burden. Successful blockade of the parasympathetic nervous system ultimately leads to decreased production of secretions in the salivary, bronchial, and gastrointestinal tracts. These effects are often used for several symptoms that originate due to parasympathetic nervous system overactivity, such as the "death rattle" and malignan

Parenteral nutrition complications in palliative medicine.

Parenteral nutrition in palliative care is contentious, and decisions on starting or continuing its treatment in palliative patients centre on an individual's preference, balanced with quality of life. This case report describes the unusual onset of pain and agitation secondary to fluid retention, in a patient with metastatic pseudomyxoma peritonei, established on 2.5 L/day of parenteral nutrition. Immediate volume reduction of the parenteral nutrition to 1 L/day successfully reversed the patient's symptoms. To our knowledge, this is the first case of parenteral nutrition inducing pain and agi

[Essential psychological care in palliative medicine].

Because of progress in cancer treatment, the development of cancer is not necessarily a death sentence. On the other hand, approximately half of patients actually die from cancer even today, and many cancer patients require care for psychological distress. From the viewpoint of psychiatric diagnosis, the most common psychiatric conditions experienced by cancer patients are adjustment disorders and/or major depression. From the viewpoint of psychological status, however, cancer patients without psychiatric disorders may still suffer from fears of recurrence during the survival period and psycho

Palliative sedation determinants: systematic review and meta-analysis in palliative medicine.

BACKGROUND: The utilisation of palliative sedation is often favoured by patients approaching end of life due to the presence of multiple difficult-to-manage symptoms during the terminal stage. This study aimed to identify the determinants of the use of palliative sedation. METHODS: To identify pertinent observational studies, a comprehensive search was performed in PubMed, Embase, Cochrane Library, and PsycINFO databases from their inception until March 2022. The methodological quality of the chosen prospective and retrospective cohort studies was assessed using the Newcastle Ottawa Scale, wh

One health considerations in veterinary palliative medicine.

Veterinary palliative medicine (VPM) is an emerging discipline within clinical veterinary science that addresses the comfort and quality of life of animals affected by chronic or terminal illness. The implications of VPM extend beyond the individual patient. From a One Health perspective, end-of-life veterinary practice encompasses animal welfare, caregiver well-being, and environmental sustainability. This paper aims to present a multidisciplinary synthesis that connects these dimensions. It highlights the health benefits and the emotional costs of the human-animal bond; the psychological ris

[Parenteral and enteral nutrition in palliative medicine].

Artificial nutritional support does not alter the natural course in patients with malignant disease. The outcome of these patients is mainly determined by the type and stage of the underlying tumor. Progress of the underlying disease is often paralleled by malnutrition which in turn facilitates complications and may reduce survival and quality of life. Nutritional support can be applied to maintain body weight, immune function and quality of life. Enteral nutrition can be applied with a functioning gastrointestinal tract and has been proven to be superior compared to parenteral nutrition. Main

Phenol Neurolysis in Pain and Palliative Medicine.

BACKGROUND: Pain is a common symptom in palliative care and affects patients' quality of life considerably. Standard analgesics are sometimes insufficient and are associated with substantial side effects. Neurolysis, the targeted destruction of nerves using thermal or chemical agents, offers an additional option for managing localized pain in palliative care. Phenol, a widely available chemical neurolytic agent known for its painless injection and hyperbaric properties when dissolved in glycerin, can effectively relieve refractory cancer pain. METHODS: A literature review was conducted on the

Palliative medicine and end-of-life care.

Palliative care is an approach to the care of patients, affected by serious illness, and their families that aims to reduce suffering through the management of medical symptoms, psychosocial issues, spiritual well-being, and setting goals of care. Patients and families affected by a neurodegenerative illness have significant palliative care needs beginning at the time of diagnosis and extending through end-of-life care and bereavement. We advocate an approach to addressing these needs where the patient's primary care provider or neurologist plays a central role. Key skills in providing effecti

Kampo medicine for palliative care in Japan.

Kampo medicines are currently manufactured under strict quality controls. The Ministry of Health, Labour and Welfare of Japan has approved 148 Kampo formulas. There is increasing evidence for the efficacy of Kampo medicines, and some are used clinically for palliative care in Japan. The specific aim of this review is to evaluate the clinical use of Kampo medicines in palliative care in the treatment of cancer. The conclusions are as follows: Juzentaihoto inhibits the progression of liver tumors in a dose-dependent manner and contributes to long-term survival. Hochuekkito has clinical effects o

[Palliative medicine and integrative medicine as complementary approaches in uro-oncological care].

BACKGROUND: Uro-oncological diseases have negative long-term consequences for physical, functional, psychological, and socioeconomic well-being. A common focus of palliative and integrative medicine is improving quality of life by alleviating symptoms. OBJECTIVES: What treatment options are available for frequently occurring symptoms in uro-oncology patients? METHODS: Based on current literature and international guideline recommendations, an overview of palliative and integrative medical therapies and their significance in the management of advanced uro-oncological tumors is given. Subcutaneous antibiotics in palliative medicine: Retrospective chart review.

OBJECTIVES: To investigate the use and safety of subcutaneous (SC) antibiotics for infection and symptom control in the palliative setting. METHODS: We conducted a retrospective chart review of the use of SC antibiotics since it was introduced to our palliative care service 7 years ago. All records were examined to ascertain therapeutic aim and tolerability. RESULTS: SC antibiotics were administered to 87 patients (79% had cancer) on 112 occasions (for infection, 105/112, or symptom control, 7/112) in the inpatient hospice or home. 85% wanted to avoid hospitalisation and requested active tre

Unwalling the clinic: Redefining medicine through palliative care.

This reflective essay explores my experience as a nonmedical volunteer at a palliative care centre, in Elamkulam Panchayat, in the Malappuram district of Kerala for two weeks in October 2023 as a master's student of Medical Humanities. It examines how palliative care functions in a community setting, addressing the needs of terminally ill patients. Through direct observations and interactions, I reflect on how the decentralised, interdisciplinary and flexible nature of palliative care service transcends some limitations of modern clinical medicine, offering a holistic approach that embraces in

Rational use of sublingual opioids in palliative medicine.

The sublingual administration of opioid analgesics has been a mainstay in the pain management of homebound dying hospice patients who are no longer able to swallow. It is also a potentially useful route of administration in other situations in which the oral route is not available and other routes are impractical or inappropriate. Potential advantages of the sublingual route include rapid analgesic onset and avoidance of hepatic first-pass metabolism. Pharmacokinetic and pharmacodynamic studies have yielded widely disparate data on sublingual morphine. Other opioids have been less studied. Ava

Hospice and palliative medicine: new subspecialty, new opportunities.

Palliative care is the physical, psychological, social, and spiritual care provided to patients from diagnosis to death or resolution of a life-threatening illness. Hospice care is a comprehensive program of care that is appropriate when patients with chronic, progressive, and eventually fatal illness are determined to have a prognosis of 6 months or fewer. Hospice and palliative medicine has now been recognized by the American Board of Medical Subspecialties as a field with a unique body of knowledge and practice. With 9 other specialty boards, the American Board of Emergency Medicine has cos

Drug infusors in palliative medicine: a Swedish inquiry.

The use of drug infusors is common in palliative care. Knowledge about the drugs being used and the handling of drug mixtures in insufficient and poorly documented. To clarify this practice, a questionnaire was sent to all departments of pain/anesthesiology and oncology, and to all home-care teams and palliative care units/hospices in Sweden (N = 156). The questions concerned specific qualities of the drug infusors and the different drugs and drug mixtures used by subcutaneous (s.c.) and intravenous (i.v.) administration. A total of 110 (70%) of the questionnaires were returned. A majority of
